Objective : Depressive disorder is syndrome that includes depressed mood, loss of interest or pleasure, insomnia, weight loss or gain, diminished ability to think or concentrating, considering of suicide and so on. Shihosogan-san is herbal medicine that used for flank pain. However, its usage has widened as its efficacy nearly proven as a antidepressant. Nevertheless, its mechanism is veiled, so aim of this report is to investigate mechanism of anti-depressant effects of shihosogan-san by systemic research within ten years.
Method : Searches were conducted Oasis, Riss, Kiss, Naver scholar information, Pubmed, EMBASE and CENTRAL. Results were screened before selection, selected articles were assessed, and identified by using key criteria.
Result : Eighteen articles including thirteen in vivo researches, two patients-related researched, and three reviews were located. As the anti-depressant effect of shihosogan-san is greater than placebo and greater than or similar to fluoxetine, but most of reports has not shown qualitatively high level, more randomized controlled trials which were well-designed and well-reported are required to demonstrate confident anti-depressant effect of shihosogan-san. Meranzin hydrate extracted from fructus aurantii plays a key role of shihosogan-san's anti-depressant effect. In terms of metabolism, the antidepressant effects of shihosogan-san mainly occur in the hippocampus, frontal lobe, and temporal lobe. Up-regulating expressions of MAPK14, Gria3 ERK1, ERK2, ERK5, miR-125a, BDNF and TrkB mRNA, down-regulating expressions of miR-182, JNK, and miR-124 and improving regional cerebral blood flow were shihosogan-san's anti-depressant mechanism. Sihosogan-san's anti-depressant mechanism was related to tryptophan metabolism, energy metabolism, calcium metabolism, and liver detoxification.
Conclusion : The mechanism of antidepressant effects of shihosogan-san has obtained meaningful results related to the mechanism of action, herb and sites, but is not fully understood. Better-designed and well-reported studies will be needed to understand and investigate the more accurate mechanism of the anti-depressant effect of shihosogan-san.
