Objectives: Radiographies for orthodontic treatment include panoramic radiography, lateral and posteroanterior(PA) cephalographies, and cone beam CT(CBCT), nowadays. Use of CBCT in orthodontic treatment has been increased, which provides three dimensional(3D) data for the analysis of 3D structure and soft tissue profile. The younger, the more sensitive to the radiation will be. The objectives of this study was to compare the effective doses of panoramic radiography, lateral and PA cephalographies, and cone beam CT(CBCT) in adult, adolescence, and child for orthodontic treatment.
Materials and Methods: Phantoms of adult male, adolescence, and 10-year child were prepared for panoramic radiography, lateral and PA cephalographies, and CBCT. Twenty four of radiophotoluminescent glass dosimeters(GD) were inserted into the critical organs and tissues to radiation of phantoms for each radiography. After exposure and process of GDs, absorbed doses of tissues and organs were obtained. Effective dose were induced from these doses using tissue weighting factors defined in ICRP 2007 publication 103.
Results: 1. Parotid gland in adult, submandibular and sublingual glands in adolescence, and parotid gland in child showed the highes absorbed dose for panoramic radiography. 2. Right side of organs and tissues showed the highest absorbed doses for lateral cephalography. Posterior side of organs and tissues showed the highest absorbed doses for PA cephalography. Parotid glands for lateral and submandibular glands for PA cephalographies showed high absorbed doses among the salivary glands. 3. Lenses in orbit was the highest absorbed dose for CBCT. Posterior cervical skin was the second highest. Then, buccal skin showed the third highest dose. 4. The highest effective dose was adult followed by adolescence. Child showed the lowest effective dose. The effective doses of CBCT were high in adolescence, followed by adult then child. 5. The effective dose of conventional radiographies were 38.8 µSv, 321 µSv, and 16.6 µSv for adult, adolescence, and child, respectively. The effective doses of CBCT were 338 µSv, 352.8 µSv, and 226.9 µSv for adult, adolescence, and child, respectively.
Conclusion: The effective doses of CBCT were 8.7-fold, 11-fold, and 13.7-fold those of conventional radiograpies in adult, adolescence, and child, respectively. Dentist should be aware of patient age because the younger patient has higher risk to radiation.
