본 연구의 목적은 아동중심 놀이언어치료 프로그램을 개발하고 언어장애아동을 대상으로 실시하여 프로그램의 효과성을 검증하고 동시에 치료사중심 언어치료 프로그램과의 비교를 통하여 그 차이를 검증하는 것이다. 구체적으로 본 연구는 아동중심 놀이언어치료 프로그램을 통해 언어장애아동의 어휘량을 증가시키고 의사소통 의도를 증진시키며 심리적 능동성을 향상시키고자 한다. 나아가 아동중심 놀이언어치료 프로그램이 언어장애아동 부모의 상호작용 행동 변화와 아동의 상호작용 행동에 어떠한 효과가 있는지 알아보고자 하였다. 아동중심 놀이언어치료 프로그램을 개발하기 위하여 아동중심 언어치료와 아동중심 놀이치료에 대한 문헌고찰을 하였으며 아동중심 언어치료의 현장연구를 통해 한계성을 검토하고 이를 보완한 예비 프로그램을 구성하여 실시하였으며 이를 토대로 전문가의 협의를 통하여 최종적으로 프로그램을 개발하였다. 연구 대상은 생활연령 만 3세에서 만 5세 사이의 언어장애아동으로 아동중심 놀이언어치료 실시 집단인 실험집단 아동 6명, 치료사중심 언어치료 실시 집단인 비교집단 아동 6명, 통제집단 아동 6명과 이들 각 집단의 어머니를 대상으로 하였다. 아동중심 놀이언어치료 프로그램과 치료사중심 언어치료 프로그램은 주 2회씩 총 16회기를 각 아동마다 1 : 1의 개별 프로그램으로 실시하였으며 회기시간은 아동은 40분, 부모상담 20분으로 총 60분을 실시하였다. 본 연구에서 수집된 자료 분석은 SPSS 18.0 통계프로그램을 이용하였으며 분석방법은 다음과 같다. 첫째, 연구대상자의 사회 인구학적 특성을 알아보기 위하여 빈도분석을 실시하였다. 둘째, 아동중심 놀이언어치료 프로그램 실험집단, 치료사중심 언어치료집단 및 통제집단 간의 사전검사 결과 차이를 알아보기 위해 비모수 검정인 Kruskal-Wallis Test를 실시하였다. 셋째, 아동중심 놀이언어치료 프로그램 실험집단과 치료사중심 언어치료집단, 통제집단 내 각 척도별 사전-사후 검사 결과 차이를 알아보기 위해 비모수 검정인 Wilcoxon signed rank test를 실시하였다. 넷째, 아동중심 놀이언어치료 프로그램 실험집단과 치료사중심 언어치료집단, 통제집단 간 각 척도별 사후검사 결과 차이를 알아보기 위해 비모수 검정인 Kruskal-Wallis Test를 실시하였으며, Bonfferoni correction을 통해 사후검정을 실시하였다. 다섯째, 어휘량과 의사소통 의도를 분석하기 위하여 자발화 수집을 통한 자발화 분석을 실시하였다. 여섯째, 아동중심 놀이언어치료 프로그램의 적용과 회기 과정에서 나타나는 변화를 살펴보기 위하여 초기, 중기, 후기에 따른 비디오 내용 분석을 실시하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 아동중심 놀이언어치료 프로그램은 기본 원리, 놀이 환경, 부모 상담, 치료사의 역할, 기법으로 구성된 언어장애 중재 프로그램으로 개발되었다. 둘째, 프로그램 실시 후 아동중심 놀이언어치료집단과 치료사중심 언어치료집단 모두에서 어휘량과 의사소통 의도가 증가하였다. 그러나, 아동중심 놀이언어치료집단에서의 어휘양의 증가폭이 더 컸으며 의사소통 의도에서 아동중심 놀이언어치료집단은 주관적 진술의 증가가 높게 나타났고 치료사중심 언어치료집단은 반응의 증가가 높게 나타났다. 셋째, 프로그램 실시 후 아동중심 놀이언어치료집단의 심리적 능동성이 향상되었으나 치료사중심 언어치료집단에서는 유의한 변화가 없었다. 넷째, 프로그램 실시 후 아동중심 놀이언어치료 집단에서 부모의 상호작용 행동에서 지지적이고 반응적이고 긍정적인 변화가 나타났으며 아동의 상호작용 행동에서도 주도적이고 적극적인 태도 변화가 나타났으나 치료사중심 언어치료집단에서는 유의한 변화가 없었다. 따라서 본 연구는 아동중심 놀이언어치료 프로그램을 개발하고 언어장애아동을 대상으로 실시하여 어휘량과 의사소통 의도, 심리적 능동성, 부모와 아동의 상호작용 행동 변화에 모두 효과가 있음이 입증되었으며 치료자중심 언어치료와의 차이를 검증하였다. 이상의 결과를 토대로 본 연구의 의의를 살펴보면 다음과 같다. 첫째, 본 연구는 놀이치료와 언어치료의 다 영역간의 통합된 중재 프로그램으로써 언어치료 현장에서 사용할 수 있는 새로운 언어치료 중재 전략의 이론적 틀을 제시했다는데 의의가 있다. 둘째, 본 연구는 언어장애아동의 언어적, 심리적 어려움을 동시에 다루어 주는 중재 접근법을 개발함으로써 언어장애아동에게 심리적 문제를 감소시켜주고 동시에 언어능력을 향상시켜주는 새로운 중재 프로그램을 제시했다는데 의의가 있다. 셋째, 본 연구는 아동중심에 대한 구체적인 방법과 예시를 제시하여 언어치료사들이 현장에서 사용하기 용이하도록 하였다. 넷째, 언어치료 현장에서 아동과 부모를 동시에 프로그램에 참여시켜 아동의 부모와 아동 모두에게 도움을 줄 수 있는 프로그램을 제시하였다는데 의의가 있다.

