알츠하이머질환(Alzheimer’s disease)은 뇌의 피질(Cortex)과 해마(Hippocmapus)에 senile plaque와 neurofibrillary tangle이 생성되어 심각한 학습(Learning)과 기억(Memory)장애를 동반하는 대표적인 신경퇴행성질환(Neurodegenerative disorder)이다. 이러한 질환을 치료하기 위해 다양한 치료적 전략이 고려되고 있으며, 가장 보편적인 치료전략은 질환초기에 산화적 스트레스(Oxdative stress), 만성염증반응(Chronic inflammatory response), Aβ와 tau 단백질의 침적 등과 같은 심각한 병리적 증상을 억제하려는 것이다. 또한, 알츠하이머에너지대사 손상의 증진, 세포손상의 회복기전 촉진, 신경전달(Neurotransmission)의 촉진 등이 다른 치료전략으로 시도되고 있다. 이러한 치료전략과 더불어 최근에서는 하나의 치료제를 이용하여 여러 가지 표적을 동시에 조절함으로서 치료효과를 극대화하려는 전략이 많이 연구되고 있다. 따라서 본 연구에서는 알츠하이머 질환 치료를 위해 신경전달 촉진과 산화적 스트레스 억제라는 두 가지 표적을 동시에 조절할 수 있는 새로운 천연물 치료제를 발굴하고 이들의 효능평가를 신경 세포주와 치매모델동물을 이용하여 평가하고자 하였다. 이를 위해 열병, 감기, 신장질환, 유방암, 염증질환, 뇌질환 등의 치료에 오랫동안 사용되어온 약용식물(medicinal plant)로서 천문동(Asparagus cochinchinensis Merr.)을 선정하였으며 천문동 열수추출물(Aqueous extract from Asparagus cochinchinensis) (AEAC)을 분리하여 주요성분과 항산화능을 분석하고 신경세포주와 치매모델동물에서 신경성장인자(Nerve growth factor, NGF) 촉진제와 항산화제(Antioxidants)로서의 기능을 분석하고자 하였다. 우선, AEAC의 구성성분을 분석한 후 항산화제와 신경성장인자 촉진제로서 작용기전을 신경세포주에서 분석하였다. 그 결과 AEAC는 플라보노이드(Flavonoids, 1.32 mg/g), 페놀(Phenols, 13.8 mg/g), 사포닌(Saponins, 57.2 mg/g) 및 프로토디오신(Protodioscin) 등과 같은 여러 가지 성분으로 구성되어 있었으며, 세포 내 ROS농도는 AEAC로 처리된 미세아교세포(Microglial cells)에서 유의적으로 감소하였다. AEAC로 처리된 B35세포의 배양액에 NGF단백질의 농도와 NGF mRNA의 발현은 Vehicle그룹에 비하여 유의적으로 증가하였지만, 세포생존율(Cell viability)는 특이적인 변화가 관찰되지 않았다. 또한 B35세포로부터 분비된 NGF의 생리활성을 확인하기 위해, AEAC에 의해 분비된 NGF를 미분화된 PC12세포에 처리한 후 이들 세포의 neuritic outgrowth을 관찰하였다. 그 결과 PC12세포의 수상돌기 길이(Dendritic length)는 Vehicle그룹에 비하여 AEAC그룹에서 유의적으로 증가하였다. AEAC그룹에서 high affinity NGF 수용체의 하위신호전달에 포함된 TrkA와 ERK의 인산화를 촉진시켰지만 low affinity NGF 수용체의 하위신호전달에서는 낮은 수준으로 관찰되었다. 다음으로 AEAC가 인간과 유사한 증상을 나타내는 질환모델동물 Tg2576에서 NGF 촉진과 항산화제 역할로서 치료효과를 확인하기 위하여 Tg2576 마우스에 4주 동안 AEAC를 격일 간 경구투여하여 아밀로이드베타 침적 (Aβ plapue), 아세틸콜린에스터라제 활성도(AChE activity), NGF 수용체의 하위신호전달, 항산화 효능을 분석하였다. 그 결과, 항산화 수용능과 항산화효소(Superoxide dismutase, SOD)의 활성도가 Vehicle+Tg2576그룹에 비하여 AEAC+Tg2576그룹에서 유의적으로 증가하였다. 또한 신경성장인자의 촉진의 기능을 평가 한 결과 신경성장인자의 농도가 Vehicle+Tg2576그룹에 비하여 AEAC+Tg2576그룹에서 유의적으로 회복되었으며 NGF 수용체의 하위신호전달 중 에서는 hgh affinity NGF 수용체의 하위신호전달에서는 Akt의 인산화를 촉진시켰다. 더불어 치매 원인 기전에 미치는 영향을 확인한 결과 아밀로이드 베타 단백질(Amyloid beta peptide, Aβ peptide)과 아밀로이드 베타 단백질의 생산에 관여하는 단백질 분해효소인 감마 시크리타제의 구성 중 프레세닐린2 (Presenilin2, PS2), APH-1 및 니카스트린 (Nicastrin, NCT)의 발현량이 AEAC+Tg2576그룹에서 Non-Tg 그룹의 수준과 유사하게 회복되었다. 뿐만 아니라 신경세포 작용기전과 관련하여 AEAC+Tg2576그룹이 Vehicle+Tg2576그룹이 아세틸콜린 분해효소(Acetyl choline esterase, AChE)의 활성도를 억제하였으며, 세포사가 유의적으로 감소하였다. 마지막으로, AEAC의 간 독성 및 신장 독성에 미치는 영향을 평가하기 위하여 수컷과 암컷 ICR 마우스에 150 mg/kg, 300 mg/kg 및 600 mg/kg의 AEAC를 14일간 경구 투여 후 체중 및 장기 무게 변화, 혈청 생화학검사, 뇨 분석, 조직학적 분석을 실시하였다. 그 결과, AEAC를 경구 투여한 수컷과 암컷 ICR마우스는 체중과 장기 무게에 영향을 미치지 않았으며, 특이적인 병리학적인 증상과 뇨 수치의 변화를 유발하지 않았다. 또한, 간의 독성 및 세포 괴사의 지표인 알라닌 아미노전이효소(Alanine aminotransferase, ALT), 젖산 탈수소효소(Lactate dehydrogenase, LDH), 아스파르트산 아미노전이효소(Aspartate aminotransferase, AST) 및 알칼리성 인산가수분해효소(Alkaline phosphatase, ALP) 와 신장 기증의 혈청 생화학적 지표인 혈액요소질소(Blood urea nitrogen, BUN), 크레아티닌(Creatinine, CRE)의 수치의 변화를 유발하지 않았다. 더불어 조직학적 분석에서도 마찬가지로 간과 신장의 조직에서 독성 물질의 유도로 인해 일반적으로 관찰되는 병리학적인 특징을 유도하지 않았다. 따라서 이러한 결과는 천문동 뿌리로부터 추출된 AEAC는 신경세포로부터 NGF의 분비촉진과 산화적 스트레스를 억제하는 두 가지 표적에 동시에 작용하는 기전으로 알츠하이머와 같은 신경퇴행성질환의 치료물질로서 가능성이 우수함을 제시하고 있다.

