Purpose : The effects of training posture on respiratory muscle function were investigated on the paretic and non-paretic sides in stroke patients undergoing respiratory muscle training. Methods: A total of 51 patients who were diagnosed with stroke and received inpatient treatment at general and rehabilitation hospitals were recruited and assigned to three groups(neutral posture group=17, side bending posture group=17, and habitual posture groups=17) by simple random sampling. Pulmonary function, respiratory muscle thickness and movement, and respiratory muscle activity were measured prior to, 3 weeks after, and 6 weeks after intervention. respiratory muscle strength and lung function were measured for pulmonary function; diaphragm and parasternal intercostal muscles were measured for respiratory muscle thickness and movement; and diaphragm, external intercostal muscles, and sternocleidomastoid muscles were measured for respiratory muscle activity. The groups received the same therapeutic exercise 30 minutes/time to be completed 5 times/week during the intervention period, and also had additional respiratory muscle training using an inspiratory muscle trainer. Each group was asked to take neutral, side-bending, and habitual postures, in which respiratory muscle training was performed 30 minutes/day, 10 sets repeating 10 times per set, 3 times/week for a total of 6 weeks. Data were analyzed with a repeated measured ANOVA at the significance level of 0.05. Results: Regarding maximal inspiratory pressure(MIP), maximal expiratory pressure(MEP), and forced vital capacity(FVC), a significant increase was detected in the duration of training between the groups, but not in the effects between training postures. As for diaphragm thickness at total lung capacity and diaphragm thickness ratio(TR) on paretic and non-paretic sides, and parasternal intercostal muscle movement of the paretic side, a significant increase was detected in the duration of training. asymmetry ratio of parasternal intercostal muscles(Asym PSm) significantly decreased. In the neutral posture group, diaphragm thickness and thickness ratio between paretic and non-paretic sides, and parasternal intercostal muscle movement on the paretic side significantly increased in comparison to the other groups. Sternocleidomastoid muscle activity on the paretic side significantly increased in the side bending posture group during the duration of training, whereas sternocleidomastoid muscle activity on the non-paretic side significantly increased in the habitual posture group, but not between training postures. Conclusion: To improve respiratory function and increase respiratory muscle thickness and movement in stroke patients while keeping the asymmetry ratio of the sternocleidomastoid muscles, it is necessary to perform respiratory muscle training in a neutral posture.
