최근 우리 사회에 충격을 주는 범죄들로 인해 조현병은 많은 사람들이 관심을 가지는 정신질환이 되었다. 조현병은 전 세계적으로 인구 100명당 1명에게 찾아올 수 있는 비교적 흔한 정신질환이며, 발병 후 회복이 되기도 하지만 회복과 재발을 반복적으로 하는 것이 대부분이며 만성화로 이어지기 때문에 완치보다는 평생 관리가 필요한 질환이다(대한조현병학회, 2017). 조현병은 약물치료와 정신 재활을 병행하면 환자의 치료 효과가 높으며(박상규, 2006), 최근 정신건강복지법 시행으로 정신질환자들은 스스로 지역사회 복귀를 결정할 수 있게 되었다(보건복지부, 2017). 국내에서는 조현병 환자의 재활과 사회복귀를 위해 낮병원, 거주시설, 정신건강복지센터, 정신재활시설 등의 정신보건사업을 운영하고 있다. 이 중 정신재활시설은 이용자의 대부분이 조현병 환자로 이들의 사회적응을 위한 각종 훈련과 생활 지도를 실시하고 있으며(한국정신재활시설협회, 2018), 조현병 환자의 재활을 위한 심리 치료적 접근으로 집단미술치료의 수요가 늘고 있다. 따라서 본 연구는 점차 늘어나는 조현병 환자의 미술치료 체험에 대한 관심과 시대 변화에 따른 흐름을 반영하면서 정신재활시설을 이용하는 조현병 환자의 집단미술치료 체험이 어떠한 의미를 지니는지 현상의 체험을 이해하는 Max van Manen의 해석학적 현상학 연구방법을 적용하여 연구를 시도해보고자 한다. 본 연구의 목적은 정신재활시설을 이용하는 조현병 환자의 집단미술치료 체험의 의미와 본질을 이해하는 데 있다. 이는 정신재활시설을 이용하는 조현병 환자의 집단미술치료에 대한 학문적 기초자료를 제공하고, 더 나아가 미술치료 현장에 도움이 되고자 한다. 본 연구의 문제는 “정신재활시설을 이용하는 조현병 환자의 집단미술치료 체험은 어떠하며, 그 의미와 본질은 무엇인가?”이다. 이에 따라 본 연구에서는 정신재활시설을 이용하는 조현병 환자의 집단미술치료체험을 생생하게 이해하기 위해 Max van Manen의 해석학적 현상학적 연구방법론을 적용하였으며, 체험의 본질에 집중, 실존적 체험 탐구, 해석학적 현상학적 반성, 해석학적 현상학적 글쓰기의 4가지 활동으로 구성된 총 11단계의 연구단계를 수행하였다. 본 연구의 참여자는 총 10명으로 S시의 정신재활시설에서 최소 4개월에서 최대 9년 동안 집단미술치료에 참여한 경험이 있다. 연구참여자 10인의 조현병 진단 병력은 5년에서 34년까지 다양하며 남성 3명, 여성 7명으로 연령은 30대 1명, 40대 4명, 50대 5명이 구성되었다. 이들 연구참여자와의 심층 면담은 2018년 6월부터 2018년 11월까지 실시되었으며, 최소 1회에서 최대 2회의 인터뷰를 진행하였다. 