연구 결과는 다음과 같다.
1) 본 연구의 만 2~18세 전체 소아·청소년, 미취학 아동, 초등학생, 중·고등학생에서 부모 비만 여부가 대상자 비만에 영향을 미쳤다(p<0.05). 전체 대상자에서 ‘부모 모두 정상’ 군보다 ‘부모 중 한 명 혹은 모두 비만’ 군에서 비만이 발생할 가능성은 단변량분석, 다중 로지스틱 회귀분석 순으로 4.27배(p<0.001), 2.74배 (p<0.001) 높았다. 연령별로 나누어 단변량 분석 결과, ‘부모 한 명 혹은 모두 비만’ 군에서 비만이 발생할 가능성은 미취학 아동, 초등학생, 중·고등학생 순으로 2.27배(p<0.05), 1.91배(p<0.05), 2.63배(p<0.001) 높았으며 다중 로지스틱 회귀분석 결과에서도 2.82배(p<0.05), 2.15배(p<0.05), 2.45배(p<0.05) 높았다. 2) 만 2~18세 소아·청소년 대상자의 연령이 올라갈수록, 부모 둘 중 한 명 이상이 비만일수록, 부모가 흡연자일수록 비만 여부에 영향을 미쳤다(p<0.05). 단변량 분석 결과, 미취학 아동보다 비만이 될 가능성은 초등학생, 중·고등학생 순으로 2.28배, 3.18배 증가하였으며(p<0.001) 다중 로지스틱 회귀분석 결과에서도 1.87배, 3.16배 증가하였음을 알 수 있었다(p<0.001). 아버지가 흡연을 ‘함’ 군에서 비만이 될 가능성은 단변량분석, 다중 로지스틱 회귀분석 결과 순으로 1.50배(p<0.05), 1.42배 높았고(p<0.05), 어머니가 흡연을 ‘함’ 군에서 비만이 될 가능성은 단변량분석, 다중 로지스틱 회귀분석 결과 순으로 2.02배(p<0.05), 2.06배(p<0.05) 높았다. 3) 초등학생 군에서 부모 비만 여부, 성별, 아침 식사 여부, 어머니 흡연 여부가 비만 발생에 영향을 미쳤다(p<0.05). 단변량분석에서는 성별이 초등학생 대상자 비만 여부에 유의미한 영향을 미치지 않았지만, 다중 로지스틱 회귀분석 결과, ‘남자’ 군보다 ‘여자’ 군에서 비만이 될 가능성은 0.54배 낮았다(p<0.05). 아침 식사를 ‘함’ 군에서 ‘안함’ 군보다 비만이 될 가능성은 단변량분석, 다중 로지스틱 회귀분석 순으로 0.35배(p<0.05), 0.37배 낮아(p<0.05) 일치한 결과를 나타내었다. 어머니가 흡연을 ‘함’ 군에서 그렇지 않을 때보다 비만이 될 가능성은 단변량분석, 다중 로지스틱 회귀분석 순으로 3.36배(p<0.05), 2.72배 높았다(p<0.05). 4) 중·고등학생 군에서 부모 비만 여부, 아버지 흡연 여부 변수가 대상자 비만에 영향을 미쳤다. 아버지가 흡연을 ‘안함’ 군보다 ‘함’ 군에서 비만이 발생할 가능성은 단변량분석, 다중 로지스틱 회귀분석 순으로 2.29배(p<0.05), 2.05배(p<0.05) 높았다.
