Introduction: Polypharmacy is a major concern in the aging society and its associations with dementia and Parkinson disease (PD) have not been adequately assessed. The objectives of this study were to evaluate the current status and trends of polypharmacy in South Korea, and to determine the associations of polypharmacy with dementia, Parkinsonism, and Parkinson disease (PD) in elderly patients.
Methods: This study consisted of three parts; Part 1 of the analysis of polypharmacy prescription status and trends in Korea, Part 2 of the association between polypharmay and dementia, and Part 3 of the association between polypharmacy and Parkinsonism/PD. Part 1 was conducted using a cross-sectional study design with the Health Insurance Review and Assessment Service National Patient Sample database (n = 5,662,576, 2012-2015). We matched dementia cases for Part 2 and Parkinsonism cases (defined by diagnosis codes for Parkinsonism/PD) and PD cases (patients who had records for both PD diagnosis and anti-Parkinson drug prescriptions) for Part 3 with controls based on the national health insurance sample cohort database ( n= 1,025,340, 2002-2013). The average numbers of prescribed daily drugs were analyzed based on the prescription records during the observational period and polypharmacy was defined as taking 5 or more prescribed daily drugs in average. The observational periods were set as a year or a month for Part 1, two years preceding diagnosis for Part 2, and a year preceding diagnosis for Part 3. Comorbidities were analyzed using the International Classification of Diseases, 10th revision, Clinical Modification and the medications potentially associated with dementia or Parkinsonism were defined based on the Beers Criteria by American Geriatric Society 2015 and analyzed using codes of main ingredients. The main evaluation variables were average numbers of prescribed daily drugs and polypharmacy prevalence for Part 1 and they were analyzed according to age group and comorbidity of target patients. In Part 2 and 3, logistic regressions were conducted to evaluate the association between polypharmacy and target diseases (dementia, or Parkinsonism/PD) with adjusting of comorbidity and medications potentially associated with dementia or Parkinsonism.
Results: In Part 1, the average numbers of prescribed daily drugs per a patient in Korea were 0.70 in the yearly analysis and 2.79 in the monthly analysis. The numbers increased with aging and elderly patients had 2.13 prescribed daily drugs during a year and 5.30 during a month in average. Age, sex, disease severity, cancer, diabetes, and hypertension had statistically significant effects on the increase of the number of prescribed daily drugs and hypertension was the most influential among them. In Part 2, 5562 patients were selected for the dementia cases and matched controls, respectively. The multiple regression analysis with adjusting comorbidities and medications potentially associated with dementia exhibited significantly increased odds rations (ORs) for dementia in polypharmacy group; the ORs were 1.17 (95% CI: 1.01-1.36) in 5 - <10 drug prescriptions and 1.43 (95% CI: 1.00-2.05) in ≥10 drug prescriptions. In Part 3, the study population included 6209 cases and 24,836 controls for Parkinsonism and 1331 cases and 5324 controls for PD. In univariate logistic regression, odds ratios (ORs) for Parkinsonism/PD increased significantly with increased polypharmacy, medications associated with Parkinsonism, CCI score, or prior hospitalizations. In multiple logistic regression, the adjusted ORs for Parkinsonism/PD also increased with increased polypharmacy. ORs (95% CI) for PD were higher than those for Parkinsonism with stronger statistical significance: 1.41 (1.28–1.55) and 2.17 (1.84–2.57) for Parkinsonism and 2.87 (2.30–3.58) and 4.75 (3.39–6.66) for PD for 5–9 prescribed daily drugs and ≥10 drugs, respectively.
Conclusion: The prescription of polypharmacy in South Korea increased with aging and presence or severity of comorbidities. Our results also indicate that polypharmacy exposure over a year prior to diagnosis may be associated with increased risks for dementia or Parkinsonism/PD.
