본 연구의 목적은 학대피해 아동의 심리사회적 적응을 위한 위기개입 부모-자녀 인지행동놀이치료 프로그램을 개발하고 위기개입 부모-자녀 인지행동놀이치료 프로그램을 실시한 실험집단과 미술치료 프로그램을 실시한 비교집단, 아무것도 실시하지 않은 통제집단과의 비교연구를 통해 효과성을 검증하는데 있다. 이를 위해 위기개입 부모-자녀 인지행동놀이치료 프로그램을 개발하고자 학대피해 아동과 가해 부모의 심층 면접을 토대로 예비 프로그램을 개발하여 실시 후 수정, 보완하였고 최종적으로 위기개입 부모-자녀 인지행동놀이치료 프로그램을 구성하였으며 구성 요인은 다음과 같다. 첫째, 위기개입 요소로 위기개입계약, 위기개입점검, 대안탐색, 행동계획, 위기재평가를 구성하였다. 둘째, 학대피해 아동이 갖고 있는 인지적 요인에 대한 자동적인 사고를 찾아 대안적 사고로 수정할 수 있는 놀이를 활용한 인지적 재구성을 연령에 적합하게 놀이를 활용하여 구성하였다. 셋째, 외상 후 증상 감소를 위해 전제되어야 하는 아동학대 상황에 대한 놀이를 활용한 노출훈련과 노출로 인한 불안 및 과각성을 다루는데 필수적인 놀이를 활용한 이완훈련을 포함하였다. 넷째, 자기표현 및 문제해결에 어려움을 보이는 학대피해 아동들의 특성을 고려하여 자기주장기술과 대처기술훈련을 포함한 사회기술훈련을 포함하였다. 다섯째, 가해 부모 요인으로 양육태도, 의사소통기술, 정서조절기술을 포함한 부모-자녀 공동회기를 구성요인으로 포함하였다. 본 연구의 대상자 선정은 서울, 경기, 인천 지역에 거주하고 최근 1년 내에 부모로부터 신체 및 정서적 중복학대를 경험하였으며, 학대 이후 시설에 분리되지 않고 가해 부모와 함께 가정에서 거주하는 만 7-11세 아동과 가해 부모를 대상으로, 임상심리사의 면접을 통해 외상 후 증상척도의 기준점에 충족하는 최종 24명을 선정하였다. 선별된 대상자들은 위기개입 부모-자녀 인지행동놀이치료 프로그램인 실험집단에 학대피해 아동과 가해 부모가 각각 8명, 미술치료 프로그램인 비교집단에는 학대피해 아동이 8명, 통제집단에도 학대피해 아동 8명을 동질성을 확보한 뒤 배정되었으며, 실험집단과 미술치료 프로그램은 각각 주 1회, 회기당 60분씩 총 14회기를 실시하였다. 프로그램의 전후 변화를 측정하기 위해 외상 후 증상(아동용, 부모용), 학대피해 아동의 인지적 변인(적대적 의도귀인 척도, 자아존중감 척도), 정서적 변인(우울 척도, 불안 척도), 행동적 변인(공격성 척도, 스트레스 대처행동 척도), 가해 부모 변인(부모양육태도검사, 부모 자녀간 의사소통 유형 척도, 정서조절곤란 척도)으로 나누어 사전-사후에 질문지를 배부하여 수집하였다. 수집된 자료 분석은 SPSS 18.0 통계 프로그램을 사용하였으며 구체적인 분석 방법은 다음과 같다. 첫째, 실험집단, 비교집단 및 통제집단 간 사전-사후검정 점수의 차이를 알아보기 위해 비모수 검정인 Kruskal-Wallis test를 실시하였고, Bonferroni correction을 통해 사후 검정을 실시하였다. 둘째, 실험집단, 비교집단 및 통제집단 내 각 변인의 사전-사후검사 점수의 차이와 실험집단 및 비교집단 내 각 변인의 사전-추후검사 점수의 차이를 알아보기 위해 비모수 검정인 Wilcoxon signed-rank test를 실시하였다. 셋째, 실험집단 및 비교집단 내 각 변인의 집단 간 추후검사 차이를 알아보기 위해 비모수 검정인 Mann-Whitey U test를 실시하였다. 넷째, 실험집단 및 비교집단 내 각 변인의 사전-사후-추후검사 점수의 변화를 알아보기 위해 비모수 검정인 Friedman test를 실시하였다. 본 연구의 주요결과는 다음과 같다. 첫째, 프로그램 사전-사후검사에서 아동용 외상 후 증상은 위기개입 부모-자녀 인지행동놀이치료 프로그램을 실시한 실험집단만 변화를 보였다. 무엇보다 프로그램의 효과성이 유지되는지 검증하고자 프로그램 종료 1개월 후 실시한 추후검사에서도 실험집단만이 변화를 유지하고 있는 것으로 나타났다. 