본 연구는 서울시 거주하는 65세 이상 고령자를 대상으로 건강·복지서비스를 제공하는 직업군의 직접적인 경험을 통한 인식을 통해 취약계층 고령자의 식생활 및 현제 진행되고 있는 식품·영양서비스의 실태를 파악하고, 지역사회 맞춤형 영양관리 방안을 도출하기 위한 목적으로 실시되었다. 설문 대상은 서울시 25개 자치구 동주민센터에서 근무하는 찾동간호사, 복지플래너, 보건소에서 근무하는 방문건강관리간호사와 영양사이며, 총 277명이 설문에 참여하였으나, 응답이 불충분한 36부를 제외한 241부를 분석에 이용하였다. 본 연구의 결과를 요약하면 다음과 같다.
1. 설문에 응답한 건강·복지서비스 담당자는 89.2%가 여성이었고, 연령은 40세 이상 64.4%로 연령층이 높았다. 담당 직무는 간호사(찾동·방문건강관리)가 54.3%, 복지플래너 38.6%, 영양사 2.5%의 순이었다. 현재 직위에서의 근무 경력은 3년 미만이 71%, 3년 이상이 28.8%로 나타나 해당 직무에 대한 경험 또는 해당 직군의 근속년도수가 장기적인지 않은 것을 알 수 있었다.
2. 취약계층 고령자의 식생활 실태에 대한 건강·복지서비스 담당자의 인식은 ‘식품/식재료 구입비용을 부담스러워한다’, ‘치아상태가 나빠 먹지 못하는 식품/식재료가 많다’가 각각 3.8점으로 가장 높았고, ‘식품을 다양하게 골고루 먹는다’, ‘가정에는 먹을 수 있는 식품/식재료가 충분하다’가 각각 2.1, 2.2로 낮았다. 또한 ‘음식을 짜게 먹는다(3.6점)’, ‘젓갈류, 장아찌 등 절임음식을 위주로 식사를 한다(3.5점)’가 보통에 해당하는 3점을 상회하는 점수로 높게 응답하였다. 직무의 특성상 고령자의 건강관리를 하는 간호사는 ‘음식을 짜게 먹는다(3.8점)’, ‘젓갈류, 장아찌 등 절임음식을 위주로 식사를 한다(3.6점)’으로 복지플래너에 비해 더 문제로 인식하였다(p<0.001, p=0.006). 3. 지역사회 취약계층 고령자에게 제공되는 도시락 및 밑반찬배달서비스 실태에서는 고령자의 안부 확인에 도움이 되는 것으로 나타났다. 반면 메뉴의 선택권과 메뉴가 고령자의 질병 상태에 적합하지 않는 문제점이 있었다. 직무에 따라 분석한 결과, 도시락배달서비스는 ‘도시락 메뉴가 어르신 기호에 적합하다(2.8점)’, ‘도시락 메뉴가 어르신의 질병 상태에 적합하다(2.5점)’, ‘어르신이 직접 메뉴를 선택할 수 있다(1.6점)’에서 간호사가 복지플래너 보다 낮게 평가 하였다(p=0.015, p=0.001, p<0.001). 이 외에 도시락배달서비스는 ‘주말이나 공휴일에 제공된다(2.4점)’가 보완되어야 할 것으로 나타났고, 밑반찬배달서비스는 ‘어르신의 씹는 기능을 반영한다’에서 간호사가 2.7점으로 낮게 평가 하였다(p<0.001).
4. 취약계층 고령자에게 제공되는 식품제공과 식품바우처의 실태에서는 서비스가 정말 필요한 어르신에게 제공되고 있는 것으로 나타났다. 반면식품제공에서 ‘어르신이 필요할 때 식품/식재료를 제공 받을 수 있다’가 2.5점으로 필요시 제공받지 못하는 어려움 있었다. 식품바우처는 ‘금액을 다 사용하지 못하고 남는다’가 2.6점으로 남는 금액에 대해서는 큰 문제가 되지 않는 것으로 나타났다. 직무에 따라 분석한 결과, 식품제공 문항에서 ‘제공 식품/식재료가 어르신의 질병 상태에 적합하다(2.7점)’, ‘어르신이 원하는 식품을 선택 할 수 있다(2.5점)’, ‘어르신이 필요할 때 식품/식재료를 제공 받을 수 있다(2.5점)’는 간호사와 복지플래너에서 의견의 차이가 나타났다(P<0.001). 반면 식품바우처의 문항은 직무에 따른 분석에서 건강·복지담당자의 의견의 차이가 거의 없이 일치하였다.

