FOMX1 발현억제 시 항암제 처리에 따른 방광암 세포주의 세포생존율이 감소하였고, 효과적인 세포사멸을 일으키는 것을 확인하였다. 또한 FOXM1 발현에 따른 약물유출 기전과 DNA 손상회복 경로조절을 조사한 결과, FOXM1의 발현억제 시 ABCG2의 발현이 감소되어 약물유출기전이 감소되고 세포자멸사가 증가됨을 확인하였다. FOXM1을 억제한 세포에서 MMC를 처리 한 경우에도 FANCD2 발현감소에 따라 DNA 손상회복, 판코니빈혈 경로 활성, 상동재조합 복구 감소와 외가닥절단과 이중가닥절단 증가 등의 DNA 손상회복경로의 활성감소가 진행되어 효과적인 세포사멸이 일어남을 확인하였다. 또한 FOXM1과 FANCD2의 발현이 임상환자의 재발에 미치는 영향을 조사하기 위해 두 유전자의 발현정도와 재발빈도를 분류하였다. 그 결과, 두 유전자들의 높은 발현과 환자의 재발률에 있어 상관관계가 확인되었고, 재발암 조직의 경우 원발암 조직에 비해 FOXM1과 FANCD2 유전자들의 발현이 유의하게 증가됨을 확인하였다. 본 연구에서 FOXM1이 ABCG2와 FANCD2의 발현 조절을 통해 약물유출 기전과 판코니빈혈 경로를 조절하고, 이를 통해 항암제 저항성경로를 조절한다는 것을 규명하였다. 그러므로 FOXM1의 억제를 통해 비근침윤성 방광암에 많이 사용되는 DOX와 MMC의 효과를 증가시킬 수 있다는 것이 확인되어, 이 유전자들이 방광암의 효과적인 항암치료에 도움을 줄 수 있다는 것을 예측할 수 있었다. 또한 FOXM1과 FANCD2의 발현이 방광암 환자의 재발을 예측하는 바이오마커로 사용될 수 있으며, 방광암 재발의 효과적인 표적치료에 사용 가능한 표적유전자로써 활용 가능성을 예측할 수 있었다.

However, NMIBC has a high recurrence rate after treatment and some of the relapsed patients progress to muscle invasive bladder cancer (MIBC). Although these patients are treated with BCG there is a still a lack of response to BCG and side effects such as sepsis occur. To solve these problems, accurate diagnosis and effective chemotherapy are needed to complement the diagnosis and treatment. However, the mechanism of recurrence has not yet been clarified and the mechanism of recurrence is expected to increase the resistance to chemotherapy by drug efflux and DNA repair. The aim of this study was to identify the biomarkers that could be used for predicting recurrence and the development of therapeutic agents based on FOXM1, a key factor of relapse, and to identify the mechanism for relapse of NMIBC by identifying new signaling pathways and drug resistance mechanisms. ABCG2 and FANCD2 were selected in the ABC transporter family which is known to regulate the drug efflux and the fanconi anemia (FA) pathway genes which restore the interstrand cross-links. In this study, we confirmed that FOXM1 binds to the promoter of ABCG2 and FANCD2 and regulates expression. The inhibition of FOMX1 resulted in a decrease in the cell survival of the BC cells after treatment with the anticancer drugs. In addition, we investigated the regulation of drug efflux and DNA repair by FOXM1. As a result, the inhibition of FOXM1 decreased ABCG2 and the drug efflux and increased apoptosis. In FOXM1 inhibited cells, it was confirmed that effective cell death occurred in accordance to the decrease of FANCD2 and the reduction of DNA repair processes such as FA pathway, homologous recombination and single strand and double strand breaks repair. To investigate the effect of FOXM1 and FANCD2 on the recurrence of patients, the level of expression and recurrence frequency of the two genes were classified. As a result, the correlation between the high expression of the two genes and the recurrence rate of the patients was confirmed and FOXM1 and FANCD2 genes were significantly increased in the recurrent tissues compared with the primary tissues. In this study, we demonstrated that FOXM1 regulates drug efflux and FA pathway through the regulation of ABCG2 and FANCD2, thereby regulating the drug resistance. As a result, the inhibition of FOXM1 could help in treatment by increasing the effects of DOX and MMC. In addition, the expression of FOXM1 and FANCD2 could be used as a biomarker to predict recurrence and as a target for treatment.
