Mental illness stigma is defined as negative thoughts, attitudes, and behaviors about mental illness that results in disadvantages to people with mental illness(PwMI). Mental illness stigma results in explicit and implicit discrimination toward PwMI and is a major obstacle to treatment and recovery. Especially, there is a significant need for research on the mental illness stigma of law enforcement officers who are the first responders to mental illness crisis, as their misconception and discrimination can directly lead to violation of human rights. This study sought to comprehensively examine the characteristics of mental illness stigma of Korean police by exploring the knowledge, attitude, and behavioral aspects of mental illness stigma in active-duty police officers and police trainees. In addition, previous contact experience with PwMI was collected to investigate the relationship between retrospective contact and mental illness stigma. 77 active duty police officers with practical experience and 55 police trainees with no practical experience were recruited for this study. Mental Health Knowledge Schedule(MHKS) was used to assess the knowledge aspect of mental illness stigma. Community Attitudes toward the Mentally Ill(CAMI) and Prejudice to the Mentally Ill Scale was used to assess the attitudinal aspect. Reported and Intended Behavior Scale(RIBS) and Social Distance Scale(SDS) was used to assess the behavioral aspect of mental illness stigma. Additionally, to control social desirability, which is a common confounding variable problematic in stigma research, Social Desirability Scale 24(SDS-24) score was used as covariance. Lastly, personal contact experience and workplace encounters with PwMI were collected. The result showed a significant difference in the behavioral aspect of mental illness stigma between the two groups. Active-duty group obtained higher scores on RIBS and SDS compared to trainee group. Furthermore, different attitudinal variables affected the discriminatory behavior of each group. Among the subfactors of attitudinal measures, incompetence, community mental health ideology, and social restriction were found to have significant effects on behavioral variables in police trainee group, while identifiableness, perceived coping ability regarding mental illness crisis, potential of recovery, and social restriction were found to have significant effects on behaviors of active-duty police group. Lastly, significant differences in the correlation coefficient between the two groups were found in identifiableness and potential of recovery variables. These findings suggest that perceived risk, which is an attitudinal factor of mental illness stigma commonly found to be higher in police groups compared to the general population, can lead to actual discriminatory behavior by increasing social distance. It also suggests the need for a differentiated approach based on the different characteristics of stigma in forming anti-stigma intervention in police and police trainee group. On the other hand, retrospective contact did not indicate a significant correlation with mental illness stigma in both personal contact experience and workplace encounter. These results suggest a number of possibilities, one being that the nature of contact may need to be considered more thoroughly when studying the relation between previous contact experience and mental illness stigma in police groups. Alternatively, there is a possibility that in police group, contact may not have significant impact on the level of stigma. Subsequent studies are needed to explore these possibilities. This study was the first research to comprehensively examine knowledge, attitude, and behavioral aspect of mental illness stigma in Korean active-duty police and police trainees. Significant differences were found in the behavioral aspect of mental illness stigma, and the attitudinal aspects of mental illness stigma affecting these behavioral aspects were identified. The effect of personal contact and workplace encounters with PwMI on mental illness stigma was also examined. Future studies need to focus on investigating the defining characteristics of mental illness stigma of police compared to the general population and establishing mental illness stigma programs based on these characteristics to specifically target police mental illness stigma.
