다양하고 복잡한 의료환경에서 보건의료 전문직간 협력(Interprofessional collaboration)과 커뮤니케이션은 환자 중심 의료에 있어 매우 중요한 요소로 작용한다. 하지만 국내 의료환경에서는 외래환자를 중심으로 한 약물요법에 개입하는 의사와 약사간 협력에 대한 인식 뿐 아니라 커뮤니케이션조차 제대로 이루어지지 않고 있다. 이에 본 연구는 외래환자 중심의 약물요법을 수행하는 두 전문가인 의사와 약사들의 협력에 대한 인식과 현황을 조사하고 향후 두 전문직 간 협력을 증진시킬 수 있는 방안을 모색하고자 하였다.

연구 결과 설문에 응답한 약사는 162명, 의사는 62명이었으며 두 집단에서 차이없이 모두 국내 의료환경에서 외래환자의 약물요법 관련 의사-약사간 협력이 잘 이루어지지 않는다고 생각하는 경향을 보였으며(p>0.05), 현재 이루어지고 있는 협력의 업무 범위에는 환자의 처방약에 대한 점검 결과 및 변경 여부 협의를 위한 의사소통이 대부분(약사 73.2%, 의사 46.7%)이었으며 이마저도 빈도수가 적은 것으로 나타났다. 의사-약사간 협력의 필요성에 대해서는 두 전문가들 모두 인정하고 있었으나 의사보다 약사가 필요성을 더 크게 인식하였다 (p<0.05). 그리고 두 전문가들 모두 약물처방 오류 감소를 협력 필요성 이유로 가장 크게 인식하였다 (약사 29.3%, 의사 43.9%). 최적의 약물요법을 위해 처방의와 처방 조제 약사간 상호공유하길 희망하는 정보(병력, 약력, 복약순응도, 처방수정 또는 변경에 대한 협의, 약물부작용)에 대해서는 약사가 의사보다 정보 공유를 더 희망하는 경향을 보였다 (p<0.05). 상호 협력해야 할 대상으로서 서로에 대한 신뢰도는 의사보다 약사에 대한 신뢰도가 더 낮았다 (p<0.05). 의사-약사간 협력이 잘 이루어지지 않는 이유로는 협업의 필요성에 대한 인식 부족 (약사 20.5%, 의사 18.0%), 협업을 수행할 여건의 미비(약사 16.5%, 의사 29.7%)가 두 집단 모두에서 공통적인 의견으로 제시되었고 원활한 협력을 위한 활성화 방안으로는 구체적이고 실질적인 지침 제공(약사 32.8%, 의사 32.8%), 지역단위 의사-약사 단체간 상호교류 기회 확대(약사 19.5%, 의사 12.9%), 임상현장의 의사-약사 개인간 상호교류 기회 확대(약사 18.8%, 의사 25.0%)로 상호교류할 기회 확대가 두 집단 모두에서 공통적인 의견으로 제시되었다.
본 연구는 외래환자의 약물요법 관련 의사-약사간 협력에 대한 의사와 약사 두 전문가 집단의 인식을 조사한 첫 번째 연구이다. 연구 결과 두 집단 모두 협력의 필요성은 인정하는 경향을 보였지만 협업을 수행할 여건의 미비로 인해 구체적인 협업에 대한 합의점이 없어 두 집단 모두 협업의 필요성에 대한 인식 부족이 협력을 방해하는 요인이라고 답한 것으로 비춰진다. 이는 구체적고 실질적인 지침 제공에 대한 요구가 두 집단에서 크게 나타나는 것에서도 알 수 있다. 또한, 외래환자의 약물요법 관련 의사-약사간 활발한 협력에 대한 구체적 모델 및 제도적 장치가 있는 해외 사례와는 달리 일차의료가 약화된 국내 의료환경에서 의사-약사간 협력 활성화를 위해서는 먼저 두 집단간 상호교류할 기회가 확대되어야 할 것이다. 상호교류 기회 확대로 함께 나아가야할 보건의료인으로서의 인식을 키우고 향후 더 넓은 방향으로 관련 정책 및 제도 개선을 통해 환자중심 의료를 통한 의료서비스 및 약료서비스의 질 향상이 가능해지길 기대한다.

