This study aims to investigate whether electro-encephalogram (EEG) testing can become an index for predicting seizure recurrence in adult epileptic patients. The subjects of this study had all experienced recent seizures prior to undergoing EEG testing and were subsequently initiated on medical therapy. Analysis of distribution of seizure recurrence showed that patients with normal EEG results had the highest seizure free rate (33.9%) with subsequent decrease in seizure free rate as the level of EEG abnormality increased. In addition, patients with the most abnormal EEG results, i.e., abnormal III, had the highest seizure recurrence rate (40.2%) with subsequent decrease in seizure recurrence rate with decreasing levels of EEG abnormality. Therefore, the recurrence of seizure was found to be significantly higher in those patients with an abnormality in EEG testing. (p<0.01). Furthermore, when looking at the rate of seizure recurrence by EEG results, the normal EEG group had a 10.7% seizure recurrence rate which was the lowest while abnormal I, II and III groups had recurrence rate of 24%, 27.6% and 39.8%, respectively. Therefore, there was statistically higher rate of seizure recurrence with increasing levels of abnormality in EEG testing results. Lastly, analysis of seizure recurrence in patients belonging to the abnormal III group showed that those interictal epileptiform discharge (IED) had higher rates of seizure recurrence. Among 45 patient who had no seizure recurrence, 29 patients (64.4%) belonged to IED nonexistence group with improved EEG result, whereas 22 (66.7%) out of 33 patients who suffered from seizure recurrence belonged to IED existence group without any improvement in their EEG results. Furthermore, when looking at the rate of seizure recurrence by IED, the IED nonexistence group had 27.5% seizure recurrence rate, whereas the IED existence group had 57.8% seizure recurrence rate. Therefore, when IED existed, the rate of seizure recurrence was significantly higher (p<0.01). This result was not significant in patients with generalized epilepsy but significant in patients with localization-related epilepsy (p<0.01). This study found that abnormal EEG results and IED correlates with increased risk of seizure recurrence in patients with localization-related epilepsy. Therefore, EEG testing can be an important prognostic index and may be a necessary tool in future studies investigating seizure recurrence and its correlation with antiepileptic use in various epileptic syndromes.
