Gauzes and cotton swabs contaminated with blood at dental clinics are potential hazards that pose the risk of infection, induce biological and chemical environmental contamination, and cause problems in the process of biological sewage treatment. Therefore, they must be disposed of in accordance with the waste management law. However, gauzes and cotton swabs placed in patients’ oral cavities during tooth extraction or surgery are often never collected, and are inappropriately disposed of outside dental clinics. Therefore, the present study was conducted with the aim of preventing infection and environmental contamination by suggesting proper methods of medical waste disposal, and providing education and increasing awareness about medical waste disposal to improve the current management of medical waste in dental clinics.
Of the patients who visited A dental clinic in Busan, and B dental clinic located in Cheongju-si, Chungcheongbuk-do, those who underwent invasive procedures including tooth extraction, implant placement, periodontal surgery, and oral surgery, or compression using gauzes or cotton swabs in the two months between June 2017 and July 2017 were included. Researchers visited each dental clinic to explain about the purpose of this study, and receive consent from the head dentist before beginning the investigation.
12.6% of all patients in A and B dental clinics were subjected to compression using gauzes and cotton swabs following an invasive dental procedure, and 7.3% of these patients had their gauzes and cotton swabs collected at the dental clinics.
The rate of gauze/cotton swab retrieval was 8.8% for men. The rate of gauze/cotton swab retrieval by age was 13.7% for patients aged 60-89 years. The rate of gauze/cotton swab retrieval by the type of medical treatment was high at 18.2% for oral surgery.
75.7% of the patients, whose gauzes and cotton swabs were not collected at their dental clinics, disposed of them in trash cans at home. Over 80% of females and those aged 40-59 years disposed of their gauzes and cotton swabs using the same method. 87.5% of all patients also disposed of gauzes and cotton swabs in trash cans at home following periodontal surgery.
Only 1.8% of all patients responded that they knew the proper way to dispose of gauzes and cotton swabs contaminated with blood. Of these, 3.7% were female, and 2.7% were aged 40-59 years. The rate of giving such a response was low among patients who underwent simple and refractory tooth extraction, and those who underwent implant placement.
In this study, we confirmed that gauzes and cotton swabs contaminated with blood are inappropriately disposed of outside dental clinics. We believe that this is due to the lack of education on medical waste disposal in the general population, and the lack of emphasis on the importance of medical waste disposal outside dental clinics.
Therefore, we suggest the need for the establishment of guidelines for proper disposal of gauzes and cotton swabs contaminated with blood, and thee education on medical waste produced in dental clinics for the general population.
