Radotherapy is leading the treatment in various types of cancer. However, surrounding normal tissues are damaged by radiation. In addition, patients often have a relapse after the treatment. Minimizing normal tissue damage, while increasing the killing effect of radiation therapy on tumors, will improve treatment efficacy and patients quality of life. While research on radiation modulators with these effects is essential, existing cell lines and animal-testing techniques have limitations that the results are not always consistent with actual clinical results. An experimental technique to overcome these limitations is an organoid culture system. Organoids are organ analogs that are artificially created in the laboratory, which must have the structural and functional characteristics of the organs they simulate. These organoid culture systems offer the opportunity to create human disease models. In this study, we investigated the fuction of radiation modulator using this organoid culture system. In patients with locally advanced rectal cancer, 5-fluorouracil (5-FU) combination therapy has been reported to improve postoperative survival and reduce recurrence rate. However, there is an urgent need to find new drugs for improving radiosensitivity, because 5-FU can cause serious toxicity to normal tissues. Butyrate, known as one of the short chain fatty acids (SCFAs), is a representative metabolite produced by intestinal microorganisms and is absorbed by colon epithelial cells and used as an energy source. Butyrate has anti-diabetic and anti-inflammatory effects. In particular, studies have shown that butyrate reduces the toxic side effects of conventional anti-cancer drugs and selectively kills cancer cells. Therefore, butyrate is considered as a good candidate for as a radiosensitivity enhancer in rectal cancer. Of the three SCFAs tested, only butyrate suppressed the proliferation of three dimensional-cultured organoids derived from clorectal cancer (CRC) patients, while propionate and acetate had no effect. Moreover, butyrate significantly enhanced radiation-induced cell death and was more effective than either treatment alone. The radiation-butyrate combination reduced the proportion of Ki-67 (proliferation marker)-positive cells and decreased the number of S-phase cells via regulation of Foxo3a transcription activity. Three of eight CRC organoids were non-responsive to butyrate and had lower expression levels of Foxo3a than those in responsive cases. Importantly, butyrate did not increase radiation-induced cell death and improved regeneration capacity after irradiation in normal organoids. These results suggest that butyrate could enhance the efficacy of radiotherapy while protecting the normal mucosa, thus highlighting a potential strategy for minimizing the toxicity of radiotherapy. Exposure to high dose ionizing radiation causes acute gastrointestinal injury and gastrointestinal syndrome. Due to these side effects of radiotherapy, the research on radioprotector is an important field for improving human health. Radiation induced the loss of intestinal stem cells and intestinal injury. Therefore, protection of LGR5 positive stem cells are the major strategy to prevent radiation-induced injury. Here, we identified valproic acid (VPA) as a potent radioprotector using murine intestinal organoid culture systems. VPA treatment improved expression of LGR5 positive stem cells and organoid regeneration after irradiation. A γ- secretase inhibitor blocked the radioprotective effects of VPA, suggesting that NOTCH signaling is the mechanism underlying the radioprotective effects of VPA. On the other hand, VPA acted as a radiosensitizer through the inhibition of histone deacetylase in CRC organoid culture systems. These results demonstrate that VPA is a two-faced molecule acting as a potent radioprotector preventing radiation-induced gastrointestinal inhury or a radiosensitizer in CRC tissues.
