In an effort to improve upon care issues that stem from the perception that in-patient care is a family responsibility, a pilot program for comprehensive nursing services was initiated in 2013. Since 2015, this program was moved into the health insurance reimbursement system. Comprehensive nursing services provide comprehensive nursing care to inpatients through expanding the nursing workforce. This was initiated as a pilot program under the name ‘Patient Anshim Hospital’ by Seoul Medical Center and ‘Comprehensive Nursing System’ by the Korea Ministry of Health and Welfare. The present study was a qualitative study based on Giorgi’s phenomenological research methodology. This methodology aims to provide basic data for heightening the quality of nursing care through exploration of the significance and essence of the care itself. Data are derived from in-depth interviews with nurses on their experiences in providing comprehensive nursing services without the presence of guardians, and with nurse managers who manage comprehensive nursing wards. The study questions were “How are the experiences of nurses who care for in-patients in comprehensive nursing service wards?” or “How are the experiences of managers who manage comprehensive nursing service wards?” The data collection period was from March 4 through June 30, 2014. Study participants included 10 ward nurses and 3 nurse managers with 10 or more months of experience in providing comprehensive nursing services at a general hospital that provided the comprehensive nursing services for over a year. In-depth interview data were collected and each participant was interviewed once, with an average of 91 minutes being required for each interview. Data analysis was performed in 4 steps, according to Giorgi’s phenomenological experience research methodology. The findings of the present study were as follows. 4 components and 23 subcomponents was derived. Some subcomponents were appeared differently or was found to be the same for nurses and nurse managers. First, some subcomponents of “waiting for comprehensive nursing” appeared differently for nurses and nurse managers. For nurses the subcomponents endorsed were, “expectations based on a decrease in the number of patients I am in charge of” and “feel stress and pressure regarding safety accidents.” The subcomponents of “having hopes about work performance from a decrease in the number of patients I am in charge of” and “feel responsibility regarding safety accidents” were endorsed by nurse managers. Meanwhile, the subcomponent that was found to be the same for both nurses and nurse managers was, “I think I want to experience holistic nursing.” Second, some “facing comprehensive nursing head-on” subcomponents appeared differently for nurses and nurse managers. The subcomponents “accept it as a necessary system,” “exhausted from excessive workload,” and “have fears about identity confusion and lowered self-esteem” were endorsed by nurses and “accept the system,” “increased stress from excessive workload,” and “feel lowered self-esteem” for nurse managers. Meanwhile, these subcomponents were found to be the same for both nurses and nurse managers: “it was worthwhile to experience holistic nursing,” “experience difficulties from frequent turnover in nurses,” “all my focus is on preventing safety accidents,” “disgruntled from moral hazard of users,” “difficulty of running into a barrier of absence of guardian,” “accept being the target of vented anger,” “disgruntled about an increase in physical and verbal abuse, drinking, and sexual harassment,” and “sharing with coworkers.” Third, subcomponents of “looking for outcomes of comprehensive nursing” appeared differently in nurses and nurse managers. “Put in an effort to increase nursing professionalism” and “play the role of one’s senior” were endorsed by nurses and “understood the essence of nursing” and “displayed leadership of a manager” by nurse managers. The subcomponent that was found to be the same for both nurses and nurse managers was “experienced improvement in quality of nursing.” Fourth, some subcomponents of “thinking of advancement of comprehensive nursing” appeared differently for nurses and nurse managers. For nurses subcomponents endorsed were “although I hope the comprehensive nursing system will continue, I am agonizing between it and the previous system,” “hoping for advancement of nursing,” “it is necessary to manage stress and raise self-esteem,” and “volunteer programs need to be expanded.” For nurse managers, subcomponents were “hoping for comprehensive nursing system to continue,” “hoping for establishment of policies with vision,” and “need stress mana- gement.” The subcomponents that were found to be the same for both nurses and nurse managers were “support policies for nurses are desperately needed” and “change of perception is needed.”
