The purposes of this study were to investigate the effects of physical strength and depression on quality of life among geriatric hospital elderly patients and provide basic data to help to increase their quality of life. The subjects include elderly patients that were hospitalized at one of four geriatric hospitals in C City, scored 15 points or more in MMSE-K, understood the goals of the study, and consented to participate in it. Data were collected on October 15~November 15, 2015 by making personal contacts with the employees in charge of nursing personnel at the geriatric hospitals convenience sampled among the four ones in C City, getting approval from the hospital directors, heads of nursing departments, and chief nurses of wards at the hospitals that agreed to participate in the study, and conducting individual interviews with them. Employed in the study were a structured questionnaire and a physical strength measuring instrument. The questionnaire consisted of total 45 items including 13 items on general characteristics, 13 on depression, seven on quality of life, and 12 on the Mini-Mental State Examination for Koreans(MMSE-K). Of physical strength, upper muscular strength and lower muscular strength was measured with a dynamometer and blood pressure gauge, respectively. Depression was measured with the Depression Inventory for Geriatric Hospital Elderly Inpatients developed by Lee Ju-yeong(2006). Quality of life was measured with the Quality of Life Inventory revised and supplemented by the Korea Institute for Health and Social Affairs(2008) based on the data of Ministry of Health, Labour, and Welfare of Japan after excluding the items that overlapped items from the Depression Inventory. Collected data were analyzed in frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficients, and stepwise multiple regression with the SPSS Win 18.0 program. The findings were as follows: 1. Subjects' Right upper muscular strength was 14.72±7.46Kg(minimum 0 –maximum 40.75) as lower level, left upper muscular strength was 13.61±7.98Kg(minimum 0- maximum 40.10), and Subjects' right lower muscular strength was 20.98±16.92mmHg(minimum 0- maximum 104), Subjects'left lower muscular strength was 19.58±7.13mmHg(minimum 0- maximum 92). The average of Depression was 4.44±4.32(instrument range : 0-26), and the average mark was 0.34±0.33(2-point scale) as lower level. The average of Quality of Life found to be 27.48±5.80(instrument range : 7-35), and the average mark was 3.92±0.82(5-point scale) as high level. 2. The study analyzed differences in quality of life according to the characteristics of subjects and found significant differences in it according to educational background(F=3.27, p=.022), state of mobility(F=26.99, p<.001), need for assistance in daily life(F=15.09, p<.001), regular exercise(t=4.25, p<.001), and antidepressant medication(t=-2.918, p=.004). Their post-hoc results reveal that the elderly recorded higher quality of life when capable of moving alone and getting in and out of bed than when incapable of getting in and out of bed. Their quality of life was higher when they needed moderate assistance for daily life and no assistance at all than when they needed assistance for daily life. In addition, their quality of life was higher when they did regular exercise and took no antidepressant than when they did no regular exercise and took an antidepressant, respectively. 3. There were intermediate negative correlations(r=-.58, p<.001) between the depression and quality of life of the subjects. Their quality of life had weak positive correlations with their upper right muscular strength(r=.26, p<.001), upper left muscular strength(r=.23, p=.002), and lower right muscular strength (r=.15, p=.039). That is, their quality of life increased when they were lower in depression and higher in upper right and left muscular strength and lower right muscular strength. 4. Of the factors influencing the quality of life of the subjects, depression had the biggest influences(ß=-.48) on it with explanatory power of 33.9%. With the addition of need for assistance for daily life and regular exercise, it explained 46.2% of overall quality of life. Those findings indicate that the quality of life of geriatric hospital elderly inpatients increased when their depression was lower and their physical strength was higher, which raises a need to increase their physical strength and decrease their depression to enhance their quality of life. There is also a need for comprehensive integrated nursing interventions for higher quality of life including programs designed to increase their physical strength through muscle exercise and reduce their depression.
