Benign prostate hyperplasia(BPH) is one of the major urinary disorders of the male elderly. During the process of aging, alteration of major endocrine hormones results in the release of androgen hormones and enlargement of the prostate gland. The enlarged prostates cause lower urinary tract symtoms (LUTS) which show urinary frequency, urgency, dysuria, and nocturia. Even though the exact mechanism of BPH still needs to be investigated, the close relation between BPH and androgen is able to speculate. It is known that the testosterone produced in the testis spreads to the prostate and is converted to dihydrotestosterone(DHT) by 5-alpha-reductase(5AR), an enzyme involved in steroid metabolism, to regulate the development of BPH. DHT binds to AR and serves to make AR function as a transcription factor in the nucleus to stimulate proliferation. Furthermore, chronic inflammation and cellular senescence are also known to participate in the pathogenesis of BPH. Current medications of BPH exhibit side effects such as orthostatic hypotension, ejaculation changes, headaches, and ejaculatory dysfunction. Therefore, attention on herbal medicine on BPH and LUTS with less adverse effects is growing bigger. Schisandrae Fructus (SF) is the ripe fruits of a perennial herb of family Magnoliaceae which is used as an adaptogen for increasing resistance to disease and stress, increasing energy, and increasing physical performance and endurance in Traditional Korean Medicine. In this study, the author investigated the effects of SF using a BPH-like in vitro model of RWPE-1 cells. TP treatment in RWPE-1 cells effectively induced proliferation of RWPE-1 cells, and SF treatment attenuated their proliferation in a concentration-dependent manner. Furthermore, SF significantly suppressed the TP-induced androgen-related factors such as 5AR, AR and ERα, and also indicator of cell proliferation and apoptosis factors. Moreover, the cellular senescence factor senescence-associated βgalactosidase were suppressed by SF treatment, and inflammatory factors including nuclear factor kappa B and cyclooxygenase-2 were as well. In the present study, the author clearly showed that SF extract effectively suppressed BPH factors in a cell model of BPH. These results propose a scientific basis for the usage of SF for BPH in Korean Medicine, and also suggest SF as a useful therapeutic drug for BPH.