본 연구를 통해 밝혀진 이상의 연구 결과로 볼 때 소아·청소년 비만에 영향을 미치는 주된 요인은 부모 비만 여부와 연령이었다. 부모의 비만이 자녀의 비만으로 대물림되는 것은 유전적인 요인을 배제할 순 없지만, 부모의 식습관, 생활습관을 어려서부터 그대로 물려받거나 함께 공유할 시간이 많기 때문이라 생각되며 연령이 증가할수록 점차 부모의 손길에서 벗어나 과식, 패스트푸드와 불량식품 위주의 잦은 간식, 불규칙한 아침 식사와 결식 등으로 불량한 식습관이 형성되고 습관화됨으로써 비만 발생에 영향을 주는 것으로 사료된다. 그러므로 연령별 일상생활 패턴에 따른 비만 예방 교육 프로그램을 적절하게 개발하고 기혼 비만 성인의 비만 프로그램에 그들의 가족 또한 참여하여 올바르고 건강한 생활습관과 식습관을 다 함께 공유하고 배우며 자녀의 비만 예방에 관심을 가지고 노력하도록 격려 해야 한다.

This study was conducted to identify factors affecting obesity among children and teenagers between 2 years old and 18 years old in Korea and to help develop effective and practical obesity prevention programs for them. The National Health Nutrition Survey data from 2015, 2016 and 2017 are used as a research tool and analyzed using the SPSS 26.0 version. Frequency analysis and percentage, chi-square test, and logistic regression were analyzed.
The findings are as follows: 1) In this study, obesity from parents affected pediatric youth obesity (p<0.05). The odds ratio in one or both parents are obese group was 4.27 (95% C.I 2.8-6.4) in a univariate analysis and 2.74 (1.8-4.0) in multiple logistic regression. As a result of univariate analysis by age, the odds ratio was 2.27 (1.0-4.7), 1.91 (1.1-3.0), and 2.63 (1.6-4.2) respectively in the order of preschoolers, elementary school students, and middle and high school students (p<0.05). Multiple logistic regression results were 2.82 (1.1-6.6), 2.15 (1.1-4.0), and 2.45 (1.2-5.0) (p<0.05). 2) In the ages between 2-18 years old, the older the children, the more obese the parents were, and the more the parents smoked affected the children’s obesity (p<0.05). As a result of univariate analysis, the odds ratio of obesity pediatric adolescents was 2.28 (1.5-3.4) and 3.18 (2.1-4.7) in elementary school, middle and high school students (p<0.001). Multiple logistic regression analysis showed statistically significant as the odds ratio was 1.87 (1.1-2.9) and 3.16 (2.0-4.9) in elementary school, middle and high school students (p<0.001). When the father was a smoker, the odds ratio were 1.50 (1.0-2.0) and 1.42 (1.0-2.0) in the order of univariate analysis and multiple logistic regression analysis (p<0.05). When the mother was a smoker the odds ratio were 2.02 (1.2-3.4) and 2.06 (1.0-3.9) (p<0.05). 3) Parents’ obesity, gender, breakfast and mother’s smoking affected the obesity of elementary school students (p<0.05). In univariate analysis, gender did not significantly affect obesity in elementary school students, but according to multiple logistic regression, the odds ratio was 0.54 (0.3-0.9) in the female group rather than the male group (p<0.05). The odds ratio for children who ate breakfast was 0.35 (0.1-0.6) and 0.37 (0.1-0.8) in order of univariate analysis and multiple logistic regression analysis. The odds ratio for mothers who smoked was 3.36 (1.6-6.8) and 2.72 (1.0-7.0) (p<0.05). 4) In middle and high school groups, parental obesity and father smoking variables had a significant effect on subject obesity (p<0.05). When the father was a smoker, the odds ratio was 2.29 (1.3-3.8) and 2.05 (1.0-3.9) in order of univariate analysis and multiple logistic regression analysis (p<0.05).
The main factors affecting obesity in children and adolescents were age and parent obesity. Parental obesity can be inherited, but the genetic factors can not be ruled out; however, children have a lot of time to inherit or share their parents' eating habits and lifestyle from childhood. As age increases, overeating, fast food, frequent snacks, and irregular meals and eating are considered to have an effect on obesity. Therefore, according to age, obesity education programs should be appropriately developed according to the daily life patterns, and the whole family should be encouraged to participate in the obesity program so that they can share healthy lifestyles and prevent their children from obesity.
