This study has examined the problems and development directions of Regional medical centers through the concept have by the public nature of healthcare, discussions to date and the closure of Jinju Medical Center. This study was based on the literature study. The concept on the public nature of healthcare in our country is divided into various perspectives such as medical itself being viewed as public nature, perspectives according to the formation type of medical institutions, perspectives related to public funding and perspectives seen as private interests of private medical institutions and confronting relationships, etc. Currently our country has no real and clear concepts and standards of what the public nature in medical is and what the role of the public hospital is. Therefore in the future, a discussion on the public nature of healthcare needs to be further progressed through many studies. Although public healthcare in our country is also being made in private hospitals, most public hospitals are performing functions such as essential medical field, immunizations, emergency care, vulnerable care and national healthcare business, etc, which are insufficiently supplied or cannot be handled by the private hospitals. Among the public hospitals that perform such role, typical Regional medical centers are being reduced of its functions due to financial difficulties. Typical problems of Regional medical centers can be summarized as follows. First, the development of public functions that meet the conditions of Regional medical centers are insufficient. Second, medical profits are less with high management cost leading to ongoing deficit and increase in debt. Third, facilities and equipments are aging and the recruitment of excellent medical staffs is becoming difficult. Lastly, an interest of government on the local medical centers are lacking such as absence of responsible management, labor disputes and insufficient financial support, etc. As seen earlier, solutions on such problems are being proposed through various related institutions and experts along with the Department of Health and Human Services.
Among them, the role of the government is the most important. The constitution stipulates that 'all citizens have the right to a healthy and comfortable environment, and all citizens shall be protected by the state in respect to the healthcare.' As such, the government is obligated to protect and fulfill the health of all citizens. The government must actively invest and try to strengthen the functions and improve the quality of Regional medical centers to avoid the problems of accessibility on the medical service due to economic and social barrier through the recognition of changes such as aging and the increase in chronic diseases. And according to Regional characteristics, the role of Regional medical centers must be clear given and the assessment on its performance should be made. An appropriate compensation should be made in accordance with the degree of performance for the deficit on the public role. Going forward, the Regional medical centers should play the role of reasonable public medical provider that compensate the market failure such as excessive care and medical privatization to provide comprehensive services for the health of all citizens such as socially beneficial health promotions, disease management, emergency, medical rehabilitation and recuperation, etc.
