It has been well known that about 80% of aircraft accidents are related to human errors, and that most of them are caused by human errors of pilots. However, before discussing human errors of pilots, poor maintenance by mechanics can be criticized to result or act as potential elements of aircraft accidents, which may be a huge risk factor disrupting flight safety. The importance of maintenance factors related to aircraft is not an exception for military aircrafts. This thesis was aimed to elicit causes of human errors related with poor maintenance for military aircrafts and to promote flight safety. Forty accidents cases occurred in recent 10 years which were attributed to human errors of mechanics were analyzed through HFACS-ME (Human Factors Analysis and Classification System-Maintenance Extension) technique which was specially devised to analyze the human factors of aircraft maintenance, as well as CHEAM (Comprehensive Human Error Analysis Model) which was developed for human error analysis. Based on those results, this study discussed what would be the human factors contributed to each accident and be the most effective method to prevent aircraft accidents related with human errors of mechanics. Results were not seriously different. Comparison with the 4M classification scheme showed that ‘Management’ factor was the highest with 52%, and 53%, respectively, and was followed by ‘Man’ factor occupied about 30% or more. Major factors in ‘Management’ were ‘insufficient supervision’, and ‘improper manpower’ whereas ‘mechanic’s personal factors’ and ‘violation of standard work procedure’ in ‘Man’. To analyze causes of human errors in detail, ‘mechanic’s personal factors’ occupied about 30%, and ‘confirmation or supervision’ and ‘violation of standard work procedure’ did about 20% and 10%, respectively. With consideration of military service which emphasizes organization, discipline, and command, most accident causes were summarized as two major factors - insufficient systematic support and improper supervision. As a consequence, in this thesis, countermeasures were suggested in those two facets – supply of qualified mechanics, and guarantee of work hours, improvement of education method as well as education contents for the former, and establishment or improvement of standard work procedure and revision of mechanical check points for the latter.