The objectives of this study were to develop a child‐centered play intervention program and to test its effectiveness by applying the program to language‐impaired children and, at the same time, to compare it with a clinician‐directed intervention program. Specifically, this study aimed to increase language‐impaired children’s vocabulary size, to reinforce their communication intent, and to enhance their psychological activity through the child‐centered play intervention program. Furthermore, it examined the effects of the child‐centered play intervention program on language‐impaired children’s interaction behavior as well as on their parents’ interaction behavior. In order to develop a child‐centered play intervention program, this study reviewed literature on child‐centered intervention and child‐centered play therapy, and discussed the limitations of child‐centered intervention through a field survey. Based on the results, a preliminary program was designed and applied, and through discussions with experts on the results, the final program was developed. The subjects of this study were 18 language‐impaired children at 3‐5 of chronological age and their mothers, and the children were divided into an experimental group (n=6) that participated in child‐centered play intervention, a comparative group (n=6) that participated in clinician‐directed intervention, and a control group (n=6). The child‐centered play intervention program and the clinician‐directed intervention program were applied twice a week and a total of 16 sessions. Each child attended an 1:1 individual session, and each session took 40 minutes for the child, and 20 minutes for parent counseling, so a total of 60 minutes. Data collected in this study were analyzed through SPSS 18.0, and analysis methods used were as follows. First, frequency analysis was used to examine the subjects’ demographic characteristics. Second, non‐parametric Kruskal‐Wallis test was conducted in order to compare the results of the pretest among the child‐centered play intervention group, the clinician‐directed intervention group, and the control group. Third, non‐parametric Wilcoxon signed rank test was used to examine difference between the pretest and posttest results for each scale within each of the child‐centered play intervention group, the clinician‐directed intervention group, and the control group. Fourth, non‐parametric Kruskal‐Wallis Test was used to compare the results of the posttest for each scale among the child‐centered play intervention group, the clinician‐directed speech therapy group, and the control group, and a post‐hoc test was made through Bonfferoni correction. Fifth, spontaneous speech was collected and analyzed for the analysis of vocabulary size and communication intent. Sixth, changes resulting from the application of the child‐centered play intervention program were examined by analyzing the videos of the early, middle and later sessions of the program. The results of this study are summarized as follows. First, the child‐centered play intervention program was developed as a language impairment intervention program consisting of basic principles, play environment, parent counseling, clinician’s roles, and techniques. Second, after the application of the program, both the child‐centered play intervention group and the clinician‐directed intervention group showed increased vocabulary size and communication intent. However, the child‐centered play intervention group showed a larger increase in vocabulary size. With regard to communication intent, moreover, while the child‐centered play intervention group showed a large increase in subjective statements, the clinician‐directed intervention group showed a large increase in responses. Third, after the application of the program, the child‐ centered play intervention group showed improvement in psychological activity, but the clinician‐directed intervention group did not show any significant change. Fourth, after the program, the parents of the child‐centered play intervention group showed supportive, responsive, and positive changes in their interaction behavior, and the children also showed initiative and active attitudes in their interaction behavior, but the clinician‐directed intervention group did not show any significant change. This study developed a child‐centered play intervention program and applied it to language‐impaired children, and proved that it was effective for the children’s vocabulary size, communication intent, and psychological activity and for their and their parents’ interaction behavior and that its effects were different from those of clinician‐directed intervention. Given these results, this study has meanings as follows. First, this study is meaningful in that it developed an intervention program integrating both play therapy and speech therapy and provided a theoretical frame for new intervention strategies applicable in the field of speech therapy. Second, this study is meaningful in that it developed an intervention approach handling both language‐impaired children’s linguistic and psychological difficulties at the same time and, by doing so, suggested a new intervention program mitigating language‐impaired children’s psychological problems and, at the same time, enhancing their language abilities. Third, this study presented specific child‐centered methods and examples for speech therapists’ easy application in the field. Fourth, this study is meaningful in that it suggested a program that involves both children and their parents and help both of them.
