본 연구는 수술실에서 이루어지고 있는 수술간호행위 중 가장 중요한 수술 중 간호행위(intraoperative nursing activity)를 규명하고, 수술환자분류에 따른 간호원가를 산정하여, 추후 적정 수술간호인력 및 수술간호관리료 산정에 기초자료를 제공하기 위해 시행되었다.
본 연구는 1단계 수술환자 분류도구 선정 및 타당도 검증, 2단계 수술 중 간호행위(intraoperative nursing activity) 규명 및 타당도 검증, 3단계 수술 중 간호행위(intraoperative nursing activity) 수행 빈도 확인, 4단계 수술환자분류도구를 이용하여 수술환자를 분류하였으며, 5단계 수술환자분류에 따른 간호원가 산정의 총 5단계의 절차를 통해 이루어졌다.
연구도구는 Hart(1985)의 Acuity System, AORN(Association of periOperative Registered Nurses 1993)의 Intraoperative Acuity Categories를 사용하여 수술환자를 분류하였으며, 수술 중 간호행위는 하루미 외(2014)의 수술환자 간호행위를 사용하여 분류하였다. 자료 수집은 2015년 3월 1일부터 4월 10일까지 이루어졌다.

자료 분석은 SPSS 19.0 프로그램을 이용하여 수술환자분류도구 및 수술 중 간호행위 타당도는 내용타당도(CVI: Content Validity Index), 영역별 수술 중 간호행위 규명 및 수술환자분류는 기술통계, 의료기관유형에 따른 수술환자분류의 비교는Chi-square test로 분석하였다. 수술환자분류별 간호원가는 다음과 같은 공식으로 산정하였다.

본 연구의 결과는 다음과 같다.
1) 문헌고찰과 현재 사용하고 있는 수술환자분류도구 고찰을 통해 Hart(1985)의 Acuity Systeme과 AORN(1993)의 Intraoperative Acuity Categories분류도구를 선정하였으며, 선정된 수술환자분류도구에 대한 타당도를 수술관련 전문가 집단 11명에게 조사한 결과, 모든 항목의 내용타당도(CVI: Content Validity Index)는 0.8 이상으로 나타났다.
2) 수술 중 간호행위는 4가지 영역, 58개 행위로 분류하였다. 각 영역별 수술간호행위는 수술준비(Suite Preparation) 영역은 15개의 수술간호행위, 환자준비(Patient Preparation) 영역은 17개의 수술간호행위, 수술과정(Surgical Procedure) 영역은 14개의 수술간호행위, 수술 후 정리(Suite Clean Up) 영역은 12개의 수술간호행위로 분류되었다. 각 4가지 영역별 수술 중 간호행위에 대한 타당도를 수술관련 전문가 집단 11명에게 조사한 결과 모든 항목의 내용타당도(CVI: Content Validity Index)는 0.8 이상으로 나타났다.
3) 수술 중 간호행위 수행 빈도를 확인한 결과 수술준비영역은 수술스케쥴 확인하기가 100%로 가장 높았으며, 독성물질 수술실 안전관리하기는 10.8%로 가장 낮았다. 환자준비 영역은 수술환자와 수술부위 확인하기, 기구상 세팅하기, 환자확인하기가 100%였으며, 유치도뇨관 삽입하기는 11.7%로 가장 낮았다. 수술과정 영역은 수술보조하기, 수술팀, 가운, 장갑착용하기는 100%였으며, 추가 수술장비 조달하기는 43.3%로 가장 낮았다. 수술 후 정리 영역은 수술방 내 정리정돈하기, 수술 간호기록지 작성하기는 100%였으며, 감염성 수술 전 후, 수술방, 수술 기구 등 처리하기가 36.7%로 낮게 나타났다.
4) Hart(1985)의 Acuity System을 사용하여 수술환자를 분류한 결과, Acuity SystemⅠ 0.3%, Acuity SystemⅡ 40.6%, Acuity SystemⅢ 55.8%, Acuity SystemⅣ 3.4%로 분류되어 2군과 3군이 96.4%를 차지하였다. AORN(1993)의 Intraoperative Acuity Categories를 사용하여 수술환자를 분류한 결과, CategoryⅠ 0.8%, CategoryⅡ 33.8%, CategoryⅢ 29.3%, CategoryⅣ 24.2%, CategoryⅤ 7.3%, CategoryⅥ 4.5%로 분류되어 2군〜4군이 87.3%를 차지하였다. Hart(1985)의 도구를 사용한 수술환자분류는 상급종합병원과 종합병원 간에 차이가 없었으나, AORN(1993)의 도구를 사용한 수술환자분류는 상급종합병원과 종합병원 간 차이가 있는 것으로 나타났다.
5) 환자분류에 따른 수술간호원가를 산정한 결과, Hart(1985)의 4군 분류는 Acuity SystemⅠ 41,360원, Acuity SystemⅡ 90,804원, Acuity SystemⅢ 142,044원, Acuity SystemⅣ 207,803원으로 산정되었다. AORn(1994)의 6군 분류는 CategoryⅠ 35,093원, CategoryⅡ 66,890원, CategoryⅢ 110,934원, CategoryⅣ 157,806원, CategoryⅤ 212,556원, CategoryⅥ 310,529원으로 산정되었다.
본 연구는 수술실에서 가장 많이 이루어지는 수술 중 간호행위를 분석하여 이를 기반으로 수술환자를 분류하여 수술간호원가 산정을 국내에서 최초로 시도하였다는 점에서 가장 큰 의의가 있다. 본 연구결과를 토대로 지속적인 수술 간호원가에 대한 연구의 기초가 될 것이며 더 나아가 이를 통한 수술실 간호사의 업무만족도 향상 및 수술간호관리료 책정의 토대가 형성되어 수술실의 경제적 가치와 적정인력 확보에도 기여할 수 있을 것이다.

