Background and objectives Regardless of comments related to severity of side effects and safety risks of herbal medicines commonly used in food and drugs, and the continuous suggestion of reclassification of items, national health remains at risk because clear discussions regarding renewal are not being carried out. This study aims to systematically review the clinical sign of side effects and clinical reporting, and to find improvement plan for enhancing safety about herbal medicines used as both food and medicine. Methods On five domestic databases (KISS, KMbase, KoreaMed, NDSL, and RISS) regarding age, sex and health status, reports of side effects when taking herbal medicines common in food and drug were studied. The search expressions are as follows: #1: herb* OR traditional OR health AND #2: side effect OR adverse OR toxic OR hepatitis OR liver injury OR liver failure OR nephritis OR kidney injury OR renal injury OR kidney failure OR renal failure OR anaphylaxis. The characteristics of the database were adjusted and the search period was June 2014. In the review of domestic studies about herbal medicine common in food and drugs, particularly herbal medicines reported to have caused death, the side effects were identified and assessed using 10 databases. The databases include EMbase, MEDLINE, Cochrane library, CINAHL, CNKI, KISS, KMbase, KoreaMed, NDSL and RISS. Keywords are scientific names and synonyms, the search period was June 2014, and the process was performed by two experts and in steps. In-depth interviews using a semi-structured questionnaire were carried out with seven experts in herbal medicines. Raw data were encoded in a document using the direct recording and categorized statements which were perused several times. The result of the analyzed interviews was sent via email, then the participants’ opinions were reviewed and validity was verified using feedback from colleagues. Results A total of 33 articles were selected from the domestic databases by steps, noting the poor level of evidence regarding case reports and retrospective researches. Herbal medicines commonly used in food and drug reported to have side effects are as follows: Licorice, Cervi parvum cornu, Umi cortex, Angelica gigas, Platycodon grandiflorum, Cornus officinalis, Aloe, Ganoderma lucidum, Acanthopanax sessiliflorum, Ginseng (red ginseng), Artemisia capillaris, Paeonia lactiflora, Pueraria lobata, Polygonum multiflorum, Hovenia dulcis and Scutellaria baicalensis. Other herbal medicines used as both food and medicine that could have resulted in emergency surgery and liver transplant totaled nine. These include Ulmi cortex, a health product that contains Licorice and Aloe, Artemisia capillaris, Polygonum multiflorum, and Hovenia dulcis. Among them, three cases that caused death were Artemisia capillaris and Polygonum multiflorum. After performing systematic reviews by steps about the side effects of Polygonum multiflorum and Artemisia capillaris, a total of 59 cases of adverse effect of Polygonum multiflorum were found in 17 articles in seven out of 11 domestic and foreign articles. All the reported side effects of taking Polygonum multiflorum involved liver injury. The reported cases of side effects were addressed using conservative treatment, but there was one liver transplantation case and one death in the foreign database. In the clinical research on combination preparation containing Polygonum multiflorum, mild symptoms were noted but no other side effects were observed. Five out of 7 articles that were selected by systematic review and involving Artemisia capillaris were about combination preparation taking Inchin-ko-to. Inchin-ko-to was taken for over one year on three cases while on four cases, the treatment contained Inchin-ko-to and Artemisia capillaris. There were two cases about toxic hepatitis (one was drug induced liver injury, the other one was random self medication), and relief of the symptoms in those cases became apparent after treatment. After interviews with experts, six domains (conception of herbal medicines used as both food and medicine, problem of selection of herbal medicines used as both food and medicine, problem of management system of herbal medicines used as both food and medicine, problem of food containing herbal medicines used as both food and medicine, revaluation and reclassification of herbal medicines used as both food and medicine, modification system related to herbal medicines used as both food and medicine) were exposed. The experts suggested the reclassification/revaluation of herbal medicine commonly used in food and drugs, establishment of an organization to monitor use of herbal medicines, unification of management organizations related to herbal medicines, and establishment of a reporting system of herbal medicines’ side effects. Conclusions The results of this study can be used as basis of clinical symptoms and reports of adverse effect in the accreditation process of food ingredients. Polygonum multiflorum and Artemisia capillaris, which were found to cause death, should be excluded from herbal medicines commonly used in food and drugs. Also, herbal medicines used as both food and medicine reported to have side effects should be reviewed through toxicity studies and herb clinical study. Items found to be hazardous to health should be excluded from herbal medicines commonly used in food and drugs. Guidelines can also be offered to promote safety. Finally, we should establish a reporting system for adverse effects of medicinal plant and food, enact a law to manage use in food and drugs, and enhance the safety of herbal items.
