Depression is associated with suicide and it is very important to continue to take appropriate antidepressants in the treatment of depression. However, in the case of domestic depression, almost half of the patients stop taking the antidepressant medication within 30 days after the start of the treatment, and the adherence of antidepressant is very low in depressed patients. Previous studies have shown that income is an important factor on depression treatment and taking antidepressants. As income is an important factor in the treatment of depression, efforts are needed to identify differences in treatment of antidepressants by income level and to reduce these disparities. Nevertheless, there are not many studies on the relationship between income and depression treatment and antidepressant medication in Korea. The purpose of this study was to investigate the relationship between income level and antidepressant medication adherence in the outpatient depression patients by establishing a retrospective cohort based on national health insurance data. The sample consists of the 142,336 adult patients who received antidepressant prescription with diagnosis of depression between January 1 and December 31, 2012. Operation definition of adherence to antidepressant treatment was that medication was possessed 80% of the time during 3 months and 6 months(MPR ≥ 80%). Hierarchical Logistic regression analysis were used to investigate the relationship between income level and adherence of antidepressant. The adherence of antidepressant during the 3 month and 6 month were 22.64% and 15.13% of the depression patients. Therefore, the adherence of antidepressant is very low in domestic depressed patients. According to the results of this study, there was a statistically significant difference between income level and the 3-month and 6-month adherence of antidepressant in both the employee and self-employed insured. And the adherence of antidepressant with lower income levels was lower than those with higher incomes. In conclusion, there was a statistically significant difference in adherence of antidepressant by income level. Depression can lead to serious social problems such as suicide if left untreated. Therefore, it is necessary to continue research on causes of differences in the adherence of antidepressant and depression treatment rate according to income level, and efforts should be made to reduce the gap.