In a diverse and complex medical environment, interprofessional collaboration and communication are critical factors in patient-centered care. However, in Korea's medical environment, not only is there a perception of cooperation between doctors and pharmacists, who are involved in pharmacotherapy centering on outpatient patients, but also communication is not properly performed. The purpose of this study was to investigate the perception and status of cooperation between doctors and pharmacists who are two experts in pharmacotherapy for outpatient patients and to find ways to promote cooperation between the two professionals in the future.
This study was conducted using a survey method and was conducted online for 14 days from May 22 to June 5, 2019 to pharmacists and doctors (including dentists) and offline surveys were also conducted for pharmacists at the Chungnam Pharmacy Education Center on June 2. The questionnaire consisted of two: pharmacists' questionnaire and doctors' questionnaire. Both questionnaires consisted of 21 questions and was divided into four categories: the characteristics of respondents (five questions for pharmacists, five questions for doctors), experience in doctors-pharmacists collaboration regarding pharmacotherapy of outpatients (five questions for pharmacists, five questions for doctors), awareness and attitude about the need for doctors-pharmacists collaboration regarding pharmacotherapy of outpatients (seven questions for pharmacists, seven questions for doctors), and the current status and activation plan of doctors-pharmacists collaboration regarding pharmacotherapy of outpatients (four questions for pharmacists, four questions for doctors).
All the questions in the four categories were analyzed using descriptive statistics such as the frequency and percentage of responses. The differences in perceptions of the need for doctors-pharmacists collaboration according to respondents' characteristics, experiences and current status were analyzed by chi-square test. In the recognition, experience, and the current status of doctors-pharmacists collaboration, the difference between doctors and pharmacists was analyzed as t-test by scoring four-step responses: 'strongly disagree', 'disagree', 'agree', 'strongly agree'. The statistical significance level was set at α=0.05.
According to the study, there were 162 pharmacists and 62 doctors who answered the survey. In both groups, there was a tendency to think that doctors-pharmacists collaboration regarding pharmacotherapy of outpatients did not work well in the domestic medical environmen (p> 0.05). In the scope of the current collaboration, the results of the examination of patients' prescription drugs and the communication for consultation on whether to change them were mostly (73.2% for pharmacists and 46.7% for doctors) and even this was less frequent. Both experts acknowledged the need for doctors-pharmacists collaboration, but pharmacists were more aware of the need than doctors (p<0.05). And both experts were most aware of the decrease in drug prescription errors as a reason for the need for collaboration (pharmacists 29.3% and doctors 43.9%). For information that is desired to be shared between prescribing physicians and dispensing pharmacists for optimal pharmacotherapy (medical history, medication history, adherence, discussion of change to prescription therapy, adverse drug events) pharmacists tended to prefer sharing the information to doctors (p<0.05). The degree of trust in each other as subjects for mutual cooperation was lower in pharmacists than in doctors (p<0.05). Lack of awareness of the need for collaboration (pharmacists 20.5%, doctors 18.0%) and insufficient conditions to conduct collaboration (pharmacists 16.5% and doctors 29.7%) were suggested as common opinions in both groups. The activation measures for smooth cooperation were presented as common opinions in both groups: providing concrete and practical guidance (pharmacists 32.8%, doctors 32.8%), expanding opportunities for interaction between doctors and pharmacists in local area (pharmacists 19.5%, doctors 12.9%), and expanding opportunities for physician-physician individual interaction in clinical sites (pharmacists 18.8%, doctors 25.0%).
This study was the first study to investigate the perception of two expert groups of doctors and pharmacists on doctors-pharmacists collaboration related to outpatient pharmacotherapy. The study found that both groups tend to admit the need for cooperation, but the lack of concrete agreement on cooperation due to insufficient conditions to carry out collaboration suggests that the lack of awareness of the need for collaboration in both groups is the reason why collaboration is not working well. This can be seen from the fact that the demand for the provision of concrete and practical guidance is large in both groups. In addition, unlike overseas cases where there are specific models and institutional devices for active collaboration between doctors and pharmacists in drug therapy for outpatients, the opportunity to promote collaboration between doctors and pharmacists in the domestic medical environment where primary care is weakened will first need to be expanded. We hope that increasing mutual exchange opportunities will increase awareness as healthcare professionals who should move forward together and improve related policies and institutions in a broader direction in the future to improve the quality of medical services and pharmaceutical services through patient-centered care.
