Methods: Seventy-eight patients who underwent surgery for pancreatic ductal adenocarcinoma (PDAC) at the Soonchunhyang University Hospital Bucheon during 2013-2019 were selected. Information on the clinicopathologic factors was obtained from the electrical medical records and by reviewing the slides. For survival data, progression-free survival (PFS) and overall survival (OS) were calculated. Immunohistochemical (IHC) staining for PD-L1 was performed using the Ventana SP263 (rabbit monoclonal primary anti-PD-L1 antibody; Ventana Medical Systems, Tucson, AZ, USA) assay. PD-L1 staining was evaluated for percentage positivity of the tumor cells with membrane staining, which is called the tumor proportion score (TPS). Results: More than half (56.4%) of the cases were completely negative for PD-L1 staining. When cut-offs of 10% and 30% were applied, 12 cases (15.4%) and 3 cases (3.9%) were positive, respectively. In this study, cases with PD-L1 TPS≥30% were defined as having PD-L1 overexpression. Decreased OS was seen in the PD-L1 overexpressed group, which was significant (p<0.0005). In addition, tumors in the PD-L1 overexpressed group had more LVI (p=0.030) than those in the PD-L1 non-overexpressed group. Tumors in the PD-L1 TPS≥10% group had worse differentiation (p=0.046) and more duodenal/stomach wall invasion (p=0.041) than those in the PD-L1 TPS<10% group. Conclusion: PD-L1 overexpression correlated with worse overall survival of PDAC patients in this study. Although several issues remain unsolved in studies on PD-L1 expression in PDAC, this study is the first study in Korea to report the relationship between PD-L1 overexpression and patient survival and clinicopathologic factors.
