At hospitalization, we rated clinical scales such as Clinical Global Impression-Severity (GCI-S), Drug Attitude Inventory (DAI), Contour Drawing Rating Scale (CDRS), Multidimensional Scale of Perceived Social Support (MSPSS) and patients’ demographic factors. We measured drug adherence using Medication Event Monitoring System (MEMS), pill count, and patients’ self-report upon outpatients visits 4 and 24 weeks after discharge.
Results Correlation between drug adherence and sex, age, education level, severity of symptoms, and the complexity of administration did not show statistical significance. There was a statistically significant negative correlation between drug adherence assessed 4 weeks after discharge and Contour Drawing Rating Scale (CDRS) difference score, which measures physical dissatisfaction (r=-0.282, p<0.05). A statistically significant positive correlation was also found between drug adherence assessed 24 weeks after discharge and Drug Attitude Inventory (DAI), which measures patients’ attitude towards their medication (r=0.383, p<0.01).
Conclusions Based on the findings of this study, patients’ attitude towards their medication and their degree of physical dissatisfaction influenced the post-hospitalization drug adherence in severe psychiatric patients. Management of active psychoeducation on patients’ attitude towards dissatisfaction of medication and body image seems to be crucial in the improvement of patients’ drug adherence.