연구 참여자의 동의하에 인터뷰 중 연구 참여자의 미술 작업을 요청하여 함께 수집되었으며, 모든 자료가 포화 된 시점에 자료 수집을 종료하였다. Max van Manen의 해석학적 현상학적 연구방법을 적용하여 자료 분석을 한 결과 <처음에는 집단미술치료에 관심이 없는 상태로 참여했지만, 점점 적극적으로 참여함>, <간단하고 쉬운 미술 작업을 통해 안도감을 느끼고 치료사의 시범과 설명이 편안함>, <미술 실력에 자신감이 없어서 스스로 부끄럽고 창피하게 느껴짐>, <집단미술치료를 통해 내면의 감정을 표현하는 즐거움과 매력을 느낌>, <동질감이 느껴지는 집단원과 함께 소통하고 교감하면서 따뜻함과 감사함을 느낌>, <미술치료사를 통해 자신을 긍정적으로 바라보며 집단미술치료 시간이 행복함>, <집단미술치료를 통해 자기를 표현하면서 집단원이 밝고 적극적으로 변화한다고 느낌>, <집단미술치료가 처음엔 치료가 될까 생각했지만 나를 사랑하게 되고 나에게 도움이 되었다고 느낌>, <처음에는 증상과 약 부작용으로 힘들었지만 집중하다 보니 우울감을 잊게 됨>으로 본질적 주제 9개와 하위 주제 27개가 도출되었다. 이 같은 연구결과와 실존적 자료 분석, 해석학적 현상학의 글쓰기, 논의를 바탕으로 정신재활시설을 이용하는 조현병 환자의 집단미술치료 체험은 다음과 같이 다섯 가지의 결론을 내렸다. 첫째, 정신재활시설을 이용하는 조현병 환자는 처음에 집단미술치료를 참여하라는 권유를 받아 의무적으로 참여하면서 증상과 약 부작용으로 어려움을 겪지만, 시간이 지날수록 집중할 수 있게 된다. 둘째, 정신재활시설을 이용하는 조현병 환자는 자신의 미술 실력에 자신감이 없어 부끄럽고 창피하다고 느끼며 쉽게 접근할 수 있는 간단하고 쉬운 미술작업을 선호한다. 셋째, 정신재활시설을 이용하는 조현병 환자는 집단미술치료가 처음엔 치료가 될지 의문을 가지지만 내면의 감정을 표현하며 스트레스가 해소되고 치료적 효과가 있다고 느낀다. 넷째, 정신재활시설을 이용하는 조현병 환자는 동질감이 느껴지는 집단원과 교감하면서 자신과 집단원이 점점 자신감이 생겨 적극적으로 변화하는 경험을 한다. 다섯째, 정신재활시설을 이용하는 조현병 환자는 자신에게 관심을 보여주며 이해하려고 노력하는 미술치료사를 통해 자신을 긍정적으로 바라보며 행복함을 느낀다. 이 같은 연구의 결론이 미술치료학의 학문적 측면과 미술치료 현장에 제공하는 의의와 기여점은 다음과 같다. 미술치료학의 학문적 측면에서 본 연구는 정신재활시설을 이용하는 조현병 환자들이 집단미술치료를 받으면서 느낀 심리적 체험을 Max van Manen의 해석학적 현상학적 연구방법을 통하여 생생한 언어로 담아내고 있다. 정신재활시설에서 실시되는 미술치료 현장에서는 미술치료사의 긍정적인 태도와 격려해주는 역할이 매우 중요한 것으로 나타났다. 이는 정신재활시설 종사자들뿐만 아니라 미술치료사들에게 조현병 환자들의 이해를 높이고 미술치료 현장에 대한 이해와 실제적인 도움을 주고 있다.