반면 부모용 외상 후 증상은 위기개입 부모-자녀 인지행동놀이치료 프로그램을 실시한 실험집단과 미술치료를 실시한 비교집단 모두 변화를 보이지 않았다. 둘째, 인지적 변인을 알아보기 위한 적대적 의도귀인과 자아존중감에서 불안에서 위기개입 부모-자녀 인지행동놀이치료 프로그램만 변화를 보였다. 또한 프로그램 종료 1개월 후 실시한 추후검사에서도 실험집단만 변화를 유지하고 있는 것으로 나타났다. 셋째, 정서적 변인을 알아보기 위한 우울과 불안 모두 위기개입 부모-자녀 인지행동놀이치료 프로그램만 변화를 보였다. 넷째, 행동적 변인을 알아보기 위한 공격성과 스트레스 대처행동은 두 집단 모두 변화를 보였으나 신체적 공격성은 위기개입 부모-자녀 인지행동놀이치료 집단만 유의미한 변화를 보이는 것으로 나타났다. 프로그램 종료 1개월 후 실시한 추후검사에서도 위기개입 부모-자녀 인지행동놀이치료 집단이 스트레스 대처행동의 변화를 유지하고 있는 것으로 나타났다. 다섯째, 가해부모 요인을 알아보기 위한 부모양육태도, 부모 자녀간 의사소통유형, 정서조절곤란에서 위기개입 부모-자녀 인지행동놀이치료 집단이 유의미한 변화를 보이는 것으로 나타났다. 프로그램 종료 1개월 후 실시한 추후검사에서는 위기개입 부모-자녀 인지행동놀이치료가 부모양육태도, 부모 자녀간 의사소통 유형과 정서조절곤란의 변화를 유지하고 있는 것으로 나타났다. 다만 정서조절곤란의 하위 요인인 충동통제 곤란은 두 집단 모두 변화를 보였으며, 프로그램 종료 1개월 후 실시한 추후검사에서 두 집단 모두 충동통제 곤란의 변화를 유지하고 있는 것으로 나타났다. 본 연구의 의의는 다음과 같다. 첫째, 아동학대에 대한 조기개입 활성화를 위해 ‘위기개입’과 ‘사례관리’ 그리고 ‘사후관리의 유기적 연계’를 통합한 프로그램이라는 점에서 의의가 있다. 한국형사정책연구원이 발표한 ‘아동학대 발생현황과 특성’에서 알 수 있듯이 아동학대에 대한 사회 전체의 감응도가 과거에 비해 높아짐에 따라 학대 가정에 대한 여러가지 개입 서비스가 이루어지고 있음에도 불구하고, 아동보호 실무면에서는 미흡한 점이 많다. 본 프로그램의 경우 학대 시기에 대한 조기개입과 지속적인 안전점검, 그리고 행동계획 등을 통한 아동학대 조기개입에 의의가 있다. 둘째, 학대피해 아동의 심리사회적 적응을 위한 위기개입 부모-자녀 인지행동놀이치료 프로그램의 효과성을 검증한 연구라는 점에서 의의가 있다. 기존 아동의 PTSD 증상에 대한 인지행동치료로는 장기적 개입인 TF-CBT를 사용한 일부만이 보고되고 있으며, 특히 아동이라는 발달적 특성을 고려하여 놀이를 활용한 인지행동놀이치료 프로그램은 찾아보기 어렵다. 따라서 본 연구는 인지행동놀이치료 프로그램과 동시에 가해 부모를 대상으로 하는 부모회기와 더불어 부모-자녀 공동회기를 병행함으로써 가해 부모의 가해 행동과 관련된 사고를 재구성하고, 적응적인 양육기술을 교육함으로써 궁극적으로 재학대를 예방하고, 장기적인 개입이 필요한 사례를 변별하는 목적으로 하는데 의의가 있다. 셋째, 학대피해 아동을 대상으로 한 대부분의 선행연구에서 아동학대와 관련하여 중요한 정보인 학대 최초 발생시기, 학대 유형, 학대 빈도 등 학대와 관련된 정보가 누락되었다는 한계가 있었으나, 본 연구는 학대 가정에 대한 구체적인 정보를 토대로 체계적이인 개입 방법을 마련하였다는데 의의가 있다. 넷째, 기존 아동 PTSD 증상에 대하여 외상에 대한 정서에 중점을 두었다면, 본 연구는 학대피해 아동의 외상 후 증상과 더불어 학대 경험으로 인한 왜곡된 사고나 귀인 편향뿐만 아니라 정서, 행동적인 요인에 대하여 모든 구성요인에 놀이를 활용함으로써 인지행동놀이치료를 아동에게 효과적으로 개입하였다는데 의의가 있다. 다섯째, 학대피해 아동과 가해 부모를 대상으로 한 심층 면접을 통해 이들의 심리 사회적 어려움을 파악하고, 이를 토대로 프로그램을 구성함으로써 학대피해 아동 및 가해 부모의 특성에 대한 이해를 돕고 구체적인 개입방법을 마련하였다는데 의의가 있다.

