본 연구의 결과는 다음과 같다.
첫째, 대상자의 마약성진통제 금단증상관리에 대한 지식의 정답률은 44.9%이었다. 둘째, 대상자의 마약성진통제 금단증상관리에 대한 태도는 4점 리커트 척도(Likert scale)로 2.83±0.25점이었다. 셋째, 대상자의 일반적 특성에 따른 마약성진통제 금단증상관리에 대한 지식수준은 40세 초과 연령의 간호사가 35세 이하보다, 10년 초과의 임상경력이 10년 미만보다, 10년 초과의 아동 분야 근무경력의 간호사가 10년 이하의 간호사보다 통계적으로 유의하게 높았다(p<.001). 근무부서별로는 신생아중환자실 간호사의 지식수준이 소아중환자실, 소아내과계 병동, 소아외과계병동, 소아응급실 간호사에 비해 유의하게 높았다(p<.001). 최종 학력별로는 석사 이상(p=.027)이 일반 학사보다, 직위별로는 전문간호 인력이 일반간호사보다 유의하게 지식수준이 높았다(p=.024). 간호사가 마약성진통제 금단증상 사정도구를 아는 경우, 근무부서 내에 마약성진통제 금단증상 사정도구가 비치되어 있는 경우, 마약성진통제 금단증상을 보이는 환자를 간호한 경험이 있는 경우에 지식수준이 높았다(p<.001). 더불어 마약성진통제 금단증상관리에 대한 교육을 받은 경우(p=.005)와 교육이 필요하다고 답한 간호사(p<.001)가 통계적으로 유의하게 높은 지식수준을 보였다. 넷째, 대상자의 일반적 특성에 따른 마약성진통제 금단증상관리에 대한 태도 차이는 40세 초과의 연령이 30세 이하보다, 10년 초과의 임상경력이 5년 미만보다, 10년 초과의 아동 분야 근무경력이 10년 이하의 경력 간호사보다 통계적으로 유의하게 태도 점수가 높았다(p<.001). 최종 학력별로는 석사 이상의 간호사가, 직위별로는 전문간호 인력의 태도 점수가 일반간호사보다 유의하게 높았다(p<.001). 또한, 마약성진통제를 사용한 환자를 간호한 경험이 있는 경우(p=.041), 마약성진통제 금단증상관리에 대한 교육을 받은 경우(p<.001)와 교육이 필요하다고 답한 간호사(p=.001)가 통계적으로 유의하게 점수가 높아 마약성진통제 금단증상관리에 대해 더 긍정적인 태도를 보였다. 다섯째, 대상자의 마약성진통제 금단증상관리 지식수준이 높을수록 대상자는마약성진통제 금단증상 관리에 대해 긍정적인 태도를 보였다(r=.26, p<.001).
본 연구의 결과로 아동의 마약성진통제 금단증상의 효율적인 관리를 위해서 간호사의 일반적 특성에 따른 교육프로그램의 개발이 필요함을 알 수 있다. 본 연구에서 점수가 낮았던 마약성진통제 금단증상의 원인, 약물적 중재, 통증이나 섬망과 구별되는 증상 등이 포함된 교육프로그램의 개발을 제언한다.

This study was conducted to investigate the pediatric nurses’ knowledge and attitude in relation to iatrogenic opioid withdrawal symptoms management, to examine whether there is a difference between their knowledge and attitude based on their general characteristics, and to identify correlations between their knowledge and attitude. A total of 253 pediatric nurses participated in the survey that was based on a self-reporting questionnaire. Data were collected from March 9 to 26, 2017 at a superior general hospital in Seoul. The research tools used in this study were developed by the researcher after conducting extensive literature reviews. The questionnaires included 35 questions on knowledge, 12 questions on nurses’ attitude toward iatrogenic opioid withdrawal symptoms management, and 13 questions on the general characteristics of pediatric nurses. The data was analyzed by frequency, percentage, mean, standard deviation, independent t-test, one-way ANOVA, post-examination of Tukey’s test, and Pearson's correlation by using SPSS WIN 21.0. The results of this study are as follows: 1. The percentage of correct answers on pediatric nurses’ knowledge toward iatrogenic opioid withdrawal symptoms management was 44.9%. 2. On the pediatric nurses’ attitude toward iatrogenic opioid withdrawal symptoms management, the participants scored an average of 2.83±0.25 out of 4 points. 3. The knowledge level of iatrogenic opioid withdrawal symptom management in accordance to the general characteristics of the subjects was significantly higher in nurses who are aged over 40 years or in nurses with more than 10 years of working experience or in those with more than 10 years’ experience in the pediatric field than those who are of an age less than 35, have under 10 years of experience, or have less than 10 years’ experience in the pediatric field respectively (p<.001). The knowledge level of nurses in the neonatal intensive care unit was significantly higher than that of the nurses in the pediatric intensive care unit, the pediatric medical unit, the pediatric surgical unit, and the pediatric emergency room(p<.001). In terms of their highest level of education, master’s or higher degree holders had significantly better knowledge level than general bachelor degree holders(p=.027) and advanced practice nurses showed a higher knowledge level compared to staff nurses(p=.024). Nurses who are aware of opioid withdrawal symptom assessment tools, who have those tools within their department, and those who had dealt with patients with opioid withdrawal symptoms showed a higher level of knowledge(p<.001). Also, those who have received education for iatrogenic opioid withdrawal symptoms management(p=.005) and those who answered that education is necessary showed a statistically significant higher level of knowledge(p<.001). 4. The attitude score regarding iatrogenic opioid withdrawal symptom management in accordance to the general characteristics of the subjects was significantly higher in nurses who are aged over 40 years or in nurses with more than 10 years of working experience or in those with more than 10 years’ experience in the pediatric field than those who are of an age less than 30, have under 10 years of experience, or have less than 10 years’ experience in the pediatric field respectively(p<.001). The attitude score was significantly higher in nurses in the pediatric emergency room(p=.019) and nurses who hold master’s or higher degree and advanced practice nurses showed significantly higher attitude score compared to staff nurses(p<.001). Attitude score of nurses who have experienced handling patients who take iatrogenic opioid medication(p=.041) and patients presenting iatrogenic opiod withdrawal symptoms showed significantly high(p=.029). Also, those who have received education for iatrogenic opioid withdrawal symptoms management(p<.001) and those who answered that education is necessary(p=.001) showed a statistically significant higher level of knowledge and therefore showed positive attitude towards the corresponding education. 5. There was a significant relationship between pediatric nurses’ knowledge and attitude toward iatrogenic opioid withdrawal symptoms management(r=.26, p<.001). Based on the results of this study, it is necessary to develop an educational program according to the general characteristics of nurses to efficiently manage iatrogenic opioid withdrawal symptoms involving children. In this study, I propose that an educational program must introduced to cover the causes of opioid withdrawal symptoms, medication therapy, and symptoms that can be distinguished from pain and delirium.