Alzheimer’s disease (AD) is a well known neurodegenerative disorder (ND) characterized by the presence of senile plaques and neurofibrillary tangles in the hippocampus and the brain cortex that is accompanied by severe learning and memory impairment. Several pathological strategies to treat this disease have been considered, including blocking several pathological events and stimulating metabolism. Recently, drugs for multiple targets have received a great deal of attention because they have been considered as novel methods for maximizing the therapeutic effects during treatment of neurodegenerative diseases. In this study, we attempted to identify novel natural products with the ability to stimulate neurotransmission and suppress oxidative stress, as well as to evaluate their therapeutic effects in neuroblastoma cells and an animal model. To accomplish this, Asparagus cochinchinensis (A. cochinchinesis) was selected as one of the candidates for multiple target drugs because it has long been used to treat fever, colds, kidney disease, breast cancer, inflammatory diseases, and brain disease. After the collection and component analysis of an aqueous extract of A. cochinchinesis (AEAC), their therapeutic effects based on the role of nerve growth factor (NGF) stimulator and antioxidant were analyzed in neuronal cells and an animal model for AD. First, we investigated the effects of AEAC on the secretion and action mechanism of NGF in neuronal cells. The concentration of NGF protein increased significantly in B35 cells treated with AEAC relative to the Vehicle-treated group without any specific cytotoxicity. To examine the bioactivity of NGF secreted from B35 cells, undifferentiated PC12 cells were cultured in an AEAC-conditioned medium and neuritic outgrowth was observed. The dendrite length of PC12 cells in the AEAC-treated group was significantly higher than that in the Vehicle-treated group. Moreover, the level of the downstream effectors p-TrkA and p-ERK of the high-affinity NGF receptor was significantly higher in the AEAC-treated group, while expression of the downstream effectors of the low-affinity NGF receptor was significantly lower in the same group. These results suggest that AEAC may contribute to the regulation of NGF expression and secretion in neuronal cells. Second, we investigated the therapeutic effects of AEAC, particularly its role as a NGF stimulator and anti-oxidant in Tg2576 mice showing AD phenotypes of human. These effects, as well as enhanced sueperoxide dismutase (SOD) levels, were also detected in AEAC-treated Tg2576 group. The expression of p-Akt among downstream effectors of the high affinity NGF receptor was dramatically recovered in AEAC-treated Tg2576 group, while the expression of p75NTR was slightly recovered in the same group. Significant recovery on the level of amyloid beta-42 (Aβ-42) peptides and the expression of γ-secretase. members including presenilin-2 (PS-2), anterior phrynx defective-1 (APH-1) and nicastrin (NCT) were detected in AEAC-treated Tg2576 group. Furthermore, AEAC-treated group showed decreased numbers of dead cells and suppressed acetyl choline esterase (AChE) activity. Finally, to evaluate the hepatotoxicity and nephrotoxicity of AEAC toward the liver and kidney of ICR mice, alterations in related markers including body weight, organ weight, urine composition, liver pathology and kidney pathology were analyzed in male and female ICR mice after oral administration of 150, 300 and 600 mg/kg body weight/day of AEAC for 14 days. Moreover, the body and organ weight, clinical phenotypes, urine parameters and mortality did not differ among the Vehicle- and AEAC-treated groups. There were also no significant alterations in alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), blood urea nitrogen (BUN) or serum creatinine (Crea) observed in the AEAC-treated group relative to the Vehicle-treated group. Furthermore, the specific pathological features induced by most toxic compounds were not observed upon liver and kidney histological analysis. Overall, the results of the present study suggest that AEAC does not induce any specific toxicity in the liver and kidney of male and female ICR at 600 mg/kg body weight/day, indicating that this is the no observed adverse effect level (NOAEL). Taken together, the results of the present study provide the strong evidence that the pathological phenotypes of AD could be improved through the stimulation of NGF secretion and suppression of oxidative stress. Therefore, we suggest that AEAC can be considered a beneficial natural product for treatment of ND.
