Background: Rhinoviruses (RV) cause common cold, pneumonia, and the majority of acute exacerbations of asthma and chronic obstructive pulmonary disease. Unfortunately, treatments for rhinovirus infection are inadequate. Evidence suggests that macrolide antibiotics have anti-inflammatory and antiviral effects. However, there is a lack of in vivo studies investigating the effect of macrolide antibiotics on RV infection. Objective: The aim of this study was to investigate the effects of clarithromycin (CM) on RV infection in a mouse model with controls. Methods: CM (3.0 mg/kg) and phosphate-buffered saline (PBS) were administered intragastrically to BALB/c mice at 9 AM and 3 PM on the first day to the CM and control groups, respectively. RV infection was induced by intranasal instillation of strain RV1b (5 × 106 TICD50/ml) to both groups immediately after the second drug challenge on the first day. Subsequently, CM and PBS were administered once daily. Control and CM mice were sacrificed on days 0, 1, 2, and 4 after RV infection. The concentrations of the proinflammatory cytokines interleukin (IL)-6, macrophage inflammatory protein (MIP)-2, and keratinocyte chemoattractant (KC) were measured in nasal lavage fluid. The RV1b-infected areas of control and CM mice were identified using immunofluorescence. Results: The production of KC, MIP-2, and IL-6 by both groups increased after 24 h and subsequently decreased. However, there were no significant differences between groups. Immunohistochemical analysis of the nasal mucosa of both groups on days 1 and 2 revealed abundant RV1b staining, which was distributed between the epithelium and the lamina propria. In the CM group, the RV1b-infected per unit area was significantly lower than that of control mice at 24 h, 48 h, and 96 h. Conclusions: Although histological analysis revealed decrease of RV infection in the CM group, the anti-RV1b effect of CM was not reflected by cytokine secretion.
