본 연구는 신경인지장애(Neurocognitive disorder, 이하 NCD)에서 빈도가 많은 알츠하이머병으로 인한 NCD(이하 AD), 혈관성 NCD(이하 VD) 그리고 DSM-5에서 NCD의 아형으로 포함된 외상성 뇌손상으로 인한 NCD(이하 TBI) 환자를 대상으로 아형별로 신경인지기능의 차이와 함께 중증도 분류(Major NCD, Mild NCD)에 영향을 미치는 인지기능 영역에 대하여 알아보고자 하였다. 연구 대상자는 2015년 7월 1일부터 2015년 11월 5일까지 인지기능저하를 증상으로 내원한 환자들 중 NCD 진단 기준에 부합하고 배제 기준에 해당되지 않는 132명의 환자를 대상으로 하였다. 이들에 대하여 병록지에 근거한 인구학적 자료 및 신경심리검사 자료(서울신경심리검사 2판)를 후향적으로 수집하였으며, 임상치매평가척도(Clinical dementia rating) 점수를 기준으로 중증도를 분류하였다. 주의집중능력에서는 숫자 외우기 검사(Digit span test)의 거꾸로 따라 외우기(Backward)에서 Mild AD가 Mild TBI와 Mild VD에 비하여 유의하게 높은 수행을 보였으며, Forward-backward에서는 Mild TBI가 Mild VD에 비하여 유의하게 높은 수행을 보였다. 언어 기능에 대한 평가로 이름대기 검사(Korean-Boston Naming Test, 이하 K-BNT)에서 Major AD가 Major VD에 비하여 유의하게 높은 수행을 보였다. 시공간 기능에서는 집단 간 유의한 차이를 보이지 않았다. 기억력에서는 노인용 서울언어학습검사(Seoul Verbal Learning Test-Elderly's version, 이하 SVLT-E) 중 즉각 회상에서 Major AD가 Major TBI에 비하여 유의하게 높은 수행을 보였다. 전두엽 및 실행기능에서는 Mild NCD 아형 간 비교에서 범주 및 의미 유창성 검사에서 Mild AD가 Mild TBI에 비하여 유의하게 높은 수행을 보였다. Major NCD 아형 간 비교에서는 Major TBI가 Major AD에 비하여 글자 유창성 검사('ㅅ', 전체 합), 한국판 색깔-단어 스트룹 과제(Korean-color word stroop test, 이하 K-CWS) 중 색깔 읽기, 숫자기호쓰기 검사(Digit symbol coding, 이하 DSC), 한국판 노인용 선로잇기검사(Korean-trail making test-elderly's version, 이하 K-TMT-E) A에서 수행이 저하되었다. 또한 Major VD가 Major AD에 비하여 DSC, K-TMT-E A에서 수행이 저하되었다. Major NCD와 Mild NCD 분류에 영향을 미치는 소검사로 AD에서는 K-BNT, 레이복합도형검사 (Rey complex Figure test, 이하 RCFT) 모사, SVLT-E와 RCFT 중 즉각 회상 항목으로 나타났다. TBI에서는 K-BNT, RCFT 모사, SVLT-E 중 즉각 회상 항목이 유의한 영향을 미치는 것으로 나타났다. Objectives: The purpose of this study was to assess differences in cognitive function of patients with neurocognitive disorder by subtypes and severity, and to investigate the cognitive tests affecting severity classification of neurocognitive disorder. Methods: We retrospectively examined 132 patients diagnosed with neurocognitive disorder(NCD) due to Alzheimer's disease(AD), vascular neurocognitive disorder(VD) and traumatic brain injury(TBI) according to the Diagnostic and Statistical Manual of Mental Disorders fifth edition. All patients were administrated the Seoul Neuropsychological Screening Battery 2nd Edition and were classified into Major NCD and Mild NCD by clinical dementia rating score. Results: In digit span test, Mild AD group scored higher than Mild TBI group and Mild VD group on backward tasks. Major AD group scored higher than Major VD group on Korean-Boston Naming Test. In Seoul Verbal Learning Test-Elderly's version(SVLT-E), Major AD group scored higher than Major TBI group on immediate recall. Major TBI group was impaired relative to Major AD group on letter fluency test in Controlled oral word association test(COWAT), color reading test in Korean-color word stroop test, Digit symbol coding test, Korean-trail making test-Elderly's version(K-TMT-E) part A. Major VD group scored lower than Major AD group on Digit symbol coding test, K-TMT-E part A. Logistic regression analyses showed that cognitive tests affecting severity classification of neurocognitive disorder were Korean-Boston Naming Test and SVLT-E. Conclusion: Major VD group exhibited greater impairment on language function, frontal and executive function than Major AD group. Major TBI group exhibited greater impairment on memory, frontal and executive function than Major AD group. The data also suggested that Korean-Boston Naming Test and memory function test could be useful tools in classifying severity of neurocognitive disorder.
