본 연구는 정상 성인과 전방 머리 자세 환자를 대상으로 머리 위로 팔 들기 검사 시 실시간 시각적인 피드백이 위쪽 상부 승모근(UT), 앞톱니근(SA), 목빗근(SCM) 및 하부 승모근(LT)의 근 활성도와 머리 위치 변화에 미치는 영향을 알아보기 위해 실시하였다. 연구 대상자는 총 30명으로 정상성인 15명과 전방 머리 자세 환자 15명으로 나누어 머리 위로 팔 들기 검사를 실시하였다. 그 이후에 전방 머리 자세 그룹이 각각 자세교정과 시각적인 피드백을 적용해서 다시 머리 위로 팔 들기 검사를 수행하였다. 머리 위로 팔 들기 검사 시, UT, LT, SA 및 SCM의 근 활성도는 SEMG를 사용하여 측정하고 머리 안정성은 머리 위치 변화를 통해 측정하였다. 머리 위로 팔 들기 검사 시, 자세 교정보다 실시간 시각적인 피드백을 적용한 후 UT, SCM와 LT의 근 활성도가 유의하게 감소하고 SA의 근 활성도가 유의하게 증가하였다(p<.05). 자세 교정보다 시각적인 피드백을 적용할 때 머리 위치의 변화가 유의하게 감소하였다(p<.05). 결론적으로 자세 교정이 중요하지만 실시간 시각적인 피드백이 환자가 머리 위치를 관찰하고 스스로 조절할 수 있기 때문에 전방 머리 자세 환자에게 시각적인 피드백과 자세 교정을 동시에 적용하는 것은 최선의 선택이 될 수 있는 것으로 나타났다.

The purpose of this study was to explore the immediate effects of real-time visual feedback using a video display on muscle activity in upper trapezius(UT), serratus anterior(SA), sternocleidomastoid(SCM) and lower trapezius(LT) and change of head position during overhead arm lift test in individuals with forward head posture(FHP). Fifteen normal subjects and fifteen subjects with FHP were asked to perform overhead arm lift test to determine the differences between two groups. Then, FHP group were asked to perform overhead arm lift test again after posture correction and visual feedback respectively to determine the differences within the FHP group. Muscle activities of UT, LT, SA, and SCM were measured using SEMG and the stability of head was measured using change of head position during overhead arm lift test. There were significant decreases in UT, SCM and LT muscle activities and significant increase in SA muscle activity under visual feedback condition than posture correction during overhead arm lift test. There was also a significant decrease in change of head position in individuals with FHP under visual feedback condition than posture correction. Thus, although posture correction is important, as real-time visual feedback can make participants observe and adjust head position themselves, apply posture correction as well as visual feedback can be regarded as a more effective treatment for FHP.
