현대 의학과 과학기술의 발전에도 불구하고 암은 지속적으로 증가하고 있으며 암의 발생은 가족 구성원 전체에 큰 영향을 준다. 장기간의 간병 과정에서 가족들은 심리적, 경제적 부담감과 의사소통의 어려움을 겪으며 역기능적인 가족 관계를 경험하기 때문에 말기암 환자의 간호에서 환자와 가족의 문제를 함께 다루는 것은 매우 중요하다. 본 연구는 호스피스완화의료에 입원한 말기암 환자와 가족이 겪는 다양한 심리 사회적 문제를 살펴보고 미술치료 중재 프로그램을 통해 환자와 가족의 가족 기능과 삶의 질에 어떠한 영향을 주는지 살펴보고자 하였으며 이들의 경험을 Strauss와 Corbin의 근거이론 방법으로 분석하였다. 연구를 위해 서울 소재 시립병원 두 곳의 호스피스완화의료 병동에서 연구참여자를 모집하였고, 직접 연구 참여 동의서를 작성한 말기암 환자와 가족을 한 쌍으로 전체 6가족, 총 17명의 가족이 연구에 참여하였다. 본 연구자는 질적 분석을 위한 자료수집을 위해 환자 또는 가족의 인터뷰 및 미술치료 전체 과정의 녹음, 사진, 메모 등 다양한 자료들을 연구 자료로 사용하였고 녹음된 자료들은 축어록으로 정리하여 분석하였다. 원자료는 근거이론의 연구절차에 따라 개방코딩, 축코딩, 선택코딩으로 분석하였으며, 전체 자료를 분석한 개방코딩의 결과 164개의 개념, 58개의 하위범주, 13개의 범주가 도출되었다. 축코딩 결과로 환자와 가족들은 ‘죽음을 앞두고 가족을 떠나보내기 힘듦’ 이라는 중심 현상을 경험하였는데, 경험의 원인이 되는 인과적 조건으로 ‘암 진단으로 온 가족이 마음고생 함’ , ‘치료보다는 고통 다루기’, ‘내 뜻대로 몸을 움직이기 어려움’, ‘자신의 원래 모습을 잊어버림’ 등 환자의 질병 과정에서 겪었던 어려움을 나타내고 있다. 또 맥락적 조건으로 ‘가족 문제로 속 썩음’, ‘간병도 요령이 필요함’, ‘사람들과 자연스럽게 멀어짐’으로 원 가족의 문제로 여전히 어려움을 겪으며 간병의 부담과 사회적 지지 약화로 인해 가족이 겪는 어려움을 나타내고 있다. 본 연구에서는 말기암 환자와 가족의 심리적 지지와 돌봄을 위한 대처전략으로 미술치료 중재 프로그램을 사용하였다. 이러한 대처전략에 영향을 주는 중재적 조건은 ‘주변의 도움으로 힘을 얻음’이었고 결과로 ‘아프고 나서 가족이 더 가까워짐’, ‘죽음을 바라지 않지만 편안한 죽음을 바람’, ‘순리대로 받아들임’이 나타났다. 호스피스완화의료 환자와 가족의 미술치료 경험에서 도출된 핵심범주는 ‘죽음을 앞두고 가족을 떠나보내기 힘들지만 받아들임’이었다. 본 연구 참여자들은 환자의 질병 경험, 가족의 간병 경험, 사회적 관계, 죽음에 대한 태도 에 따라 ‘부정적 거부형’, ‘소극적 대처형’, ‘적극적 대처형’, ‘지지적 수용형’ 등의 유형으로 나타났다. 또 미술치료 중재 전략을 통해 환자와 가족들은 ‘현실 인식 단계’, ‘현실 조정 단계’, ‘현실 수용 단계’ 등의 단계를 경험하였는데 가족의 질병과 간병의 어려움을 인식하고 주어진 상황에 적응해 나가며 가족과 의미 있는 시간을 보내고 환자의 죽음을 준비하며 받아들이는 과정으로 나타났다. 본 연구는 호스피스완화의료 병동에 입원한 말기암 환자와 가족이 함께 미술치료 작업을 하며 어떻게 호스피스완화의료 과정 안에서 가족 관계와 미술치료를 경험하는지 그 경험의 본질에 대해 고찰하려고 하였으며 이를 바탕으로 호스피스완화의료에서의 미술치료 연구에 대한 이론적인 근거를 마련하고자 하였다.

