본 연구는 제 2형 당뇨병 환자를 대상으로 실습교육을 병행한 영양교육의 효과를 알아보기 위하여 진행되었다. 2014년 3월부터 2014년 8월까지 약 6개월간 한림대 강남성심병원 내분비내과 외래에서 제2형 당뇨로 진단받은 외래환자 108명을 대상으로 실습교육이 포함된 당뇨영양교육인 당뇨조식회를 실시한 환자를 교육군으로 선정하였고, 어떠한 영양중재도 실시하지 않은 환자를 대조군으로 선정하였다. 연구시작 시점에 모든 군의 일반사항, 신체계측, 생화학검사와 24시간 회상법으로 식사 섭취 조사, 설문조사로 식습관과 식행동을 조사하였고, 그 후 교육군에게는 실습교육이 포함된 당뇨조식회를 실시하였고, 대조군에게는 당뇨영양교육을 실시하지 않았다. 영양교육 중재 3개월 후, 모든 군에 연구시작 시점에 실시하였던 신체계측, 생화학검사, 식사 섭취 조사, 식습관 및 식행동을 같은 방법으로 재조사하여 각 군의 연구 전후 차이와 각 군 간의 차이를 평가하였다. 영양교육 중재 전, 평균 연령, 성별, 평균 신장, 평균 체중에 있어 교육군과 대조군 간에 유의적 차이가 없었지만, 체질량지수(body mass index, BMI)와 허리둘레에 있어 각 군 간에 유의적인 차이가 있었다(p < 0.05). 신체계측은 영양교육 중재 후 교육군의 평균체중, 허리둘레가 유의하게 감소하였고(p < 0.05), 대조군의 허리둘레는 유의하게 증가하였다(p < 0.05). 생화학 검사는 영양교육 중재 후 교육군의 식후 혈당이 유의하게 감소하였고(p < 0.05), 혈중 총콜레스테롤은 교육군에서 증가, 대조군에서는 감소하였으며, 두 군간 변화량에 유의한 차이가 있었다(p < 0.05). 식사 섭취는 영양교육 중재 후 교육군의 탄수화물 섭취가 유의적으로 감소하였고(p < 0.05), 대조군의 탄수화물 섭취는 유의적으로 증가하였으며(p < 0.05), 두군 간의 탄수화물 섭취 변화량에서 유의한 차이가 있었다(p < 0.01). 단백질 섭취는 두 군 모두 유의적 증가하였다(p < 0.05). 에너지 구성 비율은 영양교육 중재 후 교육군에서 탄수화물의 섭취 비율이 유의적으로 감소하였고(p < 0.01), 단백질(p < 0.01)과 지방(p < 0.05)의 섭취 비율은 유의적으로 증가하여서 대한당뇨병학회의 권고비율에 가까워짐을 볼 수 있었다. 대조군의 경우 단백질 섭취 비율이 유의적으로 증가하였다(p < 0.05). 그 외 영양소 섭취 변화는 영양교육 중재 후 교육군에서 식이섬유소(p < 0.05), 칼슘(p < 0.001), 인(p < 0.001), 칼륨(p < 0.01), 아연(p < 0.001), 비타민 B1(p < 0.05), 비타민 B2(p < 0.001), 비타민 B6(p < 0.01), 나이아신(p < 0.001), 엽산(p < 0.001), 비타민 E(p < 0.01) 섭취가 유의하게 증가하였고, 대조군은 콜레스테롤 섭취가 증가하였으며(p < 0.05), 두군 간의 식이섬유소(p < 0.05), 칼슘(p < 0.01), 인(p < 0.001), 칼륨(p < 0.05), 아연(p < 0.001), 베타카로틴(p < 0.05), 비타민 B1(p < 0.05), 비타민 B2(p < 0.05), 나이아신(p < 0.01), 엽산(p < 0.01), 비타민 E(p < 0.05)의 섭취량 변화량에서 유의적인 차이를 보였다. 식습관은 영양교육 중재 후 교육군에서 하루 3끼 식사하는 경우(p < 0.05)와 규칙적으로 식사하는 경우(p < 0.001)가 유의적으로 증가하였다. 식행동은 영양교육 중재 후 교육군에서 계획적인 식사, 과일, 채소 섭취, 고지방 식품 섭취, 균형식 섭취 문항의 점수가 유의적으로 향상되었고(p < 0.001), 알콜 섭취 명수는 유의적으로 감소하였다(p < 0.05). 대조군은 고지방 식품 섭취문항에서 유의적으로 감소하였고(p < 0.001), 계획적인 식사(p < 0.05), 균형식 섭취(p < 0.001) 문항에서 유의적으로 향상되었다. 식사의 질은 영양교육 전 후 교육군의 경우 총에너지, 칼슘, 비타민 B2를 제외한 영양소의 영양소 적정 섭취 비율(nutrient adequacy ratio, NAR)이 1이상, 평균 영양소 적정 섭취 비율(mean adequacy ratio, MAR)이 1이상으로 양호하였고, 대조군 대부분 영양소 NAR이 1이상으로 양호하였다. 이상의 결과를 통하여 제2형 당뇨병 환자 대상으로 실습교육을 병행한 영양교육의 효과에 대하여 알 수 있었으며 본 연구의 영양교육 중재가 제2형 당뇨병 환자에게 효과가 있다고 판단된다. 이를 바탕으로 실습교육을 통한 실제적인 영양교육이 추후 영양교육 프로그램 개발에 반영되어 활용되어야 할 것으로 사료된다.

This study was designed to study the effects of nutrition education via a breakfast club on type 2 diabetic patients. 108 adult outpatients diagnosed with Type 2 diabetes within the Department of Internal Medicine of the Hallym University Medical Center between March 2014 and August 2014 were selected as the study subjects, and were allocated to an education or non-education group. Baseline data including the general characteristics, anthropometric and biochemical parameters, dietary intake from a 24-hour recall, and eating habits and dietary behavior, were collected. The education group received nutrition education through a breakfast club; the non-education group did not receive any nutrition education. After 3 months, the data of the two groups were collected and analyzed. There were no significant differences between the two groups in terms of age, sex, height, and weight, but significant differences in BMI(body mass index) and waist circumference(p < 0.05). Upon completion of the study, the education group displayed a significant decrease in weight and waist circumference(p < 0.05), whereas the non-education group showed a significant increase in waist circumference(p < 0.05). In biochemical parameters, the postprandial blood glucose levels of the education group were significantly reduced(p < 0.05). There was a significant difference in the changes in serum and total cholesterol levels of the two groups(p < 0.05). Following the nutrition education intervention, the education group showed a significantly reduced carbohydrate intake(p < 0.05), th non-education group showed a significantly increased carbohydrate intake(p < 0.05), and significant differences in the changes in carbohydrate intake of the two groups(p < 0.01). While the protein intake was significantly increased in both groups(p < 0.05). The carbohydrate intake ratio was significantly reduced in the education group(p < 0.01), while the protein(p < 0.01) intake and fat(p < 0.05) ratio were significantly increased, as recommended by the Korean Diabetes Association. Regarding other nutrients, the intakes of dietary fiber(p < 0.05), calcium(p < 0.001), phosphorus(p < 0.001), potassium(p < 0.01), zinc(p < 0.001), vitamin B1(p < 0.05), vitamin B2(p < 0.001), vitamin B6(p < 0.01), niacin(p < 0.001), folic acid(p < 0.001), and vitamin E(p < 0.01) were significantly increased in the education group, while the cholesterol intake of the non-education group was increased(p < 0.05).
