Objective: To assess the effects of antibiotics treatment in patients of preterm labor (PTL) and intact membranes with amniotic fluid (AF) “sludge”. Methods: This was a retrospective cohort study in women at 20-32 weeks gestation in PTL with intact membranes. 45 women with AF “sludge” were identified and received an antibiotic regimen of ceftriaxone, clarithromycin, and metronidazole. According to the alteration of AF “sludge”, patients were divided into “sludge” disappearance (group A, n=21)and “sludge” persistence (group B, n=24 ). Results: Group A was delivered later than group B, ultrasonographic examination-to-delivery interval was longer than group B,and group A had a smaller initial size of “sludge” than group B (all P< 0.05). Group A had a lower rate of preterm birth within 7 days, and before 28, 32, 34 weeks of gestation than group B (all P< 0.05). Group A had a lower rate of perinatal death and the birthweight was heavier than group B (all P< 0.05). Conclusion: Antibiotics administration may eradicate AF “sludge” in patients of PTL and intact membranes, which are associated with the initial size of “sludge”. The “sludge” disappearance patients had favorable pregnancy and neonatal outcomes than “sludge” persistence patients.
