본 연구는 무용과 소매틱 분야의 연구 성과를 활용하여 치매노인의 심신 건강과 삶의 질을 향상시키기 위한 통합적 표현예술치료 프로그램을 개발하고 그 효과를 검증하는 것을 목적으로 한다.
본 연구는 심신일원론과 인간중심 심리학, 무의식의 의식화에 대한 융의 심층심리학 이론인 ‘적극적 상상’이 이론적 기초를 형성하였다. 본 프로그램의 수행모델에는 Tamalpa 삶-예술과정(TLAP)의 RSVP, PKIP, BPM 방법론, Dance For Dementia(DFD)의 안전한 움직임 방법론과 구조적 형식, Bones for Life(BFL)의 90가지 메소드 중 노인이 수행하기에 안전하고 효과적인 33개 메소드, Playback Theatre의 즉흥 상황극이 선택적으로 적용 통합되었다. 진행자의 역할로서 이들 4개의 메소드를 유기적으로 결합하고 시너지를 내기 위해 본 연구자의 안무 방법론인 중력대응, 중심축 사용, 미러링, Effort 등의 방법론이 중재역할을 하였다.
본 프로그램(BDPT 소매틱 프로그램)의 구성, 내용, 세부 활동의 타당화를 위해 표현예술치료분야의 10명의 전문가 집단에 프로그램 타당도에 대한 설문조사를 실시하여 긍정적인 평가를 받았다.
치매노인의 심신건강과 삶의 질 향상이라는 2개의 목적을 성취하기 위해 각각 2개의 하위가설로 나누어 다음과 같은 4개의 가설을 설정하였다. 1-1. 치매노인의 인지적 기능은 향상될 것인가? 1-2. 치매노인의 신체적 기능은 향상될 것인가? 2-1. 치매노인의 일상생활 수행능력은 향상될 것인가? 2-2. 치매노인의 시설적응 사회성은 향상될 것인가?
본 프로그램의 효과검증을 위해 사전‧사후 실험설계방식을 채택했으며 연구대상자를 ‘BDPT 소매틱 프로그램’ 처치와 기타 신체요법을 함께 수행하는 실험집단과 기타 신체요법만 수행하는 비교집단, 그리고 신체요법에 참여하지 않는 통제집단으로 구분하고, 검증영역을 인지기능, 신체기능, 일상생활 수행능력, 시설적응 사회성 등으로 나누어 사전‧사후검사를 실시하였다. 본 프로그램의 실시기간은 예비프로그램 실시를 포함해서 2019년 6월 28일부터 10월 25일까지 18주 동안 진행하였다.
통계적 효과검증 결과 실험집단은 인지기능 역역에서 통계적으로 유의한 변화를 보였으나 다른 집단들은 통계적 유의 수준에 이르지 못했다. 신체기능과 일상생활 수행능력, 시설적응 사회성 영역에서 실험집단과 비교집단은 통계적으로 유의한 차이가 없었으나 통제집단은 오히려 악화되는 결과가 발견되었다. 반면에 실험집단은 신체기능, 일상생활수행능력 및 시설적응 사회성의 잔존기능 유지에 있어 가장 높은 집단으로 확인되었다. 결과적으로 치매라는 질병은 잔존기능 유지만으로도 효과적인 치료로 본다는 점에서 ‘BDPT 소매틱 프로그램’의 효과는 지지되었다.
또한 피험자(보호자), 조호자를 대상으로 설문조사를 실시하여 ‘BDPT 소매틱 프로그램’의 만족도와 효용성을 검증하였으며 대체로 긍정적인 평가결과에 도달하였다. 본 연구는 일부 제한점에도 불구하고 치매노인에 특화된 통합적 소매틱 표현예술치료의 가능성을 제시하고 다양한 평가도구를 사용하여 측정의 객관성을 확보하였다.

The purpose of this study was to develop a somatic program that is BDPT somatic program from the viewpoint of expressive arts therapy and to verify it's effect on dementia elderly for the function of mind-body and quality of life to be improved after the somatic expressive arts program ministered. For the foundation of theories and principles for this study, body-mind holistic thought, humanitarian psychology, C, Jung's depth psychology, and the expressive arts therapy were explored and applied. For the development of the practical model of the program, methodologies of RSVP, PKIP and BPM of Tamalpa's Life-Art Process(TLAP), Dance For Dementia(DFD) method, Bones for Life(BPL), and Playback Theatre's technique were applied to and integrated with. To verify the program factors specified and integrated, the constructs, contents and activities of the program were tested and consulted by 10 professionals.
To verify the quantitative effect of the program on dementia elderly, in two concepts of mind-body function and daily-life quality related to purpose of study, the following 4 research hypotheses were investigated. 1-1. Would the cognitive function of the elderly be improved? 1-2. Would the physical function of the elderly be improved? 2-1. Would the practical ability of daily living be improved? 2-2. Would the sociality for facility adaptation be improved?
The program was practiced and conducted from Jun. 28th to Oct. 25th including practices of preliminary program. At the start the subject of 23 were divided randomly into 3 groups, experimental group, comparing group, and control group but 5 were dropped out. The final subject of 18 were divided into the 3 groups, experimental group, comparing group and control group composed of. The differences of their means between pre/post-test and among 3 groups verified statistically by paired-t-test.
The instruments used here for the quantitative verification in this study were as follows: They were Korean Version of Mini-Mental State Examination(MMSE-K) and Lowenstein Occupational Therapy Cognitive Assessment(LOTCA) for cognitive function, Huhn's Scale, Braden Scale and General Health Assesment Scale for physical function, Korean Version Activities of Daily Living(K-ADL) and Korean Version Instrumental Activities of Daily Living(K-IADL) for the practical ability of daily life activities, Institutional Adaptation Assessment Ⅰ, Ⅱ for their sociality for facility adaptation.
The results of this study were as follows; First, hypothesis 1-1 was supported meaningfully(p≦0.05) and it was found that the cognitive function of the elderly was improved. Second, hypothesis 1-2 was not supported meaningfully(p≧0.05) and it was found that the physical function of the elderly was not improved and at same time the previous physical function of the elderly has being maintained without any degression. Third, hypothesis 2-1 was not supported meaningfully(p≧0.05). It was found that the physical function of the elderly was not improved and at same time the previous physical function of the elderly has being maintained without any degression. Fourth, hypothesis 2-2 was not supported meaningfully(p≧0.05). Resultingly, it was found that the effect of the program was evaluated positively and promisingly with improvement of their cognition level and maintenance of the physical, daily practical living, and sociality for facility adaptation. Hence it is suggested that development and practice of somatic expressive arts therapy integrated would be complementary to medication for dementia elderly.
