Objective : The current social environments in which requires high quality of nursing services in accordance with the development of living standards and medical technologies act as a new stress source for nurses. As psychiatric mental health nurses use themselves as a therapeutic tool, their stress level is very high. Thus, stress management for psychiatric mental health nurses is more critical. The purpose of this study was to identify the level of job stress of psychiatric nurses, maintain the health of them, and to provide quality nursing service by identifying the influenced factors such as resilience, communication competence, and nursing practice environments on job stress among psychiatric mental health nurses.
Methods : This study was a descriptive correlation study and included 146 psychiatric mental health nurses who worked at twenty-two psychiatric clinics and academic medical centers in Busan Metropolitan City and Gyeongsangnam-do from August 21 to September 16, 2017. There were 4 research tools, including questions about job stress, communication competence, resilience, and nursing practice environment. The collected data was analyzed by using descriptive statistics, t-test for independent samples, one-way ANOVA, Pearson's correlation coefficient and multiple linear regression analysis according to the purpose of the research, with SPSS WIN 23.0
Results : The results of the study are as the following.
1. Most of the subjects were females with 87.7%, and average age was 33.16 years. In terms of working hospital, mental hospitals were the highest with 83.6%, and the average hospital work experience was 7.08years. The average psychiatric work experience was 5.67 years. Only 21.9% of those with psychiatric-related certifications, and 75.3% were general nurses. 85.6% of the psychiatric mental health nurses chose psychiatric on their own wills, and 54.1% did not have received additional training for improving stress management.
2. The average degree of job stress of the subjects was 2.40(0.48)(on a 1 to 4 scale), and the average degree of resilience was 2.51(0.52)(on a 0 to 4 scale), the average degree of communication competence was 3.56(0.44)(on a 1 to 5 scale), the average of practice environment of nursing work was 2.98(0.62)(on a 1 to 5 scale).
3. According to the analysis results of the job stress differences based on general characteristics, the stress level was significantly higher in females than males (t=-2.26, p=.025), and in nurses working at academic medical centers than those working at psychiatric clinics (t=-2.17, p=.031). In addition, the stress level was significantly higher in nurses with psychiatric certificates than those who did not hold the certificates (t=-2.43, p=.016). The resilience level was significantly higher in charge nurses than that of general nurses and head nurses (F=6.43, p=.002). Furthermore, the level of communication competence was significantly higher in age groups of 20~30s (F=4.25, p=.016) and in those with less than 6 years and greater than 8 years of workin experience in hospitals (F=3.35, p=.012), compared to the groups of 40s and those with 6~8 years of working experience, respectively.
4. After calculating the correlation among the job stress, resilience, communication competence, nursing practice environment, job stress was shown to have a statistically significant negative correlation with resilience (r=-.23, p=.005), nursing practice environment (r=-.21, p=.011). As well, resilience was shown to have a statistically significant positive correlation with nursing practice environment (r=.67, p<.001).
5. The influence factors the showed statistically significance on job stress among psychiatric mental health nurses were type of working hospitals (β=.16, p=.036), psychiatric mental health nursing certificates (β=.22, p=.005), hardiness among sub-factors of resilience (β=-.25, p=.005), and sufficient workforce and job support among sub-factors of nursing practice environment (β=-.26, p=.025). The explanatory power of these factors for job stress was 20.3%. Conclusion : This study showed that job stress among psychiatric mental health nurses is inversely correlated with the level of resilience and the nursing practice environments. It was also shown the hardiness, sufficient workforce and job support, type of working hospitals, and the certificate of psychiatric mental health nursing significantly impacted job stress among psychiatric mental health nurses. Therefore, to lower the job stress of psychiatric mental health nurses, it is necessary to provide opportunities for competence improvement certification and education, which can help the nurses feel pride as healthcare professionals. In addition, by developing a program the can improve hardiness, it is needed to enhance the nurses' ability to handle various changes and environments in an effective manner. The systemic organizational supports by hospitals with sufficient workforce are also needed.
