The study is aimed to compare and analyze the health condition of older adults living in 16 regions of South Korea by different income levels and equity in their health care use. It used the Korea National Health and Nutrition Examination Survey (KNHANES) Ⅴ-1, 2 and involved 2,915 adults aged 65 or older. The concentration index (CI) and Le Grand index were applied to the analysis. The result of this study was as follows. First, health equity of older adults with different income levels by regions was analyzed with the CI. The analysis found that the CI for the number of chronic diseases, limited activity of daily living and subjective health condition was negative in Busan, Daegu, Incheon, Gangwon, Jeonbuk, Jeonnam, Gyeongbuk and Gyeongnam. This suggested that the health condition of the low income class was worse than that of the high income class. Whereas, the CI for all the variables was found positive in Gwangju and Chungbuk, meaning the high income class had a poorer health condition than the low income class. Second, equity in the health care use by older adults with different income levels by regions was analyzed with the CI. The analysis showed that the CI for the number of visiting outpatient clinic and hospitalization and the cost of visiting outpatient clinic and hospitalization to measure the equity in the health care use was negative in Daegu, Incheon, Gangwon and Gyeongbuk. This implied that the low income class used health care more frequently. On the other hand, Seoul and Daejeon recorded a positive CI for all variables, meaning that the high income class used health care more frequently. Third, equity in the health care use reflecting the health condition was analyzed by regions with Le Grand index. The low income class living in Daegu, Gangwon and Chungbuk visited outpatient clinics more frequently while those in Incheon, Ulsan, Gyeonggi, Chungnam and Jeonbuk were hospitalized more than in other regions. The high income class in Ulsan visited outpatient clinics more frequently and were hospitalized more in Daejeon, Chungbuk and Jeonnam. In the meantime, it was surveyed that the low income class spent more outpatient cost in Daegu, Chungbuk and Chungnam and more hospitalization cost in Incheon, Gwangju and Jeonbuk. However, the high income class spent more for visiting outpatient clinics in Seoul, Busan, Daejeon, Ulsan, Gyeonggi, Jeonbuk and Gyeongnam and for hospitalization in Seoul, Daejeon, Ulsan, Chungbuk and Chungnam. The study result indicated that the health condition of the older adults with different income levels and the characteristics of their health care use varied by regions. Therefore, each local government should establish policies to support the older adults that can reflect such differences by income levels and regions.