The purpose of this study was to develop a parent-child cognitive behavioral play therapy program with intervention into crisis for the psychosocial adaptation of abused children, and is to verify effectiveness through comparative studies of an experimental group that implemented a parent-child cognitive behavioral play therapy program into crisis intervention, a comparative group with art therapy program, and a control group that have not conducted anything. To this end, to develop a parent-child cognitive behavioral play therapy program into crisis intervention, a preliminary program was built, implemented, corrected and supplemented based on in-depth interviews of abused children and their parents, and a parent-child cognitive behavioral play therapy program into crisis intervention was finally organized, and the composition factors are as follows. First, the factors of crisis intervention crisis intervention contracts, crisis intervention check, alternative exploration, action plan, and crisis reassessment are composed. Second, cognitive reconstruction using play that can find automatic thinking about cognitive factors of abused children and correct to alternative thinking is constructed appropriately for their age. Third, exposure training using play on child abuse situations that should be a prerequisite for reducing symptoms after trauma and relaxation training using play that is essential to deal with anxiety and hypersensitivity caused by exposure are included. Fourth, social skills training, including self-assertive skills and coping skills training, is included in consideration of the characteristics of abused children who had difficulty in self-expression and problem solving. Fifth, as a contributing parents factor, the parent-child joint session including parenting attitude, communication skills, and emotional control skills was involved as one of the composition factors. The subjects selected for this study were 7-11 years old who lived in Seoul, Gyeonggi, and Incheon, experienced physical and emotional overlapping abuse from their parents within the past year, and live in their homes with their parents without being separated from their facilities after abuse from their parents. The final Twenty four patients who met the criterion of the post-traumatic symptom scale were selected through interviews with a clinical psychologist. The selected subjects were eight children abused and eight abusing parents in the experimental group, a parent-child cognitive behavioral play therapy program into crisis intervention, eight children abused in a comparison group, an art therapy program, and eight children abused in a control group. The subjects were assigned after securing the homogeneity, and the experimental group program and the art therapy program were conducted once a week, 60 minutes per session, respectively, and 14 times in total. Post-traumatic symptoms (for children, parents), cognitive variables (hostile intention attribution scale, self-esteem scale), emotional variables (depression scale, anxiety scale), and behavioral variables (to measure the pre- and post-traumatic changes in the program) Aggression scales, stress coping behavior scales), and assault parent variables (parental attitude test, parent-child communication type scale, emotional control difficulty scale) were collected by pre-post questionnaire distribution. SPSS 18.0 statistical program was used to analyze the collected data, and the specific analysis method is as follows. First, the Kruskal-Wallis test, a nonparametric test, was conducted to examine the differences in the pre-post test scores between the experimental group, the comparative group, and the control group, and the post-test was performed through Bonferroni correction. Second, the Wilcoxon signed-rank test, a nonparametric test, was used to determine the difference between the pre-test scores of each variable in the experimental group, the comparison group, and the control group and the pre-test scores of each variable in the experimental group and the comparison group. Was conducted. Third, a nonparametric test, the Mann-Whitey U test, was conducted to investigate the differences in the follow-up tests between groups of each variable in the experimental and comparative groups. The main results of this study are as follows. First, in the pre- and post-tests of the program, the symptoms of trauma for children showed only changes in the experimental group that had undergone a crisis intervention parent-child cognitive behavior play therapy program. Above all, it was found that only the experimental group maintained the change in the follow-up examination conducted one month after the end of the program to verify that the effectiveness of the program was maintained. On the other hand, post-traumatic symptoms for parents did not show any change in the experimental group with the parent-child cognitive behavioral play therapy intervention program and the comparative group with art therapy. Second, only the parent-child cognitive behavioral play therapy program was involved in crisis from anxiety to hostile intention attribution and self-esteem to identify cognitive variables. In addition, it was found that only the experimental group maintained the change in subsequent tests conducted one month after the end of the program. Third, both depression and anxiety to find out the emotional variables showed only changes in parent-child cognitive behavioral play therapy intervention programs. Fourth, aggression and stress coping behaviors to identify behavioral variables showed a change in both groups, but physical aggression showed a significant change only in the parent-child cognitive behavioral play therapy intervention group. A follow-up examination conducted one month after the end of the program also showed that the parent-child cognitive behavioral play therapy intervention group in crisis maintained a change in stress coping behavior. Fifth, the parent-child cognitive behavioral play therapy group showed a significant change in parental attitude, parental communication type, and emotional control difficulties in order to investigate the causes of abused parents.
