In this study, inter-rater reliability were performed from January 11 to May 31, 2019, and a total of 800 on 400 adult (≧15 years) and 400 pediatric (<15 years) patients who visited the emergency medical center in Incheon, were selected for sampling for convenience. The triages of this study consisted two nurses and two paramedics with clinical career of 7 years or more, including experiences in emergency rooms, and a pair of one nurse and one paramedic performed the KTAS, at the same time, on actual patients. Inter-rater reliability for the KTAS was analyzed with the weighted kappa, by using Medcalc (version 19.0.3) program. The results of this study are as follows. The weighted Kappa value for nurse-paramedic inter-rater reliability for the KTAS performed on adult subjects was 0.71, substantial, and on pediatric subjects was 0.66, also substantial. Discordance Among the results of the KTAS classification, Especially, there were significant differences in the classification of pediatric patients.In the KTAS conducted by nurses the of patients received the Level 1 or 2 was 10 while only two in the KTAS conducted by paramedics. Among the causes of triage discordance, for adults the most frequent was a difference in the measurement of pain intensity (28%). For both adult and pediatric patients, multiple chief complaint were identified as major discordant factors, Especially, there were larger frequencies for pediatric patients. In pediatric patients, the body temperature assessment item by subjective judgments was the highest (50.7%). The study result suggested that the KTAS has been serving a reasonable level of reliability as a standardized triage tool for rational emergency medical services. However, various problems of the KTAS were also revealed: the issue of level discrepance, among nurses and paramedics in pediatric patients, and undertriage or overtriage. Therefore, it recommends to offer the education through the preceptor-ship in order to raise the inter-rater reliability and triage competence of KTAS, revise the current all-integrated KTAS course to be differentiated for each job group, revise the KTAS regarding its pain and body temperature evaluation parts, currently depending on subjective judgement, and strengthen the qualification criteria for its raters.
