SCORing Atopic Dermatitis (SCORAD) index를 이용한 중증도와 비타민 D 섭취량을 일부 아토피피부염 대상자에서 측정하였다. 식품 항원에 대한 감작은 감작 항원의 수(무감작, 단일감작, 다감작) 와 감작 정도(미검출, 저농도, 고농도)에 따라 분류하였다. 결과: 25(OH)D 농도는 항원 감작 수(p=0.006)와 정도(p=0.005)에 따른 군의 분류에서 유의한 차이를 발견하였다. 다감작 군에서 무감작 군(p=0.001)과 단일감작 군(p=0.023)에 비해 의미 있게 낮은 25(OH)D 농도를 보였다. 고농도 감작 군에서도 미검출 군(p=0.005) 과 저농도 군(p=0.009)에 비해 의미 있게 낮은 25(OH)D 농도를 보였다. 비타민 D가 결핍하면 항원 감작의 위험도(OR 5.0, 95%CI 1.8-14.1)가 증가하였는데, 특히 우유(OR 10.4, 95% CI 3.3-32.7)와 밀(OR 4.2, 95% CI 1.1-15.8)에서 증가하였다. SCORAD index 는 감작의 농도를 보정한 후에도 25(OH)D 농도와 독립적으로 연관성(p=0.031, adjusted R2=0.112)을 보였다. 결론: 비타민 D 결핍이 식품 항원 감작의 위험도를 증가시키고, 비타민 D 결핍 영아에서 아토피 피부염 증상이 더 심할 수 있음을 시사한다.

Objective: To investigate the association between serum vitamin D levels, sensitization to food allergens and the severity of atopic dermatitis in infants. Methods: We investigated serum 25-hydroxyvitamin D (25(OH)D) and specific IgE levels to common or suspected food allergens in 226 infants with atopic dermatitis or food allergy. The severity of atopic dermatitis by the SCORing Atopic Dermatitis (SCORAD) index and amount of vitamin D intake was measured in subcohort children. Sensitization to food allergen was categorized by the number (non-, mono- and poly-) of sensitized allergens and the degree (undetected-, low- and high-level) of sensitization. Results: Significant differences in 25(OH)D levels were found between groups on number (p=0.006) and degree (p=0.005) of food sensitization. The polysensitization group had significantly lower levels of 25(OH)D than the nonsensitization (p=0.001) and monosensitization (p=0.023) group. High-level sensitization group had also significantly lower 25(OH)D levels when compared to undetected (p=0.005) and low-level (p=0.009) group. Vitamin D deficiency increased the risk of sensitization to food allergens (OR 5.0, 95%CI 1.8-14.1), especially to milk (OR 10.4, 95% CI 3.3-32.7) and to wheat (OR 4.2, 95% CI 1.1-15.8).