Although there has been great improvement in the modern medicine and scientific technology, the prevalence of cancer is continuously increasing in tend. Cancer experience significantly influences on the lives of family members. Cancer patients and their family suffer from psychological and economic burdens, have difficulties in communication within members, and undergo dysfunctional family relationship. So it is quintessential to deal with problems of the patient and family at the same time in the course of caring for the terminally ill cancer patients.
In this study the investigator inspected closely various psycho-social problems of the terminal cancer patients and their family in hospice palliative care units. The investigator undertook art therapy intervention programs for the terminal cancer patients and their family, conducted in-depth interview about how the cancer experience have influence on their family function and quality of life, and analyzed their experience using methodology of the grounded theory developed by Anselm Strauss and Juliet Corbin(1998).
The study participants were recruited from two hospice palliative care centers of municipal general hospitals located in Jungnang-gu, Seoul, Republic of Korea. After giving written autonomous consents, 6 terminally ill cancer patients and their family members as pairs, in total 17 persons participated in the art therapy intervention and interview.
The raw data verbatim records were analyzed according to the procedures of the grounded theory: open, axial and selective coding. Through open, axial and selective coding, a total of 164 concepts, 58 subcategories and 13 categories were identified. The contents of categories consists of ‘the whole family suffer from inner-struggle after cancer diagnosis’, ‘dealing with pain rather than treatment’, ‘hard to move body on my way’, ‘forgetting one’s own feature’, ‘difficult to let family member go before death’, ‘struggle with problems of the original family’, ‘care-giving for the sick also needs know-how’, ‘natural distancing from other people’, ‘art therapy for patients and families’, ‘getting encouraged by the assistance of surrounding people’, ‘closer relationship within family members during illness period’, ‘although don’t want death, but want comfortable death’, and ‘acceptance of the law of nature’.
The central phenomenon was ‘acceptance of the death although it is hard to let family member go before death’. Causal conditions were ‘the whole family suffer from inner-struggle after cancer diagnosis’, ‘dealing with pain rather than treatment’, ‘hard to move body on my way’ and ‘forgetting one’s own feature’, all of which were hardships of them during the course of illness. Contextual conditions were ‘struggle with problems of the original family’, ‘care-giving for the sick also needs know-how’ and ‘natural distancing from other people’. They suffered from not only problems of the burden of care-giving and weakening of social support but also problems within the original family.
In this study the investigator utilized art therapy intervention program as a coping strategy for the psychological support and care on the terminally ill cancer patients and their family. They experienced ‘self-expression and emotional refresh’, ‘displaying repressed emotions’, ‘wholly focusing on oneself’, ‘remembrance of the life with family’, ‘leaving family inheritance, and ’spending meaningful moments with family’ during the art therapy sessions. Intervening conditions were ‘getting encouraged by the assistance of surrounding people’. Patients and their family realized that they were taken care of by surrounding people and expressed gratitude to them. As a result of these coping and intervening conditions, they experienced ‘closer relationship within family members during illness period’, ‘although don’t want death, but want comfortable death’, and ‘acceptance of the law of nature’.
Family were classified into four types according to their family function, quality of life and attitude to death: ‘negative rejection types’, ‘passive management type’, ‘active management type’, and ‘supportive acceptance type’. Through the art therapy intervention, patients and their family members experienced three stages over time: ‘reality- awareness stage’, ‘reality-adjustment stage’ and ‘reality-acceptance stage’. They realized the illness of family member and the hardship in care-giving for them at first. It was revealed that they adjusted themselves to the given circumstances, spent meaningful time with their family members, and prepared and accepted the death of the patients according to the stages.
This research focused on the essential nature of the family relationship and art therapy experience of terminal cancer patients and their family through the art therapy intervention in the process of hospice palliative care. Based on this observation, researcher intended to construct the theoretical rationale of the patient and family art therapy intervention in the process of hospice palliative care.
