만성 요통 환자의 통증, 기능, 관절가동범위에 대한 글라스톤 기법의 효과 : The Effect of Graston Technique on the Pain, Function and Range of Motion in Patients with Chronic Low Back Pain원문보기

Clinicians have reported the effects of various instrument-assisted soft tissue mobilization (IASTM) methods in patients. However, few experimental studies have examined the effects of IASTM in patients with chronic lower back pain (CLBP). Thus, we investigated the effects of the Graston technique on pain, function and range of motion (ROM) in patients with CLBP.
Thirty patients with CLBP participated in the study (Graston technique group: 15; control group: 15). Before and after the 4-week intervention program, back pain was assessed using a visual analog scale (VAS), and a functional evaluation of the back was assessed using the Oswestry Disability Index (ODI). The lumbar range of motion (ROM) was measured using an android smartphone. The main effects and their interaction were analyzed by two-way repeated measures analysis of variance (ANOVA), and if a significant main effect or time-by-group interaction effect was detected, a post hoc t-test was used.
A significant time-by-group interaction was observed for the VAS, ODI and ROM (p<0.05). A post hoc paired t-test showed that pain decreased significantly post-intervention in the Graston group (p<0.05). The mean difference in pain was significantly greater in the Graston group than in the control group (p<0.05). The pain and ODI scores significantly decreased post-intervention (p<0.05), and the lumbar ROM significantly increased post-intervention in both groups (p<0.05).
Both the Graston technique and general exercise resulted in pain relief, improvement of function and increased ROM. However, the Graston group showed significantly increased VAS, ODI, and ROM compared to controls. These findings suggest that the Graston technique can be useful in decreasing pain and increasing function and ROM for patients with CLBP.
