여름철 고온이 우리나라 지역사회 성인의 걷기 실천에 미치는 영향에 대한 다수준 분석 : 지역사회건강조사 자료를 활용한 단면 연구 A multilevel analysis of effect of high temperature on walking of community-dwelling adults in South Korea during summer : a cross-sectional study using data from the Korean community health survey원문보기

The Korean Community Health Survey (KCHS) conducted nationwide by the Korea Centers for Disease Control and Prevention examines the level of physical activity of community residents. In the KCHS, the walking practice index is an important indicator, because walking has many positive health effects, such as lowering high blood pressure, diabetes, and obesity, which are risk factors for cardiovascular disease, and reducing overall mortality. Since the KCHS is conducted every August to October, weather conditions such as high temperatures and heavy rains in summer may affect the rate of walking practice. In this paper, we tried to identify the effect of high temperature on walking practice of community-dwelling adults by utilizing the KCHS data. This paper used data from 18 cities and counties in Gyeongsangnam-do among the 2018 KCHS data. In the KCHS, the health level of community residents is not only affected by individual characteristics, but also by the characteristics of the area to which the individual belongs. Thus, in this paper, a multilevel logistic regression analysis was conducted that could simultaneously consider variables of two levels, the individual level and the neighborhood level, which affect local residents‘ walking. In the multilevel logistic regression model, the dependent variable was defined as walking more than 30 minutes a day and more than five days a week, over the past week from the date of the survey. The independent variable was the daily maximum temperature (degrees Celcius), and the average daily maximum temperature of the relevant Eup·Myeon·Dong area was used for the previous seven days from the date of the survey. The covariate variables were corrected using individual level variables such as sex, age, educational attainment, employment, driving a car, marital status, subjective health, depression, meeting neighbors, meeting friends, traffic culture index, and proportion of road pavement. Of the 17,750 surveyed, 6,359 (35.8%) persons practiced walking. Odds Ratio (OR)=0.97 (95% Confidence Interval (CI)=0.96-0.98) of walking practice when the daily maximum temperature was increased by 1 degrees Celcius. In the subgroup analysis, OR=0.98 (95% CI=0.96-0.99) for men and OR=0.97 (95% CI=0.95-0.98) for women. In the participants under 65 years old, OR=0.98 (95% CI=0.97-0.99), while OR=0.96(95% CI=0.94- 0.97) for the participants over 65 years old. Calculating ORs for sex and age, men under 65 years old showed no statistically significant results with OR=0.99 (95% CI=0.97-1.01). However, OR=0.97 (95% CI=0.96-0.99) for women under 65 years old, OR=0.96 (95% CI=0.93-0.98) for men over 65 years old, and OR=0.95 (95% CI=0.93-0.97) for women over 65 years old. In addition, OR=0.98 (95% CI=0.97-1.00) in urban and suburban areas, and OR=0.95 (95% CI=0.94-0.97) in rural areas, with lower ORs in rural areas than in urban and suburban areas. In conclusion, high temperatures were negatively affecting the walking practice of community-dwelling adults in summer. It is necessary to prevent poor physical activity during high temperatures and heat waves, and also to consider ways to reduce the effects of high temperatures when developing health projects and policies to improve the level of physical activity in the community.