5. 취약계층 고령자의 12 가지 유형 별로 적합한 식품·영양서비스의 적합성 문항은 독거어르신, 80세 이상 어르신, 중증질환이 있는 어르신, 거동이 불편하신 어르신, 심리적으로 고립된 어르신은 ‘식사배달서비스’가 가장 적합한 것으로 응답하였고, 영양교육서비스는 ‘방문영양상담서비스’가 적합한 것으로 평가되었다. 어르신 부부, 만성질환이 있는 어르신, 주변 지인에게 가끔 음식을 받는 어르신은 ‘식품지원서비스’와 ‘방문영양상담서비스’가 적합하였고, 다른 가족을 부양하는 어르신의 경우 ‘식품지원서비스’가 가장 많았으나, ‘조리교육서비스’가 함께 필요한 것을 알 수 있다. 경제적으로 어려움이 있는 어르신은 ‘식사배달서비스’와 ‘식품지원서비스’가 영양교육서비스에 비해 적합하였고, 반대로 식생활정보가 부족한 어르신, 잘못된 정보로 건강적 피해 위험이 있는 어르신의 경우 ‘방문영양상담서비스, 조리교육서비스(실습), 영양교육서비스(이론)’와 같이 영양교육서비스가 적합한 것으로 평가되었다.
6. 취약계층 고령자에게 적합한 식품의 특성은 ‘어르신의 씹는 기능을 반영한 제품’, ‘영양보충이 가능한 제품’, ‘어르신의 질환관리에 도움이 되는 제품’이 4.2점으로 공통으로 가장 높았다. 10문항에 대한 분석 결과 모든 문항이 4.2∼3.8점으로 보통(3점)이상을 상회하는 것으로 나타났으며, 중요도에 따라서는 고령자의 신체적 특성〉비용적 특성〉이용이 편리한 특성〉고령자의 기호에 따른 특성〉활용적인 특성 순이었다.
7. 취약계층 고령자 대상 식생활교육 운영 시 발생되는 장애요인 문항은 전체적으로 보통(3점) 이상으로 ‘어르신은 한 번의 교육으로는 이해시키기 어렵다(3.8점)’으로 가장 높았고, ‘어르신 식생활교육을 위한 지원이 부족하다’, ‘어르신 식생활교육을 위한 시설이 부족하다’의 문항은 각 3.7점으로 가장 큰 어려움으로 응답하였다. ‘내 자신이 업무량이 많아 시간이 부족하다(3.5점)’, ‘내 자신이 어르신의 다양한 질환에 대한 전문지식이 부족하다(3.2점)’는 교육자가 느끼는 장애요인으로 두 문항 모두 직무에 따른 차이가 있었다. 특히 ‘내 자신이 어르신의 다양한 질환에 대한 전문지식이 부족하다’ 문항의 경우, 간호사 직무에서 3.0점으로 보통에 해당함에 반하여 복지플래너 직무에서는 3.6점으로 어려움이 있는 것으로 나타났다.
8. 취약계층 고령자 대상 식품·영양상서비스가 지속적으로 운영되기 위한 조건에서는 개인에게 필요한 맞춤형 지원과 대상자 선정 기준의 타당성이 가장 중요한 것으로 나타났다. ‘개인에게 필요한 맞춤형 지원’, ‘ 효과적으로 이용되고 있는지 모니터링’, ‘가능한 최대 인원의 어르신에게 지원’은 복지플래너보다 간호사가 더 중요하다고 응답하였다(p=0.017, p=0.002, p=0.022).
9. 연구의 결과를 토대로 지역사회 취약계층 고령자를 위한 식품영양관리모델은 1) 방문건강관리(대상자 발굴 및 사례 공유, 사전 평가), 2) 식품·영양서비스지원 3) 모니터링 및 평가(대상자 모니터링, 건강 및 영양 상태 평가), 4) 지역사회연계(인력 및 시설, 식사 및 건강 관리 프로그램)까지 4 단계로 제시하였다. 제시된 취약 계층의 고령자를 대상으로 제안된 맞춤형 식품영양관리모델은 대상자인 취약계층 고령자, 방문건강관리, 식품영양관리를 제공하는 기관 및 모든 과정과 연계되는 지역사회까지 3 가지 요소의 상호작용을 통해 이루어지므로 이들이 각자 위치에 맞는 역할과 임무를 제시하였다. 이 모델의 최종 목적은 지역사회 취약 계층 고령자의 안전한 식생활과 건강을 개선하는 것이다.
