For the improvement of sitting position and upper limb function in the daily activities of children with cerebral palsy, previous studies have suggested such methods as vestibular stimulation, sensory integration training, balance training using weight shifts, trunk stability exercises, and horse-riding simulation. These intervention methods have resulted in improvements to sitting position control and upper limb functions in cases of spastic cerebral palsy. However, few studies use children with Gross Motor Function Classification System (GMFCS) levels 3 and 4 spastic cerebral palsy as samples. Moreover, previous studies mostly focused on a single intervention method and it is difficult to find studies comparing diverse intervention methods. Hence, the present study examines and compares the impact of balance training, trunk stability exercises , and horse-riding simulator exercises on balance, posture control, and upper limb function in children with spastic cerebral palsy who require improved posture control for independent sitting in daily activities and for functional movement. A total of 41 school-age children diagnosed with spastic cerebral palsy were randomly allocated and eight children who refused to participate or who showed a low participation rate during the research process were excluded from the sample. The sample consisted of a total of 33 subjects: 10 in the balance training control group, and 12 and 11 in the experimental trunk stability group and the horse-riding simulator exercise group, respectively. Each of the intervention methods was performed for 40 minutes, three times a week for seven weeks. To examine changes in sitting position balance, posture control, and upper limb function performance ability, we evaluated sway distance in the sitting position, and performed the Trunk Impairment Scale (TIS), a sitting evaluation using the Gross Motor Function Measurement (GMFM), and the Jebsen Taylor hand function test (JTHFT). According to the study results, all of the items caused significant effects in all three groups after the intervention (p<.05). Among the three groups, in the measurement of sway distance, which investigated the improvement of sitting position balance ability, the horse-riding simulator exercise group showed a significant improvement compared to the balance training group. In the evaluation of sitting using the GMFM and TIS score averages, which indicated improvement in posture control in the sitting position, both the trunk stability group and the horse-riding simulator exercise group showed significant improvements compared to the balance training group (p<.05). Performance times for seven items were measured in the JTHFT to compare the functional improvement of upper limb ability in the sitting position among the three groups. In the trunk stability group, four sub-items, including short sentence writing, putting small objects inside, mimicking eating, and moving large light cans, showed a significantly decreased time compared to the control group (p<.05).
