ISO 10993-1(2018)에서 제시하는 원재료 기반의 독성학적 위험평가를 전기수술장치용전극의 생물학적 평가에 도입하기 위한 5단계 절차를 도출하였다. 즉, 생물학적 평가계획, 동등성 비교, 물리적/화학적 독성학적 위험평가, 생물학적 시험(endpoint) 및 생물학적 결론인 5단계 절차를 통해, 국제 조화된 위험관리 프로세스에 따른 전기수술장치용전극의 생물학적 평가방법을 도출하였다. 또한 본 연구를 통하여 기 허가된 전기수술장치용전극의 허가정보 및 임상사용(clinical use)에서 축적된 원재료 물리적/화학적 정보 및 독성정보 등을 바탕으로 신규 전기수술장치용전극의 인체에 미치는 독성학적 영향을 비교 평가할 수 있음을 확인하였다. 이러한 독성학적 영향평가는 기허가 전기수술장치용전극과 신규(신청) 전기수술장치용전극간의 사용목적, 작용원리, 접촉특성, 원재료, 제조방법 및 저장방법의 독성학적 동등성을 우선적으로 확인해야 한다. 이와 같은 독성학적 비교 항목을 평가하고 기 허가제품과 신청제품간의 독성학적 차이점을 독성학적 위험평가(Toxicological risk assessment, TRA) 및 in vitro/in vivo 시험 등을 통해 비교 평가한다. 이와 같은 생물학적 평가계획, 동등성 비교, 물리적/화학적 독성학적 위험평가(TRA), 생물학적 시험(endpoints) 및 생물학적 결론인 5단계 절차를 통해 기 허가제품과 신청제품간의 생물학적 비교평가를 완료할 수 있다. 이를 통해 전기수술장치용전극의 인?허가시에 심사의 전문성 및 공정성을 제공할 수 있는 기초자료로 활용할 수 있을 뿐만 아니라 국내 의료기기의 국제 경쟁력 강화에 기여할 수 있기를 기대한다.

To support biological evaluation of medical devices to Ministry of Food and Drug Safety (MFDS), it is conducted according to the ‘Common Standard on Biological Safety of Medical Devices (MFDS Notification No. 2019-4)’ based on the international standard ISO 10993 series. Recently, ISO 10993-1 was revised in 2018 (5th edition), and it is necessary to revise the MFDS Notification in order to internationally harmonize medical device biological evaluation. In the revised ISO 10993-1 (2018), a toxicological risk assessment with a clinically established(previously predicated) medical device is presented to demonstrate the toxicological equivalence of reducing or exempting unnecessary biological endpoint. In this paper, the biological evaluation process of electrosurgical electrode is presented in detail to support application of ISO 10993-1 (2018). Existing toxicological information based on materials is collected, and biological assessment plan development for toxicity and physical/chemical information gathering are preferentially performed. Compared with the predicated electrosurgical electrode, toxicological evaluation of existing data can be carried out by collecting existing toxicological information (pre-clinical, clinical, post-market surveillance data, etc.) based on materials, and safety and potential toxicological information can be identified using the existing data. For prevent potential hazards, additional physical/chemical analysis is required. Also, toxicological risk assessment of material, and biological tests are performed to demonstrate toxicological equivalence. This study covers the application of ISO 10993-1(2018) for biological evaluation of the electrosurgical electrode, which is a popular approved medical device in MFDS. Material-based toxicological equivalence assessment can be used to identify materials that can cause side effects or injuries from actual human use. In addition, biocompatability evaluation endpoints can be performed or further testing can be determined to reduce material-based risks. In order to identify and cope with hazards of materials with toxic substances, the toxicologist's experience, scientific approach and research methods should be considered together. In other words, this study shows overall biological evaluation process for electrosurgical device through the biological evaluation plan, Substantial Equivalence Discussion, Physical and chemical information for biological risk analysis and Biological testing. The toxicological risk-benefit assessment of material-based toxicological hazards should be carried out to suggest that the biological safety comparison between the predicated device and the proposed product can be completed. The main changes compared to the previous edition(ISO 10993-1(2009)) are as follows : A total of five amendments in ISO 10993-1 (2018) are adding biological endpoints, Providing guidance on risk management process, adding information on nature of human body contact, adding requirements to check the safety of medical devices composition in contact with user’s body and applying ISO 18562 standard. These major changes to ISO 10993-1 (2018) should be reflected in the biological evaluation of electrosurgical electrode. This paper draws on the solutions to the problems that may arise when introducing 10993-1(2018) amendments to change the material, clarify nature of human body contact, and reflect ISO 18562 standard. Among these major changes, the addition of biological endpoints, risk management process and information on nature of clinical practitioner contact are described in the biological evaluation of electrosurgical electrode following the ‘Systemic application process of ISO 10993-1(2018) for biological evaluation of Electrosurgical electrode’ can be performed. The expansion of nature of body contact and application of ISO 18562 standard suggest the following three cases and propose solutions accordingly. The first case is the change of raw materials body contact, which occurs frequently in the design change of electrosurgical electrodes. The second case is the expansion of human indirect contact subjects following the revision of the ‘Categorization by the nature of body contact’ in ISO 10993-1(2018). The third is to propose solutions to situations that may arise when reflecting ISO 18562 standard. The solution to the three examples may vary depending on the purpose of use, technical characteristics and materials of electrosurgical electrode. This study aims to suggest a preemptive approach to the requirements of ISO 10993-1 (2018). In the future, ISO 10993-1 (2018) will be introduced in South Korea, and based on accumulated knowledge such as expertise and related experience, it should be expanded to the evaluation research on high-risk medical devices. Medical device regulatory experts, biomedical engineers and manufacturers in South Korea should make efforts to establish and gradually expand the international level of medical device biological evaluation systems. This study provides fairness and transparency of permit and screening by applying the biological evaluation method according to the risk management proposed in many exciting new devices as well as electrosurgical electrode, and is expected to be used as basic information in domestic and foreign approvals.