이상의 결과를 볼 때, 현재 취약계층 고령자를 대상으로 다양한 식품·영양서비스를 제공하고 있지만 각각의 서비스별 문제점들이 인식되고 있었으며, 이를 보완하여 개선할 수 있는 방안을 모델로 제시하였다. 앞으로의 취약계층 고령자 대상 식품·영양서비스는 고령자 개개인에 적합한 맞춤형 식품·영양서비스 형식으로 제공되어야 하며, 대상자 발굴 및 선정부터 최종평가에 이르기 까지 타당성 있는 평가도구를 활용하여 다차원적인 평가가 이루어 져야 할 것으로 사료된다.

The purposes of this study were to identify the dietary life of the vulnerable older adults, to investigate food and nutrition service practices targeting the older adults, and to develop a customized nutrition management model for vulnerable older adults residing in community. In order to achieve the objectives, a survey was conducted with health and welfare service providers in seoul. A total of 277 nurses, dietitians, and social workers participated in the survey and 241(response rate 87%) responses excluding significant missing data were used to analysis. The survey showed that 89.2% of health and welfare service providers were female, and 64.4% are over 40 years old. The respondents were nurses(54.3%), social workers(38.6%), and dietitians(2.5%). Seventy one percent of the respondents worked for less than 3 years. In terms of dietary life of the vulnerable older adults, they perceived that the cost of purchasing food is burdensome to older adults(3.8/5.0 point), and poor dental conditions prevent older adults from eating various foods(3.8/5.0 point). The statements ‘older adults eat various foods(2.1/5.0 point)’ and ‘older adults have enough food at home(2.2/5.0 point)’ were rated low. Nurses rated higher than social workers for ‘older adults tend to eat salty foods(3.8/5.0 point)’ and ‘older adults eat salty fermented foods such as jeotgal and jangajji(3.6/5.0 point)’(p<0.001, p=0.006). The health and welfare service providers perceived that lunch box and side dish delivery services are effective for checking on older adults but rated low for availability of menu choices. Nurses rated lower than social workers for the statements, ‘the meal menus meet the older adult's preference’, ‘the menus are suitable for disease conditions of the older adults’, and ‘the older adults have choices to select menus’. The health and welfare service providers answered that products that reflect the chewing abilities of the elderly, products supplemented with nutrients, and products that help disease management for the elderly were suitable for the older adults. In conducting nutrition education for the older adults, they perceived ‘repeated education is needed’ as the biggest barrier, followed by ‘there is a lack of support for elderly dietary education’, and ‘there is a lack of physical environment for elderly dietary education.’ The health and welfare service providers perceived that providing personalized service and selecting service recipients by valid criteria were the important components for the food and nutrition service model for vulnerable older adults, followed by regular monitoring of the service and connecting older adults with community networks. The purposes of the proposed model were to improve the safe dietary habits and health of vulnerable older adults in community. The results of this study suggest that the model of customized food and nutrition management for vulnerable older adults in community is presented in step 4: 1) identifying and screening service beneficiaries, 2) providing customized food and nutrition service, 3) monitoring and evaluation, 4) connecting the recipients with other community services. The proposed customized food nutrition management model consists of four stakeholders : older adults, clinical dietitians, visit-nurses, and others in community and presents roles and tasks that fit into their positions. This study suggested that food and nutrition services for vulnerable people should be provided in a customized for each of the elderly. It is considered that a multidimensional assessment should be undertaken using reasonable assessment tools from discovery and selection of the vulnerable people to final assessment.
The purposes of this study were to identify the dietary life of the vulnerable older adults, to investigate food and nutrition service practices targeting the older adults, and to develop a customized nutrition management model for vulnerable older adults residing in community. In order to achieve the objectives, a survey was conducted with health and welfare service providers in seoul. A total of 277 nurses, dietitians, and social workers participated in the survey and 241(response rate 87%) responses excluding significant missing data were used to analysis. The survey showed that 89.2% of health and welfare service providers were female, and 64.4% are over 40 years old. The respondents were nurses(54.3%), social workers(38.6%), and dietitians(2.5%). Seventy one percent of the respondents worked for less than 3 years. In terms of dietary life of the vulnerable older adults, they perceived that the cost of purchasing food is burdensome to older adults(3.8/5.0 point), and poor dental conditions prevent older adults from eating various foods(3.8/5.0 point). The statements ‘older adults eat various foods(2.1/5.0 point)’ and ‘older adults have enough food at home(2.2/5.0 point)’ were rated low.
