Objective: The purpose of this study was to identify the factors that affect the development of dental caries, an important chronic disease, in providing essential data for the establishment of prevention programs and eventually to improve people's quality of life through promoting public oral health.
Methods: From the research data of the 2013 Korean National Examination Health and Nutrition survey, 5,439 adults older than age 19 who responded to dental examinations were selected as subjects of a total of 8,018 persons. The Chi-square test was conducted to evaluate the relation between dental caries and other variables such as socio-economic characteristics, oral health management method, and frequency of food intake. The amount of nutrient intake was analysed by t-test. Finally, logistic regression analysis was conducted with the variables revealed as statistically significant in the preceding tests. All the analyses were executed using the SPSS version 21.0 (SPSS Inc., Chicago, IL, USA). The level of significance was set at 5% (α=0.05).
Results: Among socioeconomic factors, sex, age, education level, household income, residential area, and occupation were the variables affecting dental caries. The prevalence of dental caries was higher in males (p=0.000), younger age groups (p= 0.002), lower education level (p=0.008), households with low monthly income (p=0.003), rural residents (p=0.001), and blue collar (p=0.000). The prevalence of dental caries was different depending on the management of oral health. People who didn't undergo oral examinations in the last year showed higher prevalence than people who did (p=0.000). The prevalence of people who didn't brush teeth after each meal was 37.1%, the highest among the four group(p=0.002) and the prevalence of people who didn't use dental floss or an interdental brush were higher than those who used oral hygiene products (p=0.000). Vitamin C was the only nutrient that showed a statistically significant difference between groups with, and without dental caries. The mean vitamin C intake of people with dental caries was 90.69±108.65 mg and it was less than 103.83±119.59 mg, mean vitamin C intake of people without dental caries (p=0.000). The carbohydrate intake didn't show a statistically significant difference following the presence of dental caries, although the carbohydrate intake of people with dental caries was larger than people without dental caries (p=0.073). The food intake frequency also affected dental caries and 28 foods were statistically significant of a total of 112 food items. The groups with higher intake frequency of rice, instant noodle, jajangmyeon, pork belly, ham, fruit juice, carbonated drinks, coffee and soju had higher prevalence ratio of dental caries. The higher intake frequency of multigrain rice, potato, sweet potato, vegetables such as cucumber, daikon, broccoli, fruits, anchovy, mushroom, peanut, and semisolid yogurt resulted in lower prevalence ratio of dental caries in reverse.
Conclusion: Dental caries prevalence was affected by sex, age, education level, household income, residential area, and occupation. The oral health management method also affected dental caries. People who didn't brush teeth after meals or didn't use dental floss and an interdental brush had higher prevalence. Lower vitamin C intake and higher carbohydrate intake had a tendency to increase the dental caries prevalence ratio. The types of food and intake frequency were other important factors that affected the prevalence of dental caries. However, different cooking methods altered the effect of food on dental caries even though the ingredients were the same.