In short, the schizophrenia should be managed for a life-time rather than be fully treated. (Korean Society for Schizophrenia Research, 2017). Recently, as the Mental Health Welfare Act began to be in effect, the mentally-ill people could decide to return to the community for themselves. (Ministry of Health and Welfare, 2017). In Korea, various mental health projects are operated to help the schizophrenics rehabilitated and returned to the society: daycare hospital, nursing home, mental health welfare center, mental rehabilitation facilities, etc. Among them, the mental rehabilitation facilities are used mostly by the schizophrenics, and thus, they are training and guiding their patients in various ways. (Korea Association of Community Psychiatric Rehabilitation Center, 2018). On the other hand, the demands for the group art therapy increase, because the art therapy is perceived to be an effective psychological group therapy for the mental patients. Namely, the schizophrenics who rely on the art therapy for their treatment increase, while our society is becoming more and more concerned with the art therapy. Thus, this study was aimed at reviewing the effects of the group art therapy on the schizophrenics’ rehabilitation. To this end, this study applied Max van Manen’s method of the analytical phenomenology comprehending the phenomenological experiences. More specifically, the purpose of this study was to understand the meanings and essence of schizophrenics’ experiences of the group art therapy at the mental rehabilitation facilities, and thereby, provide some basic academic data useful to the group art therapy for the schizophrenics, while conducing to the art therapy at the relevant facilities. For such purpose, the study point was set up as follows; “What experiences would the schizophrenics using the mental rehabilitation facilities have? And what meanings and essences would such experiences have?” In order to find the answer to the above study point, this study reviewed Max van Manen’s research method of the analytical phenomenology, and thereupon, proceeded through a total of 11 stages consisting of the 4 activities: concentration in the essence of experience, exploration of the existential experience, analytical phenomenological reflection, and analytical phenomenological writing. The researcher sampled a total of 10 mental patients who had participated in the mental rehabilitation facilities for 4 months at minimum and 9 years at maximum. The period for which the subjects suffered from the schizophrenic was 5 ~ 34 years. 3 were males and 7 were females. One was in 30’s, four in their 40’s and 5 in their 50’s. The researcher conducted in-depth interviews with the subjects for the period from June to November, 2018. The researcher had one or two interviews with each of the subjects. The subjects agreed to the procedures of the study. Thus, they were urged to be engaged in art activities, while the data were being collected. The collection of data stopped when a considerable amount of data had been collected. As a result of applying Max van Manen’s method to analyze the data collected, 9 essential themes and 27 sub-themes could be determined. The 9 essential themes were as follows; “At first, I participated in the group art therapy with little concern, but I would be a positive participant gradually”; “I feel a sense of relief through simple and easy art working, feeling comfortable for therapist’s demonstration and explanation; “Because I am not good at art, I feel a shame, being embarrassed.”; “I feel happy for and attracted to the group art therapy expressing my internal emotion.”; “I feel warm-hearted and grateful, while I feel homogeneous for my group members who are communicating and sympathizing with me.”; “I feel happy for the group art therapy, because I could see myself as positive through the art therapist.”; “I feel that the group art therapy would help the group members to change into positive and bright people.”; “At first, I questioned that the group art therapy would heal me, but I would feel that I love myself more and that it has helped me much”; “At first I feel painful for the symptom and mal-effects of the medicine. But as I could concentrate in the art therapy, I would forget about the depression.” Based on such results and others – the result from analyzing the existential data, analytical phenomenological essays and discussions, this study could make the following 5 conclusions regarding the group art therapy for the schizophrenics; First, the schizophrenics using the mental rehabilitation facilities were obliged to participate in the group art therapy at first, and therefore, they would feel some difficulties for the symptoms of their diseases and the mal-effects of their medication, but as time went on, they would be concentrated in the art therapy. Secondly, the schizophrenics using the mental rehabilitation facilities would feel a shame and embarrassed at first because they were not good at art, and so they would favor a simple and easy art working that they could approach relatively easily. Thirdly, the schizophrenics who uses the mental rehabilitation facilities question the effect of the group art therapy at first, but they would feel that they could better express their internal emotion, being relieved of their stress, and therefore, that the group art therapy had some effects of treatment on their illness. Fourthly, the schizophrenics using the mental rehabilitation facilities would sympathize with their group members and would become more and more confident to change positively. Fifthly, the schizophrenics using the mental rehabilitation facilities would feel good for the art therapists who were concerned with them, trying to understand them, and thereby, would see themselves positively, feeling happy. The above findings of this study would be significant in the following ways; First, in the academic aspects of the art therapy, this study applied Max van Manen’s analytical phenomenological method to the group art therapy, and as a result, it contains the art therapy apporach with vivid languages. Secondly, It was found that art therapist’s positive attitudes and encouragement were essential for the group art therapy. After all, this study would help not only the mental rehabilitation facility workers but also the art therapists to more understand the schizophrenics, being conducive to people’s understanding of the art therapy as well as the conditions for art therapy.
