This study was to investigate the effects of an intervention consisting of music, ear plugs, and eye masks on vital sign, pain, consciousness, and delirium among acute stroke patients in a neurological intensive care unit at a university hospital in Chonbuk. This was a quasi-experimental study with a nonequivalent control group and non-synchronized design. A total of 61 acute stroke patients were recruited and assigned to experimental (n=29) or control (n=32) groups.
The intervention was playing Mozart with headphones at 10 AM for 1 hour and applying ear plugs and eye masks between 11 PM and 5 AM, and continued for 5 days. The effects were assessed four times (before, and after the 1st, 3rd, and 5th intervention) on vital signs including systolic blood pressure, heart rate, and respiration rate using patient monitor, pain with Critical Care Pain Observation Tool, consciousness with Glasgow Coma Scale, and delirium with Confusion Assessment Method for the Intensive Care Unit. Friedman test was used to determine significance of changes in each group on systolic blood pressure, respiration rate, pain, and consciousness. Repeated measures ANOVA and ANCOVA were used to compare changes in two groups on heart rate. Chi-square test was used to compare changes in two groups on delirium.
Significant differences were observed between two groups in the systolic blood pressure (χ2=87.00, p<.001), heart rate (F=108.80, p<.001), respiration rate (χ2=46.41, p<.001), pain (χ2=59.01, p<.001), and delirium (χ2=12.16, p<.001). The findings showed that the mixed music and environment intervention was effectively lowered systolic blood pressure, heart rate, respiration rate, and pain, and decreased the number of patients with delirium. Therefore, a simple and easy intervention such as playing music and applying ear plugs and eye masks can be an useful nursing to help patients’ recovery.
