This study is a descriptive research to provide basic data needed to develop programs to create a safe working environment and improve the quality of professional life via identifying the correlation between violence experience, social support, and professional quality of life for operating nurses. The study was conducted on 133 operating nurses, excluding those with less than three months of experience in the operating room and head nurse at two senior hospitals in the B metropolitan city. The data collection period was from May 18, 2018 to May 25, 2018, using a self-reporting questionnaire. The collected data were analyzed with technical statistics, t-test, ANOVA, and Scheffé test Pearson’s correlation coefficient using the SPSS/WIN 23.0 program.
The results of the study were as follows :
1. Gender were 88.0% female, average age was 28.4, 82.7% single, 87.2% four-year college graduates, and 66.2% without religion for the subject. The average working experience was 5.0 years, 4.8 years working in the current department, 80.5% working for regular nurses and 76.7% for three shifts. 42.1% of the subject experienced violence in hospital. among them, 60.7% of the most violent types of surgery had regular surgery. Among the types of violence perceived as a bigger injury, verbal violence accounted for 91.1%. 67.7% of the respondents said there is a method for treating violence reports in hospitals, 64.5% said they prefer to report violence to department heads. The average number of classes for preventing violence was 0.85.
2. The participants' experience of violence were 94.7% of verbal abuse on a weekly basis, 73.7% of physical threats on a monthly basis, and 12.89% of physical violence on a yearly basis. The score of social support was 3.49±0.75 points from fellow nurses and 3.16±0.88 points from senior nurses out of 5 points. The score for each sub-area of the professional quality of life was 26.28±0.75 points for compassion satisfaction, 29.69±0.51 points for exhaustion, and 22.46±0.77 points for secondary traumatic stress out of 50 points. The results categorized as a combination of the sub-concepts for the professional quality of life presented by Stamm(2010) were that 14.3% of those with the high quality of professional life, 2.3% of those with the low quality of professional life, and 10.5% with the highest risk of exhaustion.
3. The experience by perpetrators and by type of violence based on the general characteristics of the subject was significantly different 1. from that of the position and current department. The verbal abuse of doctors differed significantly in their positions(t=2.64, p=.009). The verbal abuse(t=2.44, p=.016) and physical(t=3.56, p=.001) of their senior nurses also differed significantly in their positions threat respectively. The verbal abuse of a fellow nurse was a significant difference in the current department's work experience(F=2.71, p=.047), position(t=2.13, p=.035). The physical threat of a fellow nurse differed significantly in her position(t=2.79, p=.006).
2. The social support of fellow nurses was significantly different depending on the total clinical experience(F=2.84, p=.040) in social support according to the general characteristics of the subject. There was no significant difference in the social support of the senior nurses according to the general characteristics.
3. The differences in compassion satisfaction, exhaustion, and secondary trauma stress, sub-area of the professional quality of life according to the general characteristics of the subject, are as follows. Compassion satisfaction was a significant difference depending on age(t=4.32, p=.015), marital status(t=-2.59, p=.011), total clinical experience(F=3.92, p=.010), current department experience(F=3.22, p=.025), and position(t=-3.36, p=.001). Exhaustion was significantly different depending on age(F=3.28, p=.041), marital status(t=-2.59, p=.011), total clinical experience(F=6.78, p<.001), and current department experience(F=6.43, p<.001).
4. The subject's violent experience, social support, and the professional quality of life related relationships are as follows. Compassion satisfaction, a sub-division of the professional quality of life, had a negative correlation with exhaustion(r=-.39, p<.001), and a positive correlation with a secondary trauma stress(r=0.34, p<.001). Compassion Satisfaction also had a positive correlation with a social 1. support of fellow nurses(r=.39, p<.001) and senior nurses(r=.42, p<.001). Exhaustion, a sub-division of the professional quality of life, had a positive correlation with secondary trauma stress(r=.46, p<.001). Exhaustion also had a positive correlation with the experience of violence, verbal abuse(r=.40, p<.001), physical threats(r=.35, p<.001) and physical violence(r=.21, p=.014). Exhaustion had a negative correlation with social support from senior nurses(r=-.17, p=.046). Secondary traumatic stress, a sub-division of the professional quality of life, had a positive correlation with the experience of violence, verbal abuse(r=.23, p=.007), physical threats(r=.21, p=.015), and physical violence(r=.29, p=.001).
As a result of this study, operating room nurses experienced 94.7% verbal violence, 73.7% physical threats, and 12.8% physical violence. Such violent experiences have increased stress on secondary trauma and exhaustion, the sub-area of the professional quality of life. In addition, social support from both the senior nurses and fellow nurses has increased compassion satisfaction, a sub-division of the professional quality of life, and especially the boss's social support has reduced exhaustion. Therefore, the professional quality of life can improve by creating a preventive education that can reduce the violent experience of operating nurses and an environment that can enhance social support of supervisors and fellow nurses. Finally, it is believed that quality care can be provided through improving the professional quality of life.
