(BDNF rs6265, COMT rs4680, DRD1 rs4532, DRD2/ANKK1 rs1800497, DRD3 rs6280). (odds ratio 11.90; 95% CI, 1.78-79.54, p < 0.01).

The dopamine system plays a key role in motor learning and neuroplasticity. Studies targeting the dopamine system to enhance motor recovery after stroke have yielded controversial results. Although genetic variations in plasticity-related genes influence individual capacities for recovery after stroke, few studies have investigated the effects of polymorphisms of dopamine-related genes. Objective. We aimed to investigate the association between motor recovery in stroke patients and polymorphisms in brain-derived neurotrophic factor (BDNF), the dopamine-related gene catechol-O-methyltransferase (COMT), and dopamine receptors (DR) D1, DRD2, and DRD3. Methods We enrolled 74 patients with first-ever stroke (42 males, 32 females, mean age 61.4 ± 14.1). During admission, blood samples were collected, and genotypes of BDNF, COMT, DRD1, DRD2, and DRD3 were analyzed with PCR-directed sequencing method. Primer sequences and restriction enzymes were chosen based on established protocols for BDNF rs6265, COMT rs4680, DRD1 rs4532, DRD2/ANKK1 rs1800497 and DRD3 rs6280. Primary outcome was assessed with Fugl-Meyer Assessment (FMA) at 1 week, discharge, and 3 and 6 months after stroke; secondary outcome was assessed with Functional Independence Measure (FIM) at discharge and at 3 and 6 months after stroke. Association between polymorphisms and functional outcome was analyzed. Results COMT genotypes were significantly associated with FMA and FIM at 3 months (p < 0.01 and p < 0.05, respectively) and at 6 months (p < 0.01 and p < 0.05, respectively). There were no other significant associations between individual polymorphisms and FMA or FIM scores. Multiple regression analysis of individual or combined polymorphisms showed that COMT was associated with FMA at 6 months (odds ratio 16.13; 95% CI, 2.62-99.20, p < 0.01), and combined DRD1/2 polymorphisms were associated with FMA at 3 months (odds ratio 11.90; 95% CI, 1.78-79.54, p < 0.01). Conclusion These data suggest that genetic variation of dopamine-related genes may affect motor recovery after stroke. COMT, DRD1, and DRD2 polymorphisms may be useful for predicting motor recovery.
