Spine, hip, wrist, and humerus fractures are defined as major fractures, and pelvic bone, sacrum, ribs, distal femur and distal humerus and ankle fracture are defined as additional fractures. Most fractures are associated with low BMD levels. Fracture in an elderly population is not only difficult to surgically treat but also lowers the quality of life. The prevalence of osteoporotic fracture in an elderly population depends on the site and type of fracture, but there are few studies on the relationship between osteoporotic fracture and fracture type in ankle fracture. In this study, we investigated the relationship between fracture type and osteoporotic ankle fracture in elderly population. METHODS - Among the patients with trimalleolar fractures, 115 patients who underwent open reduction and intramedullary fractures were studied. All patients underwent pre-operative CT and measured the mean bone attenuation of the distal tibia on CT image. In order to evaluate the severity of fracture, the fracture fragment size, fracture surface step-off length, and comminution degree were measured. RESULTS - There was a significant relationship between the increase in age and the decrease in bone attenuation (P<0.001). The degree of comminution decreased with increasing bone density(P=0.047), the size of the fracture was also reduced (P=0.005). As the bone shadow increased, the pre-operative step-off length decreased (P=0.042). Male and female patients were divided into two groups and analyzed the relationship between bone morphology and fracture patterns. The difference was significant. CONCLUSIONS - In older patients, distal tibial bone attenuation was decreased, and in patients with decreased bone attenuation, the fracture pattern was significantly worse.
