Medical 3D printing applications can classify into simulators, surgical guides, and implants. The simulator is the anatomy model of a specific disease based on the medical images. It is used for patient education, preoperative rehearse, and simulate blood flow. The surgical guide is used as a patient-specific medical device for guiding incision, resection, insertion, and marking. As 3D printers can output materials that can be inserted into the human body, the patient-specific implant device that reflects the patient’s anatomy and medical plan could be of relevance. In addition, patient-specific aids, including gibs, splints, prostheses, and epitheses, could be used for a better outcome. Finally, bio-printing is also used to cultivate cells to produce functional artificial tissues. On medical 3D printing, segmentation is based on medical images, which require medical imaging and anatomical knowledge. Besides, expert knowledge of 3D printing technology is needed to select appropriate technologies and materials according to clinical demand. Therefore, it is necessary to establish a workflow that can be applied more effectively and reasonably. This study focused on customized surgical guides, based on these four sub-studies, developed and applied customized surgical guides reflecting clinical demands based on medical images and evaluate their utility and improvements. In the first sub-study, the patient-specific surgical guide for breast-conserving surgery was applied to marking cancer of patients from MRI. It applied to a total of 150 people from 2015 to 2018. Besides, evaluated the accuracy of the surgical guide by in-vitro phantom experiments. The trajectory evaluation conducted by the analysis of the insertion angle with the distance between the entry points and the cosine similarity. As a result of the in-vitro phantom experiment, the average distance between the entry points was 4.856± 3.488mm, and the cosine similarity between the insertion angles was 0.993± 0.014. A total of 150 cases were applied to the clinical application, 11 cases were positive margin, and all other cases were clean margin. In the second sub-study, the patient-specific surgical guide for difficult skin cancer excision surgery was applied for marking cancer of patients. It applied to a total of 30 people from 2018 to recent. Patient-specific surgical guides for incision of skin cancer were designed based on CT and 3D scans of patients. Positive margins were in 3 out of the 16 cases in which pathologic evaluation performed. In the Third sub-study, the Beagle-specific surgical guide for repeatedly injecting stem cells into the hippocampus was applied for positioning the custom device. The repeated administration is necessary to increase the therapeutic efficacy of mesenchymal stem cells (MSC) in the previous phase I clinical studies. To develop a stereotactic surgical guide for placing the accurate trajectory of an intra-brain injector (IBI) to repeatedly inject stem cells on the hippocampus of beagles using 3D printing and MRI images and to evaluate its accuracy of IBI trajectory with the surgical guide. The 3D printed surgical guide was modeled so that IBI could be placed in a planned trajectory. Here, two screws in the sagittal crests and one screw in left parietal bone were inserted to be landmarks for tailored guidance in MRI and operation room. Surgeries for placing IBI devices in seven beagles were performed using these surgical guides. The average distance of entry points and the average of cosine similarity between two trajectories were 1.04 ± 0.56 mm and 0.990 ± 0.001, respectively. Finally, in the fourth sub-study, the mice-specific stereotaxic surgical guide was applied for observing eye of awake mice. The mice-specific stereotaxic surgical guide was designed based on computer-aided design (CAD) models of the mouse head, which were acquired using CT scans of an adult mouse. The mouse grimace scale (MGS) was used to determine the level of pain and discomfort of mice during the donning process. As the bars were tightened, the mice expressed small amounts of discomfort, ranging from 0 to 1 in the orbital tightening and ear position metrics of the MGS, and remained at 0 in the check bulge metric. In addition, the blood vessels of the mouse eyes without anesthesia successfully observed. A patient-and-surgeon-specific surgical guide by 3d printing technology based on the medical image could be marked or cut the tissue like a tumor. Also, another medical device could be positioned or mounted. In conclusion, by defining the elements of clinical and design demand needed to produce customized surgical guides, it is possible to produce easier and more effective surgical guides.
