This study was designed to develop the Reconstructed Balance Scale (RBS) across Pediatric Balance Scale (PBS), Fullerton Advanced Balance Scale (FAB), and Balance Evaluation System Test (BESTest), and to identify its reliability, validity, and clinical use in children with cerebral palsy (CP). A total of 123 normal children were assessed with PBS, FAB, and BESTest, whereas 12 items were extracted using factor analysis and RBS was developed. To identify the RBS applicable to children with CP, 104 children with CP were evaluated using the Korean version of PBS (KPBS) and RBS. The statistical analysis performed conventional statistical methods and the Rasch analysis. Internal consistency of the RBS was found to be at a high level with Cronbach's alpha (0.92), and test–retest reliability was indicated excellent (ICC = 0.96, 95% confidence interval: 0.95 – 0.97). The RBS was shown high correlation with the KPBS (r = 0.91). In the Rasch analysis, the most difficult item was “one leg standing”, and the easiest item was “retrieving object–forward”. In the rating scale, threshold calibration distinguished sufficient distinction between categories after a 5–point scale converted to a 4–point scale by combining categories 2 and 3. The item separation index was 8.70 and the reliability was 0.99. The person separation index was 2.87 and the reliability was 0.89. This result represents that the RBS is appropriate for measuring persons depending on their balance ability. Consequently, the RBS might be suitable for clinical setting as a balance assessment tool in children with CP.
