Background of Research: As nursing students experience various kinds of violence including verbal abuse, sexual harassment and physical threat at the clinical practice sites, I researched the students’ emotional responses and coping actions against those behaviors and their social support. Also, on the basis of such research result, this study aims to make a better clinical practice environment by enhancing nursing students' abilities to cope with various kinds of violence faced at those sites and to provide the basic resources for setting up a social support program.
Methods of Study: Among the students having clinical practice experiences studying at 4-year nursing colleges in their 3rd and 4h year in the universities in the area of Busan, Gyeongnam, and Gyeongbuk, we collected materials from a total 560 respondents, 168 from Busan, 273 from Gyeongnam, and 119 from Gyeongbuk, by receiving self-report survey. It consisted of 230 questionnaires including 41 questions on verbal abuse, 35 on sexual harassment, 20 on physical threat, 56 on emotional reaction, 56 on coping and 22 on social support. The collected answers were analyzed by technical statistics, t-test, ANOVA, and multiple regression analysis using SPSS/Win 22.0 program.
Result of Study and its Conclusion: It was shown that 97.7% of nursing students in clinical practice suffered violence (verbal abuse, sexual harassment, or physical threat) by medical providers (doctors, nurses), patients, guardians and other people; and regardless of the portions of different offenders, the overall types were classified into 96.8% verbal abuse, 65.2% physical threat and 39.8% sexual harassment. The most portion of violators were medical providers (doctors or nurses) scoring 97.8%, followed by patients and guardians with 96.3%, and others (nursing assistants, care givers, lab employees, cleaning staffs, and ambulance employees, etc) with 54.8%. The most portion of people for different categories were medical providers rating 97.6% at verbal abuse, patients and their guardians at 83.0%, and medical providers rating 83.8% at physical threat category.
The categories causing more emotional responses against violence were physical threat, sexual harassment and verbal abuse in order. The most responded coping reactions against violent experiences were emotional regulation, and against physical threat, problem-solving action was the most. The point of social support was 2.81±0.86 (range: 1~5), representing that the point was lower than other ordinary university students in general.
About the conceptual rates upon violence by genders, female students showed a higher conceptual rate and male students scored a higher rate for sexual harassment. By age, the population between the ages of 23 and 25 marked higher points than the age group of 22 and lower on aspects of violence experience, emotional response and coping reaction while 22 and lower age group made higher rates than the 23-25 age group on aspect of social support. Results of the analysis by grades were similar to that by age groups; the 4th year group made higher rates than the 3rd year group in aspects of violence experience, emotional response, and coping action while the 3rd year rated the highest at the social support category. For its geographical aspect, the students in Busan and Gyeongbuk area made higher rates than those in Gyeongnam area in aspects of verbal abuse by medical providers (F=8.495, p<.001), for sexual harassment by patients and their guardians, Gyeongbuk was higher than Gyeongnam (F=3.296, p=.039), and Gyeongbuk scored higher than Busan and Gyeongnam area in aspect of physical threat by patients and their guardians (F=4.747, p=.009).
I did an additional research for 61 male students and 58 respondents (95.1%) experienced violence during their clinical training practices, which were higher than the average rates for both male and female students for other categories such as verbal abuse of 90.0%, sexual harassment of 32.0 %, and physical threat of 52.0%. The most rated offenders of each category were 92.7% verbal abuse by medical providers, 75.0% sexual harassment by medical providers and 78.1% physical threat by patients and their guardians. The most emotional responses after experiencing a violence were against sexual harassment while the most non-effective emotion centered reaction was against physical threat and the most category for positive problem-solving coping action was against sexual harassment. The social support for male students at the clinical practice was higher than that of female students. The result of analysing differences in general characteristics for male students were similar to the combined analysis for both male and female students. The result above shows that nursing students at the clinical practice site experience more violence than the regular nurses working at the hospitals, so we can realize that the professional nurses training fields are threatened by various kinds of violence. Because the emotional responses of nursing students can cause emotional and physical negative effects, and the rate of counter measurements at both practicing institutions and social support that nursing students conceive after experiencing violence were low, we can understand that not only providing violence prevention system but also establishing a suitable support program or system is essential. It is needed that a practical violence prevention program and a support system reflecting different general characteristics in this study such as gender, location and age be provided.