ABSTRACT
This Study study is to define Intraoperative Nursing Acuity which is main responsibility in OR, to estimate nursing costs, and to eventually provide basis for an adequate estimation of human resources and care in OR nursing.
In the process, we 1) selected surgical Patient classification method, 2) defined Intraoperative Nursing Activity and its validity, 3) verified the frequency of Intraoperative Nursing Activity, 4) categorized patients using surgical Patient classification method, and 5) estimated nursing cost based on surgical Patient classification.
As a study method, Hart(1985)’s Acuity System and Intraoperative Acuity Categories by AORN(1993) were utilized to categorize surgical patients and Nursing Activity of Harumi et al.(2014) was utilized on estimation of OR nursing activities. Data collection was conducted from March, 1st, 2015 to April 10th.
Data analysis through SPSS 19.0 program was completed to analyze validity of surgical patient classification method using Content Validity Index(CVI), identification of nursing activities per function and surgical patient classification using descriptive statistics, and Comparison of surgical patient classifications by each hospital type using Chi-square test. The calculation of nursing cost per each patient class was estimated as below;

= Nursing Cost per minute × average nursing time per patient type per minute
The results are as follow;
1. Hart (1985)’s Acuity System and Intraoperative Acuity Categories by AORN (1993) were selected through literature review and current surgical Patient classification method. Validity of selected surgical patient classification method was researched by 11 field professionals and all contents recorded higher than 0.8 in Content Validity Index.
2. Intraoperative Nursing Activity was categorized in 4 categories and 58 activities. Each Intraoperative Nursing Activity category consists of 15 activities in Suite Preparation, 17 in Patient Preparation, 14 in Surgical Procedure, and 12 in Suite Clean-up. Validity of Intraoperative Nursing Activity on each category was evaluated by 11 field professionals and all contents recorded higher than 0.8 in Content Validity Index.
3. After verifying the frequency of Intraoperative Nursing activity, Checking the operation schedule was recorded the highest with 100% in Suite Preparation and ‘Toxic substances management’ with 10.8%. To identify surgical site and patients, setting for operation, patients confirmation (time-out) was 100% and Foley catheter insertion with 11.7% in Patient Preparation Category. Operation assist and to wear gloves and gown to surgical team was 100%, the highest, and procurement for additional surgery instrument was 43.3%, the lowest in Surgical Procedure. In Suite Clean category, to organize within the operation room and to write a operation nursing record was 100% and infection pre-operation/ post-operation instrument cleaning was 36.7%.
4. The result of classification with Hart(1985)’s Acuity was 0.3% for Acuity System Ⅰ, 40.6% for Acuity System Ⅱ, 55.8% for Acuity System Ⅲ, and 3.4% for Acuity System Ⅳ; the majority was under Acuity System Ⅱ and Ⅲ with 96.4%. Using Intraoperative Acuity Categories of AORN(1993), 0.8% of OR nursing activities were in Category Ⅰ, 33.8% in Category Ⅱ, 29.3% in Category Ⅲ, 24.2% in Category Ⅳ, 7.3% in Category Ⅴ, and 4.5% in Category Ⅵ; the majority was from Category Ⅱ to Ⅳ with 87.3%. With Hart(1985)’s method, there showed no significant difference between Top medical centers and other medical centers; however, with AORN(1993)’s method showed some differences between the two groups.
5. OR nursing cost price under Intraoperative Acuity Categories with Hart(1985)’s method, 41,360KRW was calculated for Acuity System Ⅰ, 90,804KRW for Acuity System Ⅱ, 142,044KRW for Acuity System Ⅲ, and 207,803KRW for Acuity System Ⅳ. With AORN(1993)’s classification method, it was 35,093KRW for Category Ⅰ, 66,890KRW for Category Ⅱ, 110,934KRW for Category Ⅲ, 157,806KRW for Category Ⅳ, 212,556KRW for Category Ⅴ, and 310,529KRW for Category Ⅵ.
This study has its meaningful signification in that it was the first attempt to utilize surgical patient classification based on OR nursing time and to estimate OR nursing cost price using OP patient classification with required nursing time, which consists the most of what OR nurses do in the OR. Based on the study results, this study will provide basis for consistent research and development in estimation of Nursing cost, and eventually increase the satisfaction of OR nurses. Furthermore, it can contribute to set an adequate Nursing Cost estimation and to provide an opportunity to introduce the importance of OR in financial values and human resources of OR.
